A word about 'cardiac health check-ups'.

Q. Nowadays many hospitals, clinics and path labs have come up with a variety of health check-ups especially cardiac health check-ups. Many corporates doing annual check-ups. So how useful are they, who should get it done and when?

Ans. Doing a variety of unnecessary investigations in healthy individuals without any symptoms is a big no. It should not be done and are totally useless.

Q. 2. What tests do you say are useless as part of cardiac health checkup in apparently healthy individuals.

Ans. Ecg, echo, TMT, stress test, ct angio should not be done at all, do not add any value to your overall health status.

Q3. Why do you say these tests are useless.
Ans. They are useless because many trials around the world over last 20 years have proven beyond doubt, that even if you have underlying blockages in heart, which does not cause any symptoms, then treating them will neither prevent you from a future heart attack nor prolong your life.
It is also possible that today you have ECG, echo normal and still, you develop a heart attack tomorrow. So these tests give a false sense of security which has no value actually.
Also due to this false security, such people with normal reports have a tendency to continue their unhealthy lifestyle

Q4. What are the tests you say are useful?
Ans. Know your blood pressure, blood sugar, lipid profile (cholesterol, LDL, triglyceride and HDL) and body mass index (BMI) as part of your routine health check-up. That's it.
In lipid profile, there is no use of getting unnecessary tests like lp (a), homocysteine, apo (b) and a variety of other scary variables that you commonly see in the report.

Note - in a specific subset of people who have a strong family history of cardiac disease or sudden cardiac death may undergo ECG, echo, TMT as part of routine evaluation

Lastly and most importantly no health check-up can replace a healthy lifestyle and exercise. It is the surest way of avoiding heart ailments.

Arthritis mutilans - an extremely severe form of chronic rheumatoid arthritis, in which the bones are reabsorbed, resulting in the collapse of soft tissue. When the hands are affected it can cause a phenomenon called telescoping fingers; the feet may also be affected. Patients with other arthritic conditions, such as osteoarthritis or rheumatoid arthritis in the joints of the fingers have a higher risk.
Risk of developing psoriatic arthritis - it is estimated that between 10% and 20% of all patients with psoriasis develop psoriatic arthritis; in about 20% of these people the arthritis symptoms occur before the psoriasis ones.
Cancers - patients with severe psoriasis who received systemic medications (those that affect the whole body) have a higher risk of developing skin cancers and lymphomas.
Psychological and emotional consequences - living with psoriasis often has emotional and social consequences.
Patients may feel embarrassed by having visible plaques and that can lead to depression. In some cases the individual may withdraw from society.
Symptoms may become so severe that patients have to leave their jobs, further increasing the risk of psychological and emotional problems.
Several surveys have shown that a significant number of patients with psoriasis report a negative mental and physical impact that is similar to several chronic conditions, including cancer, hypertension, heart disease, depression and diabetes.

Natural Treatment For Gout

1. Try essential oils for relief

Essential oils for relief:-

There are many ways to use an essential oil. The simplest way to use an essential oil for the treatment of gout is with a carrier oil.

  • Select the oil of your choice and a bottle of carrier oil (olive oil and coconut oil are both oils that you may have in your kitchen that is good carrier oils).
  • In a small bowl, mix a tablespoon of the carrier oil and 4-6 drops of the essential oil. Blend well.
  • Apply topically to affected joint and massage into skin.
  • Repeat 2-3 times per day.

2. Supplements and other natural treatment for gout:-

There are a variety of supplements that can be taken that have reported benefits for the natural treatment of gout.

  • Vitamin c may reduce uric acid. Some experts recommend taking sodium ascorbate as it is less acidic than ascorbic acid; the typical recommended dosage is 500-1000mg twice daily, but you should discuss this with your physician.
  • Apple cider vinegar reportedly makes the body more alkaline. Apple cider vinegar for gout may help with gout symptoms. The recommended daily dose is 1-2 tablespoons in 8 ounces of water. This can be sipped throughout the day.
  • Baking soda is another reported natural treatment for gout and is supposedly a pain reliever. To try this treatment, mix ½ teaspoon of baking soda in 8 ounces of water and drink in one sitting. This can be repeated several times throughout the day, with the maximum daily dosage being four teaspoons of baking soda in one day, and should be decreased as pain improves.
  • Bromelain is a compound that is found in pineapple; it can also be purchased as a supplement. It is thought to be a highly effective compound in treating gout and may even have anti-cancer benefits.
  • Castor oil can be used topically to relieve pain. Soak an old cloth with castor oil and apply to the affected joint. Wrap the fabric and joint with plastic wrap and an ace bandage. If you do not have any issues with neuropathy, apply heat to the joint as well. Leave on overnight.
  • Epsom salts baths can be relaxing and also relieve pain. Place 1-2 cups of epsom salts in a warm bath and soak for 20 minutes. You can also use a footbath if you desire just to soak your feet; place 1/3 cup of epsom salts in the footbath with warm water.  

