It is no longer a secret that, both obese males, as well as females, could be victims of infertilityEstrogen is produced primarily in our ovaries. But more than 30 percent of the estrogen in our body is produced by fat cells. In a normal weight person, the amount of estrogen produced is adequate. 

In an obese person, there will be extra deposits of fat which will be secreting excess estrogen. This estrogen excess interferes with ovulation and causes irregular menstrual cycles. In women who are severely obese (with BMIs ranging from 35-40), the hormonal imbalance becomes so severe that it can cause problems with conception leading to infertility.

Obese men have lower circulating testosterone which affects sperm production known as spermatogenesis. This affects the quantity and quality of a man's sperm. Obese men have increased the risk of oligozoospermia, having fewer than 15 million sperm per milliliter of semen, and far fewer motile sperm than a man of healthy weight. Sperm with high amounts of damaged DNA is significantly more common in obese men than in normal-weight men.[13] Ejaculate volume is also affected. Altered male hormones can also give erectile dysfunction which is the major cause of infertility in obese men. ​

Bariatric surgery - Experience it to believe it!

If Obesity is the root cause of infertility in the males or the females, losing weight is the first step in increasing the chances of fertility. Loss of significant weight leads to a gamete of changes in the hormones, thus resetting the balance as before. In my practice, I have seen the impossible become possible in just a matter of 6 to 12 months after significant weight loss. Couples who have been knocking doors of fertility centers ranging from 3 years of married life till 17 years of married life have yielded excellent results after weight loss surgery/ bariatric surgery. However, I have been very choosy in selecting my cases. By this I mean to choose cases in whom the only reason not to conceive is morbid obesity, wherein all other causes of infertility have been ruled out. Loss of the first 15 to 20 kgs sets the periods back to regularity. In fact now we have to ask the couples to please take precautions to avoid pregnancy until the mother is able to take in adequate quantities of food intake (as it takes few months for the stomach to stretch after bariatric surgery).

In my practice, I have witnessed gratifying results after bariatric surgery in Bangalore in terms of both primary infertility as well as secondary infertility due to Obesity. Be it the sleeve gastrectomy or the gastric bypass.
It is a myth that one can’t get pregnant after bariatric surgery. These could include the use of c-arm fluoroscope, other one is called the ultrasound or may be even an arthroscope. 
  4. The correct dosage: This is a gray area, as knowledge needs to be further refined about the precise dosage required for the treatment. Specialised instruments and digital centrifugation systems help in  acquiring and concentrating the necessary amounts of stem cells for the procedure. 
  5. Understanding that the treatment works: Since stem cell treatment of any kind is a fairly new concept, most people feel that it may not work as advertised. However, if you go to a reliable facility where such surgeries are performed, you will get to know that not only does the treatment work but there have been several patients whose quality of life has improved greatly. Post-Transplant Lymphoproliferative Disorder (PTLD) - How To Diagnose It Correctly?

Post-Transplant Lymphoprolipherative Disorder (PTLD) is a condition or a reaction that happens post your liver transplantation or any kind of organ transplantation. There is a rapid increase in the immune cells or more precisely, the lymphoid cells in the body. These cells start increasing without any control. The seriousness of this growth can be harmless but sometimes, it can develop into lymph node cancer as well.

The major reason behind PTLD is the anti-rejection medicines that are taken by the patient to avoid rejection and attack by the immune system.

Types of Post-Transplant Lymphoprolipherative Disorder (PTLD)

  1. Early lesions: This will go away by itself, if the dose of anti-rejection medicine is reduced.
  2. Polymorphic PTLD: This is a mixture of different kinds of lymphoid cells.
  3. Monomorphic PTLD: This is the most common type of PTLD found in many people who have undergone a liver transplantation. This has just one type of lymphoid cell.
  4. Hodgkin Lymphoma: This is a very rare kind of PTLD.

