Hello, my first question to you would be what is the major concern in Indian society which is associated with diabetes? a

As you all know in this modern world, digital world India is having a big threat of diabetes. India is having the largest diabetic population in the world in 2015 WHO and Indian diabetic Federation found out that 6.91 crore people are there in India who are diabetic. In next 2-3 years there will be 12 crores diabetic, every 9 person is diabetic in India. That’s an alarming figure.

Why is that this disease called diabetes is not easily managed?

This can be divided into 2 factors: patient factor and doctor factor.

Doctor factor is that we have very good doctors and physicians in India but good doctors are very busy they are having 20-30-40-60 patients a day so they are not being able to give sufficient time to patients, there is a study in USA that biggest issue amongst doctors patient relationship is that poor communication poor time given by the doctor to the patient that is lacking so doctor could not provide information, pura jankari nahi de pata patient ko usko kya kya chaiye because diabetes bohot badi issue hai. Usme lifestyle changes chaiye, diet ka role chaiye, exercise ka role chaiye aur manage ho nahi paati hai so 80-90 % patients ka blood sugar manage nhi ho pata. Ye bohot bada issue hai toh isko control karna bohot zada zaruri hai , toh patient mein kya hota hai patient ek toh darai mein rehta hai mujhe bimari kese hogayi no 2 ignorence ki keise hogayi kyun hogayi keise thik karu toh knowledge hota nhi hai aur doctor k pass time nehi hai samjhane ka teesra fear k abb kya hoga thik hoga nehi hoga aur isike wajah se complies kharab ho jata hai means wo doctor ka prescription follow hi karta test thik se nhi karta manage hota nhi hai aur wo door to door doctor prescription change karna dawai change karna toh pareshan rehta hai patient aur kabhibi diabetes sugar manage nhi ho paati hai toh patient ko ek regime follow karna chaiye ek strict regime aur doctors k sath bethna chaiye aur discuss karna chaiye ki mein apni diabetes ko keise thik kar sakta hoon.

Ok we have also heard that diabetes never come alone so what are the other complications associated with diabetes? diabetes agar kaafi saal uncontrolled rahe toh cholesterol ki abnormalities ho jati hai , lipids kharab ho jatein hai cholesterol jamm jate hai apke blood vassals mein aur cardiovascular diseases , hyper tension long term mein kidney failure , erecting mein problem pet mein azma nhi rehta kabzi hoti hai aur aankhon ka parda aur aankhon ki roshni jaa sakti hai so ye haar major aang ko kharab kar sakti hai , bohot bada issue hai .

And how do you compare Ayurveda with modern medical practices? My wife is an Ayurvedic consultant and I am practicing modern medicine so we try to combine modern medicine with Ayurvedic. So the evicasy of modern medicine is maintained and patients can tolerate them very well so Ayurveda mein bohot classical drugs aati hai jo prescription mein milti hai jaise triphala hai lasan adibati hai jo sugar level ko niche rakh sakta hai , aur neem aur jamun aur karela ka sevan se bhi kamm rehta hai particular matra mein lena chaiye doctor k prescription k upar lena chaiye toh inse jo modern medicine hai uske side effect bhi kamm rehta hai aur control bhi badiya rehta hai . aur hum ye yahan pe practice karte hai ki modern medicine aur Ayurveda ka mishran patients k liye kaafi suitable rehta hai .

What is Erectile Dysfunction (ED)?

A person having erectile dysfunction (ED) is not able to achieve and/or maintain an erection of the penis sufficient for sexual activity. So, penis do not get hard at all or not enough for doing sex. 

ED is responsible for great deal of stress and anxiety as well as low self-confidence in men. Mostly its occasional, when you cannot get an erection and it is only temporary. Normally after emotional or physical stimulation there is an increased filling of blood in sponge like tissue of penis and decreased blood flow out of penis, so blood remain trapped in penis and it becomes hard and erect.

Your ability to have erection is dependent of multiple factors like our emotions, hormones, nerves, blood vessels and cells of penis itself. A problem with any of these elements can cause ed. It can occur at any age, but becomes more common with increasing age. In the older adult male, it is a major problem affecting more than 75% of men.

Common Causes of ED

Physical causes indicate you have problems with penile structure itself or any bodily disease affecting it. Diseases include less blood flow to the penis or injury of the nerves going to the penis (for example, multiple sclerosis, a stroke, parkinson's disease, spinal injury etc.).

Lifestyle disorders like high blood pressure (bp), obesity (high body weight than normal for our height and age), diabetes (high blood sugar).

Also lack of male sexual hormone called testosterone, alcohol and drug abuse, clogged blood vessels (atherosclerosis) and side effects of some medications causes ed. Sometimes psychological issues like anxiety, stress due to work or home environment, depression, relationship difficulties leads to ed.

Myth 1: ED is part and parcel of your body's aging process

Contrary to popular belief, ED is not a normal part of the aging process. If you're having difficulty in getting an erection, the underlying cause can be medical in nature such as undiagnosed or diagnosed cardiovascular disease, high blood pressure or diabetes.

Myth 2: ED can't be reversed

Through the right treatment and certain lifestyle changes, most of the cases of ED can be reversed to almost normal. It is said that sexual health and erectile performance can be enhanced when you improve the health of your heart. Exercising to bring about weight loss and quitting smoking are ways that experts advocate for improving the health of your heart.

