Thanks for joining me again on lybrate And I hope my previous videos are very helpful to you for understanding your disease and for personal education about your physical fitness. Today, I’m going to touch a very taboo topic, a topic which is always discussed under wraps the topic of diabetes and impotence. Impotence is not about sterility. Sterility and impotence are two different things, impotence is the inability of a partner to satisfy its partner on sexual basic, sterility is the difficulty in the ability to have production, reproduction and that is a very important topic that I’m going to discuss today diabetes and impotence. Well, a lot of my patients of diabetes always have a query that doctor, how is diabetes going to affect my sex life, well my answer to you is yes diabetes is going to affect your sex life is not one but many ways. first of all diabetes in itself is a problem that leads to nerve dysfunction and in spite of getting stimulated some people in diabetes in later stages who got diabetes uncontrolled for very long, they start to develop a problem by the name of erectile dysfunction.

Erectile dysfunction is a problem in which after maximal stimulation a man is not able to have a proper erection and this problem actually comes up out of the nervous system problems that happen because of the uncontrolled blood sugar level well, don’t get me wrong I don’t want to imply here that only diabetes is the reason because of people develop erectile dysfunction but diabetes is a very important reason because of which people develop erectile dysfunction. The second problem that people with diabetes may develop because they can have an effect on their sex life is about premature ejaculation.

Premature ejaculation is the lack of stamina in continuing the sexual act and the culmination of the sexual act by ejaculation which is premature and this means that a patient is not able to have a sexual satisfaction with his partner and neither does the partner achieve complete satisfaction because the male partner has ejaculated and there is no further stamina left for the continuation of the sexual act. I don’t understand that these people who have these problems of erectile dysfunction and premature ejaculation they are more concerned about the sexual factor affecting them rather than diabetes that is affecting them but they have to understand that the core problem is the uncontrolled blood sugar, if they are able to control their blood sugar then probably they will be able have the cure and a solution to both these problems. I also would like to mention a very important fact that the levels at which the TSH is monitored during the pregnancy are different from the levels during which it is monitored the non-pregnancy state, in fact, a very strict control of thyroid is necessary during the time of pregnancy and if that is not maintained then not only congenital malformation but there could be also imminent fetal loss which can happen at any of the trimesters of pregnancy. In fact, conception it self-becomes a problem when the thyroid is not under control. And today we are into an era that a lot of females they are developing this problem, are facing this problem that they are not able to conceive in time and are very disturbed and agitated and their families are really frustrated because they are not able to conceive. Well that matter of fact, you need to check your thyroid and keep it under control. French thyroid can be controlled only by one pill in a day and needs to be monitored only. We all know that liver is the primary organ in the body that deals with metabolism and metabolism is the process by which we digest food, we assimilate it in our body and also eliminate the toxins that come with it. This hasn't happened suddenly but has happened because of 2 common causes known as diabetes and blood pressure. Diabetes forms the biggest cause of Kidney failure and kidney diseases and today for any stage of kidney illness diabetes is the number one cause for the people of USA. India is also fast approaching that scenario.

People believe that in diabetes when they get their kidney function test done and it comes out to be normal they think everything is fine which isn't the case. Kidney starts the sign of deterioration with diabetes very early on and it is very important for the Dr to detect if there is any kidney issue or not. The test required here is the urine test. In very early stage of diabetes there is microalbuminuria in urine which is the passage of protein from the body in very small amount, that is the result stage of kidney failure. Hello everyone! My name is Dr. Hanish Gupta and I’m here today to discuss with you the important effects of thyroid gland on human body.
We have appropriately titled this video as three minute thyroid consult because I’m going to explain you within three minutes that what is thyroid gland and how it affects the human body.
Thyroid gland is a butterfly shaped gland which is located in front of your windpipe just like this. This is the thyroid gland. Thyroid gland despite its small size is a very very important gland for the human body because it controls the growth and the energy processes of the human body that is why the effect of this hormone has a far reaching effect on the entire functioning of the human body.