Diagnosis of Post-Transplant Lymphoprolipherative Disorder (PTLD)

  1. Small and painless lump under the armpit, groin, or neck area
  2. Night sweats
  3. High fever
  4. Excessive weight loss
  5. Low appetite
  6. Unwell feeling

Usually, it is not at all easy to diagnose PTLD. The lump may grow so deep inside the body that it can be hard to see or feel it from outside. Apart from the lump, others are general symptoms and they can be a reason for any kind of infection in the body post transplantation. It can be a reaction to any new medicine taken by you or a reaction to rejection. Early diagnosis can help you in increasing the chances of a successful treatment. A biopsy is also a good way to detect PTLD, where a small piece of the liver is taken and tested under a microscope to detect PTLD.

Management of Post-Transplant Lymphoprolipherative Disorder (PTLD)
Your liver transplant surgeon is going to work closely with the lymphoma doctor in order to provide the best treatment for you. For early lesions or Polymorphic PTLD, reduction of the anti-rejection dose can give you better results. Your immune system will restore back and will destroy all the abnormal cells in the body. But it has to be done slowly so that it does not cause organ rejection again. If reduction of anti-rejection medicine is not possible or if reduction causes any kind of harm, then other treatment methods like antibody medicine such as Rituximab, or chemotherapy regimen or radiotherapy may be used.

Lybrate-user, you are approx. 20 to 25 kgs overweight and you have a fatty liver which is a matter of concern. You need to restrict your calorie intake to 1000 per day and also go for a brisk walk 45 min per day. Ensure to hydrate yourself well by drinking plenty of water. According to your height and weight Your BMI is 31 ,during Pregnancy you should gain 5-9 kg weight only. You should do regular exercises and daily 20-30 minutes walk. Today I would like to speak upon how our hormones play a major role in weight loss journey. So what are hormones exactly? Hormones are tiny chemicals messengers delivering the message to organs and tissues. They travel throughout our body. And if there is an imbalance in one or the other hormone it disturbs the whole endocrine system. There are few diseases related to imbalance like PCOS, diabetes, thyroid and anxiety, sleep disorders, acne, facial hairs and many more. Now there are few steps how we can control these imbalance. For doing that I can tell you few points are this: eat real foods, eat real foods means include green veggies in your diet, green veggies have a fight to nutrients in it which helps to break down your hormones which are not in use at that particular time. Avoid to have processed and packed foods because it has neurotoxins present in it that disturbs our endocrine system. Include healthy fats in your diet,healthy fats means coconut oil, olive oil, avocado, hand full of nuts and seeds. And the second would be don t use toxin products, toxin products that are disturbing your hormonal system, like use of plastic products: plastic bottles, plastic utensils, chemicals presents in these also is disturbing your hormone. So replace them with glass or steel. And you can avoid using cosmetics as much as possible because nowadays there is a chemical present in these cosmetics is also seeping from our skin into our body and it disturbs a hormonal balance. So use occasionally as you can do it. Don t use it on daily basis. Third would be reduce your stress and have quality of sleep. For reducing your stress you can do the following steps. Now there is a hormone related to stress i.e. a cortisol hormone and if it is raised then you are not able to lose weight if you are doing a good diet and exercise also. So there is a key role to reducing the cortisol i.e.you have to remain happy, for reducing your levels for reducing stress you can do meditation, yoga, you can listen songs, go for a shopping, and go for a long drive and many more things that makes you happy. You should have quality sleep. Quality sleep of at least 7-8 hours that s make your body s recovery more. So that is very necessary for our body to have quality sleep. The next would be limit your caffeine intake. Caffeine is a chemical present in colas, energy drinks,in tea coffee. So limit as much as possible because it also elevates your cortisol level, insulin level, adrenal i.e. produce from an adrenal gland. So try to limit from 1-2 tea cups of coffee ortea. And replace it with a infused water or herbal teas. Now the last would be exercise. Exercise plays a major role in your weight loss journey because it helps you to balance your hormones, it aids sleeps, regulates your appetite and many more things. So I would suggest to do your physical activity in your daily practice for 30-45 minutes.

We will land up with the accumulation of fat in the internal organs and the abdominal area. After a certain age, our muscles become loose and the energy required for calorie burn is less because our muscle mass goes down. So if we have less muscle mass, then fewer calories are burnt as required in our body. As we age, less calories are consumed and required.
