Post-Transplant Lymphoprolipherative Disorder (PTLD) is a condition or a reaction that happens post your liver transplantation or any kind of organ transplantation. There is a rapid increase in the immune cells or more precisely, the lymphoid cells in the body. These cells start increasing without any control. The seriousness of this growth can be harmless but sometimes, it can develop into lymph node cancer as well.

The major reason behind PTLD is the anti-rejection medicines that are taken by the patient to avoid rejection and attack by the immune system.

Types of Post-Transplant Lymphoprolipherative Disorder (PTLD)

  1. Early lesions: This will go away by itself, if the dose of anti-rejection medicine is reduced.
  2. Polymorphic PTLD: This is a mixture of different kinds of lymphoid cells.
  3. Monomorphic PTLD: This is the most common type of PTLD found in many people who have undergone a liver transplantation. This has just one type of lymphoid cell.
  4. Hodgkin Lymphoma: This is a very rare kind of PTLD.

Diagnosis of Post-Transplant Lymphoprolipherative Disorder (PTLD)

  1. Small and painless lump under the armpit, groin, or neck area
  2. Night sweats
  3. High fever
  4. Excessive weight loss
  5. Low appetite
  6. Unwell feeling

Usually, it is not at all easy to diagnose PTLD. The lump may grow so deep inside the body that it can be hard to see or feel it from outside. Apart from the lump, others are general symptoms and they can be a reason for any kind of infection in the body post transplantation. It can be a reaction to any new medicine taken by you or a reaction to rejection. Early diagnosis can help you in increasing the chances of a successful treatment. A biopsy is also a good way to detect PTLD, where a small piece of the liver is taken and tested under a microscope to detect PTLD.

For early lesions or Polymorphic PTLD, reduction of the anti-rejection dose can give you better results. Your immune system will restore back and will destroy all the abnormal cells in the body. If reduction of anti-rejection medicine is not possible or if reduction causes any kind of harm, then other treatment methods like antibody medicine such as Rituximab, or chemotherapy regimen or radiotherapy may be used.

Surgery ke baad bhi apki diet healthy hi honi chaiye and routine test ki jrurat pdti hai. Hum follow up apka 1 year tak krte hain. 2nd year me apko every 6 months me ek baar diet follow up ke lia bulaya jayega. 3rd year ke baad apka sugar jab normal ho jata hai to apko aage 1 saal me 1 baar bulaya jata hai. Bacha bhi aap 1.5-2 years ke baad hi plan kren.

I am Dr. Harish Tolia. I am a family physician in Ghatkopar, Mumbai. Today we will talk about obesity. There are lots of questions coming up here on lybrate regarding on how to reduce your weight. Many of the questions are like- I want to reduce my weight in just 1 month. That's a difficult target to achieve. Practically when we advise patients we try to understand their own diet what it is and according to the diet we try to decrease the calorie intake. Usually, it is 10 to 15% initially gradually we decrease it. The target for decreasing the weight should be a maximum of 2 kg per month. Sometimes we get a question like I want to reduce 30 kgs and within a month or so which is not easy not possible and it will give a very rebound phenomenon.

So it is better to go very gradually. Try to adjust yourself to the new diet and go further. We get patients from all over India. Sometimes from South India patient will say I want to decrease my weight then I must know his diet. If I don't know his diet and ask him to eat two chapatis it will be too much for him because he's not used to. If he is from north India if he is eating regularly chapati, if I ask him to eat rice also it's difficult for him. So every individual has to give his full detail of his diet and accordingly we can plan a program for you. Some people are veg some people are non-veg. So for that also we must have a clear idea of what type of food you are going to eat.

Type of work you are doing very heavy manual work or a sedentary type of work. In that case, also calorie intake has to be adjusted according to it. The heavy manual worker cannot take a very low-calorie diet and survive so he should take a high-calorie diet. So initially what I want to tell you is- do not try to reduce your weight drastically. Go very gradually. take a pause and let your body adjust to that type of weight. then again restart your diet or increase your percentage of diet reduction. Welcome to lybrate to watch my video. Today I am going to discuss a very important topic with you. The topic of why it is so difficult to lose weight.

Many of my patients they come to me and they have this pertinent question which keeps on rising day by day that doctor I am trying everything which I can do as far as diet is concerned, exercise is concerned, in spite of all these measures I haven't been able to lose weight such as the weight I would like to lose. Well friends this question would be best addressed if I give you the right quantum of advice and the mechanism behind the reasons because of which weight loss is such a difficult proposition for the human body. You all are aware that all of us have got a very specific temperature, beyond this temperature we feel fever and below this fever, below this temperature, we start feeling hypothermia. So body has got a thermostat installed in itself because of which only a specific temperature suits the body. The normal of this temperature is set for every human being and that is why that normal is normal that doctor is seeking as far as the temperature is concerned.

In the similar manner the body has got an osmostat, the problem with osmostat is but the body decides on itself and what is the proper way which is applicable for a particular human body. So our bodies are accustomed to a particular weight, a person who has got a weight of 80 kgs and want to lose it till 60 it is so difficult for that person to lose weight because the body thinks that if you are losing weight you are under stress and it tries to bring you back to the same level of weight which you are presently or currently rappelling with. So to say it in a very simple manner a person who is at 80 kgs of weight would find it very difficult because the body would not let itself to lose weight because of the osmostat start mechanism. Today I would like to speak upon how our hormones play a major role in weight loss journey. So what are hormones exactly? Hormones are tiny chemicals messengers delivering the message to organs and tissues. They travel throughout our body. And if there is an imbalance in one or the other hormone it disturbs the whole endocrine system. There are few diseases related to imbalance like PCOS, diabetes, thyroid and anxiety, sleep disorders, acne, facial hairs and many more. Now there are few steps how we can control these imbalance. For doing that I can tell you few points are this: eat real foods, eat real foods means include green veggies in your diet, green veggies have a fight to nutrients in it which helps to break down your hormones which are not in use at that particular time. Avoid to have processed and packed foods because it has neurotoxins present in it that disturbs our endocrine system. Include healthy fats in your diet,healthy fats means coconut oil, olive oil, avocado, hand full of nuts and seeds. And the second would be don t use toxin products, toxin products that are disturbing your hormonal system, like use of plastic products: plastic bottles, plastic utensils, chemicals presents in these also is disturbing your hormone. So replace them with glass or steel. And you can avoid using cosmetics as much as possible because nowadays there is a chemical present in these cosmetics is also seeping from our skin into our body and it disturbs a hormonal balance. So use occasionally as you can do it. Don t use it on daily basis. Third would be reduce your stress and have quality of sleep. For reducing your stress you can do the following steps. Now there is a hormone related to stress i.e. a cortisol hormone and if it is raised then you are not able to lose weight if you are doing a good diet and exercise also. So there is a key role to reducing the cortisol have to remain happy, for reducing your levels for reducing stress you can do meditation, yoga, you can listen songs, go for a shopping, and go for a long drive and many more things that makes you happy. You should have quality sleep. Quality sleep of at least 7-8 hours that s make your body s recovery more. So that is very necessary for our body to have quality sleep. The next would be limit your caffeine intake. Caffeine is a chemical present in colas, energy drinks,in tea coffee. So limit as much as possible because it also elevates your cortisol level, insulin level, adrenal i.e. produce from an adrenal gland. So try to limit from 1-2 tea cups of coffee ortea. And replace it with a infused water or herbal teas. Now the last would be exercise. Exercise plays a major role in your weight loss journey because it helps you to balance your hormones, it aids sleeps, regulates your appetite and many more things. So I would suggest to do your physical activity in your daily practice for 30-45 minutes.