Now I will talk about the excess and the deficiency of this hormone in the human body which needs two conditions called hypothyroidism when there is a deficiency of this hormone in the body and hyperthyroidism when there is an excess of this hormone in the human body. The most common condition of hypothyroidism or deficiency of this hormone in the human body is hashimoto thyroiditis in which the thyroid gland looks like this. This is a condition in which all the energy processes of the human body, they become slower. Hello everyone, my name is Dr. Hanish Gupta. I’m a specialist in internal medicine and mainly an expert in diabetes and cardio vascular diseases. Today I’m here with you to discuss a very important symptom of Chest pains.
It is a very worrisome symptom for any patient. It is very frustrating for the family members because they believe that every chest pain could be a hall mark of a heart attack. But my friends, this is not the case every single time. Chest pain is a pathogenic symptom of heart attack but all heart attacks do not necessarily come with chest pains and vice versa. It is most importantly found in patients of gastric diseases like Peptic Ulcer disease by which a patient suffering from hyper acidity would also come to his doctor with the same complaint. So if your chest pain is related to food, meals or associated with feelings of vomiting like nausea then definitely this could be related to the gastro-intestinal diseases. Moreover some patients come to us and say that they have a pin point chest pain which can be pointed at any part of the chest specially if they are pressing on that part and fuelling that same act of pain, then perhaps there is a muscular-skeletal chest pain which deals with muscles and the rib cage other than the heart itself.


Having said that, chest pain which is related to the Heart attack is almost always related to breathlessness. Nobody ever had a heart attack without having breathlessness. So this is a very important symptom to be noted in the patient that if he is having breathlessness associated with chest pains then it is a definite symptom which is going towards a ‘red flag’ sign of heart attack. Moreover I’d like to point out here that some of the patients of diabetes can present to us without having chest pain and just presenting to us with breathlessness. This is primarily so that just like there is a feeling of numbness in the limbs of a diabetic patient. Similarly there is a numbness id the muscles and nerves around the rib cage due to which this patient may have a silent heart attack which presents only with breathlessness and chest pain. That is why in patients of diabetes only breathlessness itself is a hall mark of heart attack.

If a person has chest pain which is related specially to activity, example: climbing up the stairs, walking, jogging, swimming or running or any kind of strenuous activity and that is the time when the pain but goes away when the patient is resting. This kind of a chest pain could be related to the ischemic heart diseases or the coronary artery disease that leads to heart attack in future.

Diabetes is not basically a sugar problem, rather it is a problem for Insulin. It is a problem of imbalance of Insulin in the body. There is sugar in the body that needs to enter the muscles, but it is not able to enter the muscles, primarily because Insulin is not available in the body. And that is why, whenever a diabetics comes to a doctor, the choice of medicines that runs across the minds of a doctor, is Insulin. Doctors believe that it is easy to take medicines by mouth rather than taking by injections. And that is the primary reason that Insulin is not prescribed to everyone.

There is a myth that is attached to Insulin that Insulin is a medicines which is prescribed to failure patients. Patients who cannot do anythinga and those which are at last stage of the disease are prescribed Insulin. But belive me, Insulin is not such a medicines which can only be prescribed to the patients on last stage or the failure patients. Insulin infact, can be prescribed at any stage of diabetes. And it can be discontinues also at any stage of the diabetes. There are some patients who have been taking Insulin from number of years but because their diabetes get controlled due to good amount of exercises and good amount of diabetic control, their Insulin is stopped. And they coninues to have a very healthy and a very natural life onwards. Just taking medication. There is also one thing that Insulin causes highest amount of hypoglycemia, meaningby which the lowering of blood sugar. I believe, that patients dont take Insulin that they believe that their sugar level rae going to be so low that there will be no sutainance of life.

But that is the problem, that is more prominent with cases of oral medicines taken. Infact, oral medicines cause Hypoglycemia upto a very prolonged period, upto a matter of 48 hours. But in Insulin, when there is a lowering of blood sugar or Hypoglycemia, tat lasts only a matter of 4-6 hours. Because the action of Insulin is such that it stops acting after it is eliminated from the body after 6 hours. But as far as the oral medication concern, they continue to be in the body for a long period of time, they also keeps on continuing and the Hypoglycemia also continues.