3. Using food as a natural treatment for gout:-

Changing your diet is one of the ways you can treat gout effectively. There are some types of foods which offer an anti-inflammatory defense and others which lower uric acid levels.

1. Coffee – whether it is regular or decaf – appears to lower uric acid levels.

2. Cherries – both cherries and cherry extract – reduce uric acid levels in the blood, as well as decrease the number of gout flare-ups.

3. Blueberries and blackberries – these berries are loaded with a type of antioxidant which helps to protect the arterial walls damaged by gout. You can reduce inflammation by adding blackberries and blueberries into your diet.

While we all feel sad, moody or low from time to time, some people experience these feelings intensely, for long periods of time (weeks, months or even years) and sometimes without any apparent reason. Depression is more than just a low mood – it's a serious condition that affects your physical and mental health.

Types:

-Unipolar and bipolar depression:
If the predominant feature is a depressed mood, it is called unipolar depression. However, if it is characterized by both manic and depressive episodes separated by periods of normal mood, it is referred to as bipolar disorder (previously called manic depression).

Unipolar depression can involve anxiety and other symptoms - but no manic episodes. However, research shows that for around 40 percent of the time, individuals with bipolar disorder are depressed, making the two conditions difficult to distinguish.

-Major depressive disorder with psychotic features:
This condition is characterized by depression accompanied by psychosis. Psychosis can involve delusions - false beliefs and detachment from reality, or hallucinations - sensing things that do not exist.

-Postpartum depression:
Women often experience "baby blues" with a newborn, but postpartum depression - also known as postnatal depression - is more severe.

-Major depressive disorder with seasonal pattern:
Previously called seasonal affective disorder (SAD), this condition is related to the reduced daylight of winter - the depression occurs during this season but lifts for the rest of the year and in response to light therapy.

Causes:

The causes of depression are not fully understood and may not be down to a single source. Depression is likely to be due to a complex combination of factors that include:

-depressed woman
-Depression has a wide range of causes and potential treatments.
-genetics
-biological - changes in neurotransmitter levels
-environmental
-psychological and social (psychosocial)

Some people are at higher risk of depression than others; risk factors include:

-Life events: These include bereavement, divorce, work issues, relationships with friends and family, financial problems, medical concerns, or acute stress.
-Personality: Those with less successful coping strategies, or previous life trauma are more suceptible.
-Genetic factors: Having a first-degree relativeswith depression increases the risk.
-Childhood trauma.

-Abuse of recreational drugs: Abuse of alcohol, amphetamines, and other drugs are strongly linked to depression.
-A past head injury.
-Having had one episode of major depression: This increases the risk of a subsequent one.
-Chronic pain syndromes: These and other chronic conditions, such as diabetes, chronic obstructive pulmonary disease, and cardiovascular disease make depression more likely.

Symptoms:

-Depressed mood

-unintentional weight loss (without dieting) or low appetite
-insomnia (difficulty sleeping) or hypersomnia (excessive sleeping)
-psychomotor agitation, for example, restlessness, pacing up and down

-impaired ability to think, concentrate, or make decisions
-recurrent thoughts of death or suicide, or attempt at suicide

We've become accustomed to doctors using specialized blood tests or other extensive laboratory tests to help them make a conclusive diagnosis. However, most laboratory tests are not very helpful when it comes to diagnosing depression. In fact, talking with the patient may be the most important diagnostic tool the doctor has. The recommendation is that doctors routinely screen all individuals for depression. This screening might occur during a visit for a chronic illness, at an annual wellness visit, or during a pregnancy or postpartum visit.