The mention of the word "orgasm" is enough to numb your senses. It is like a pleasure trip that nobody wants to end. It gives a person an inexplicable eternal feeling, a climax that most couples wait for with bated breath. As known to all, an orgasm is a zenith of a sexual interaction or intercourse. You will seldom come across a person who doesn't enjoy an orgasm. 

Sex without an orgasm can be quite dull and boring. 
Have you ever been intrigued as to why an orgasm feels so good? What is it that makes an orgasm such an enjoyable experience? This article is an attempt to unravel this simple yet important query that has been baffling people for long. 

An orgasm is a way our body reacts physiologically on being sexually aroused. Though the level of excitement and its impact may vary from person to person, most people (both men and women) feel good and euphoric on reaching an orgasm. There is an interesting science behind this. It is important to mention that the brain plays a pivotal role in an orgasm. As in any other organ of the body, there are numerous nerve endings present in the genitalia. These nerve endings are in turn connected to the brain and the spinal cord via the large nerves. 

In this regard, the nerves that are instrumental in sending the signals and the stimulation to the brain (different regions of the brain) on being aroused or excited sexually include 

  1. The Pelvic nerve is responsible for sending signals from the rectum (in both the sexes) and also from the vagina and cervix (women). 
  2. The Hypogastric nerve is responsible for sending signals from the prostate (men) and also from the uterus and the cervix (women).
  3. The Vagus nerve (the longest of the 12 cranial nerve) is only involved in sending the signals from the cervix, uterus, and vagina in women. 
  4. The Pudendal nerve transmits the signals from the scrotum and penis in men. In women, it transmits the signals from the clitoris. 

The signals sent to the brain during an orgasm results in its activation, especially the reward circuits or the pleasure center. The pleasure center is responsible for all kinds of pleasurable feelings in a person. The areas mainly affected by the sexual excitement include 

  1. Amygdala: It is mainly involved in the regulation of emotions like sweating, increased heart rate, and blood pressure. •    Ventral tegmental area (VTA): It is responsible for the release of the neurotransmitter, Dopamine. 
  2. Nucleus accumbens: The nucleus accumbens is an important part that controls or regulates the release of dopamine. 
  3. Others: Cerebellum (maintains and coordinates the muscular activities in the body) and the Pituitary gland (responsible for the release of vasopressin, oxytocin, and beta-endorphins). 

The arousal, followed by activation, acts as a catalyst which further results in release of neurotransmitters and hormones. A few other things that are emphasized are, encouraging physical activity, avoiding objects, actions, and events that threaten the body, overcoming anxiety and stress that compromise the psychological, mental, physical and welfare of the body, and seeking and giving support for self-care. 

Types of counselling: 

  1. Patient centered counselling: It changed the focus of counselling from a single informative one to one that concentrated on a patient’s circumstances and needs. This involved active listening, to render better support and managing client's particular prevention needs. 
  2. Risk reduction counselling: This includes examining client’s awareness about sexual activity and their feelings.Using their current HIV knowledge as an appealing tool, the limitations they have for protected sex is discussed. A better understanding is ensured in the patients so that unsafe behaviors can be avoided. 
  3. Grief counselling: People who are contaminated with HIV have an elevated risk of acquiring depression. Counselling helps you to deal with any emotional outburst. 

Aims of counselling in HIV infection-

Prevention is the first purpose of HIV counselling. It decides whether the lifestyle puts that individual at a higher risk. Helping to recognize and distinguish the definitions of high-risk behavior. The counselling contributes to behavioral change and also aims at working with the person and his family to obtain and sustain that change in approach and behavior.

However, this is an issue that can only be solved by a frank discussion, and when that is done, it might seem like there was no problem in the first place. 

Listed below are some things that can lead to a sexual drive. It is to be note that women and men have different reasons leading to reduced libido. 

  1. Reduced hormone levels: The most prominent cause is reduced levels of testosterone and estrogen, which are hormones required for development of sexual characteristics in males and females respectively. The levels can reduce depending on various reasons - from hypogonadism to loss of ovaries to psychological stress to medications to drinking and smoking. The first step in managing this would be to establish the fact that there are reduced hormone levels and pin the cause for it. Most people respond almost immediately to replacement therapy. Additionally, in some cases, the cause for reduced hormones can be addressed when identified. 
  2. Physiological problems: Especially in women, if there is painful intercourse, then the drive is naturally reduced. The reason for the pain could be spasms or infection and need to be identified and treated. Psychological constraints are another major reason that most females have painful intercourse. Counseling along with therapy if required can help these patients. 
  3. Medications: From anti-depressants to anti-hypertensives, though they are meant to cure some medical condition, they lead to some other condition. Drugs like selective serotonin reuptake inhibitors (SSRIs), tricyclic antidepressants (TCA), and beta-adrenergic blockers are famous for their ability to reduce libido. 
  4. Lifestyle habits: From smoking to drinking alcohol to junk food to lack of exercise, the effect on libido is very strong. Smoking decreases blood flow in general, leading to reduced desire. 
  5. Chronic diseases: Whether it is depression or diabetes, hypertension or Respiratory diseases, they all have an indirect effect on the sex drive of a person. Additionally, the constant stress of having to monitor health has effect on the overall health and definitely the sex drive. In some cases, this could lead to drinking and smoking and use of medications - all of which again can affect the sex drive. It goes into a vicious cycle and the patient needs to accept some facts and manage to work around the others. 

It is indeed very stressful when a person founds out that he is not able to have a good sexual drive. However, it is not impossible to manage with a little help and support, both from the partner and the doctor.