Extremely Low Thyroxine And Its Complications!

What is Thyroxine?
Thyroxine, also known as T4, is the main hormone which is secreted by the thyroid gland into the bloodstream. Although it is an inactive form, most of it is converted to an active form called triiodothyronine by organs such as the liver and kidneys. The hypothalamus in the brain along with the pituitary and thyroid glands modulate the production and release of thyroid hormones.

What Happens If You Have Too Little Thyroxine?
If the level of thyroxine in the blood is low, then the pituitary gland releases more thyroid-stimulating hormone (TSH) to try and stimulate the thyroid gland to make more thyroxine. Therefore, a raised level of TSH means the thyroid gland is underactive and is not making enough thyroxine. When the thyroid gland produces very little quantity of thyroxine, then the condition is known as hypothyroidism.

As thyroid hormones are essential for physical and mental development, so hypothyroidism during development or before birth and during childhood causes mental impairment and reduced physical growth.

Symptoms usually show a slow and gradual progression before they become worse over months or years as the level of thyroxine in the body gradually falls.

In adults it causes a decrease in metabolic rate resulting in symptoms that include:

  • Fatigue
  • Intolerance of cold temperature
  • Low heart rate
  • Weight gain
  • Reduced appetite
  • Poor memory
  • Depression
  • Stiffness of the muscles
  • Constipation
  • Infertility
  • Slow movements and thoughts
  • Muscle aches, cramps, and weakness
  • Dry skin

Less Common Symptoms include:

Etiology

  1. Autoimmune diseases: The immune system makes antibodies against your body cells
  2. Poor iodine intake 
  3. Use of certain drugs like amiodarone and lithium
  4. Surgical removal or injury to thyroid gland
  5. Underactive thyroid gland (congenital hypothyroidism)

Complications

  1. Low thyroxine level causes the blood fats (lipids) - cholesterol etc., to rise
  2. Pregnant females have an increased risk of developing some pregnancy complications like:
  3. Pre-eclampsia (high blood pressure and protein in urine)
  4. Anemia
  5. Premature labor
  6. Low birth weight
  7. Stillbirth
  8. Serious bleeding after the birth
  9. Hypothyroid coma (myxedema coma) is a very rare complication
  10. Myxedema coma is a rare but life-threatening state of extreme hypothyroidism

Diagnosis and Investigations:

It involves a blood test which screens the levels of TSH and free T4 and T3. Usually, the reports show high TSH and T3 and T4 levels.

Treatment:

Treatment usually involves taking a daily tablet of thyroid hormone levothyroxine to replace the missing thyroxine. Treatment works very well for most people with hypothyroidism but is required for life.

What is Hormone Replacement Therapy?
Hormone replacement therapy is a treatment which is used to treat the symptoms that occur after menopause. As age advances, the ovaries of the female reproductive system produce fewer amounts of hormone, and because of this low level, women start showing various symptoms. Sometimes, these symptoms are so severe that they hamper activities of daily living. Thus, in order to lessen these symptoms, the hormones are administered in the form of oral medications.

The most common menopause symptoms are as follows:

  • Hot flushes, night sweats
  • Dry, wrinkled skin, and dry hair
  • Mood swings
  • Vaginal dryness, recurrent infection
  • Reduced sex drive, pain during the sexual act
  • Weakening of the bones

The decrease in estrogen hormone is responsible for this condition. Therefore, in hormone replacement therapy, the medications which contain estrogen are given. Hence, it is also known as estrogen replacement therapy. Some types of hormone replacement therapy contain both progesterone and estrogen, some contain only estrogen, and sometimes, testosterone is also used.

Ways of Administering Hormone Replacement Therapy

  1. Estrogen pills: Most estrogen pills are taken once a day without food. In cyclical hormone replacement therapy, tablets are taken at the end of the menstrual cycle for 14 days, or a daily dose of estrogen and progesterone for 14 days for every 13 weeks. In continuous hormone replacement therapy, the patient takes a continuous combination of estrogen and progesterone.
  2. Estrogen patch: The patch is usually applied on the skin of the abdomen. Depending on the patient requirement, some patches are replaced every few days, while others can be worn for a week.
  3. Topical estrogen: It includes creams, gels, and sprays. In this type, estrogen is absorbed through the skin directly into the bloodstream.
  4. Vaginal estrogen: It comes in the form of a cream, vaginal ring, or vaginal estrogen tablets. It is usually given in females having symptoms of vaginal dryness, itching, etc.

Side-Effects of Hormone Replacement Therapy

  1. Bloating, indigestion
  2. Breast tenderness or swelling
  3. Swelling in other parts of the body
  4. Headaches
  5. Vaginal bleeding

Who Should Not Got For Hormone Replacement Therapy?

  1. Hormone replacement therapy is contraindicated in following diseases as it can worsen the condition of the patient:
  2. Uncontrolled blood pressure
  3. Severe migraines
  4. Blood clotting disorders
  5. Stroke
  6. Heart disease
  7. Endometrial, ovarian, or breast cancer

Lifestyle Modifications along with Hormone Replacement Therapy to Relieve Symptoms of Menopause

Following lifestyle modifications can be used along with hormone replacement therapy to relieve symptoms of menopause:

  1. Reduce consumption of caffeine, alcohol, spicy, and deep-fried food
  2. Stop smoking
  3. Perform regular exercise, yoga, and meditation
  4. Take proper sleep
  5. Use cooling gel pad, or pillows, or be in an air-conditioned rooms

UPT is to be done after 10 days of the missed for better reliability and accuracy. Also, its advised to use kits from different manufacturers in case one is confused about exposure during unsafe period of the cycle and the possibility of pregnancy is high on mind.

I took Blood test on Thyroid for me and my report shows TSH is 9.23 you iu/ml ,T3 is 139 ng/dl and T4 is 7.6 ug/dl. What is the impression and will be diagnosis for it.

Hypothyroidism is the diagnosis. TSH is out of range and behaves as a signal which goes up when the thyroid is functionally deficient. You would require thyroid replacement therapy which may or may not continue for long - depending entirely on your body response.

Hi. I have thyroid I'm taking thyroxine 25 every day. I have problem my heart bmp is 85 to 90 always. Is this high. I have sweating on my hand and palms. Please suggest.

Heart Rate of 85 to 90 Beats per minute (BMP) id falling in the normal range and as such there is nothing to worry about. Sweating on palms may or may not be related to any medical problem.

My advise to you is to just restrict yourself to the test that the doctor is asking. There is no such thing as a "whole body test" The labs all over the country have made this strategy to sell more and more of their tests to the public at large. So they marketed and sold the term - "whole body test" to the people. Essentially, all the people need to understand that a medical problem is not a problem until it is disturbing you symptomatically or the doctors observe something in the physical examination. Unfortunately, we all have become "seekers of sickness" as opposed being "seekers of health" We are always worried, infact, terrorised by hospitals and medical bills and thats why try to adopt an active approach for being diagnosed with something. Well, thats the wrong approach. If you get tested for "extra" tests - the only thing that you are helping is the revenue of the lab - you are not helping yourself!

A person should eat only that much amount of sugar which is as per his caloric recommendation which takes into account the energy requirement of a person. Which basically mean that a person involved in physical labour is recommended to eat more carbohydrates (read sugar) as compared to a person involved in desktop job As per WHO recommendation - daily salt intake should be 5gms salt or less.

But few days earlier tested in Kolkata then TSH level is 2.2.
What should she do right ...
TSH value is 6.7 and its definitely out of range. If report had been greater than 10 - then I would have recommended you to start treatment with thyroid replacement medicines straight-away but the guideline for treatment in your case where TSH is less than 10 is to correlate with another test with the name of ANTI-TPO. If ANTI-TPO is also way above range then you definitely need treatment for hypothyroidism.
