Hi, this is Dr. Suresh Ade. I am practicing as efficient Diabetologist and Neurologist in Navi Mumbai. My clinic has the highest diabetes and and neurologic clinic in Nerul. I am attached to various corporate hospital like Fortis Hiranandani Hospital Vashi, Reliance Hospital Kopar Khairane and Apollo Hospital Belapur. I’ve done speciality in diabetes like post graduate diploma in a diabetes and endocrinology from the Royal College of Physician, UK. I have done fellowship in neurology and stroke for 2 years and since 4-5 years I am practicing mainly diabetes and neurology.

Today I am going to talk about diabetes. Diabetes is increasing like anything. In 2020 it is expected to be more than 80 millions of patients in India suffering from diabetes. Diabetes is increasing at a very faster rate and India is now a capital of diabetes all over the world and expected to be double in a few years. There are many types of diabetes, type 1 diabetes, type 2 diabetes, secondary diabetes and pregnancy induced diabetes.

Type 1 diabetes is because of the Autoimmune destruction of pancreas and usually presented in childhood. The children are presented in the hospital with emergency like pneumonia and urine tract infection where the diabetes is detected and they have to be on an insulin all through the life. Type 2 diabetes is mostly inherited comes from the parents but many patients they get it after the sedentary lifestyle, stress, working all through the night and having a bad lifestyle.

Then secondary diabetes is usually seen in patients suffering from the endocrinopathies like thyroid disorders, pituitary disorders, patient being on a steroid for a long time, sometime infection is a reason and there are many other reasons of secondary diabetes. Pregnancy induced diabetes is usually seen in a pregnancy where all the patient when they consume they should undergo sugar test and confirm the diabetes and also sugar has to be monitored in the pregnancy. Type 2 diabetes is being very common, We have to focus more on Type 2 diabetes is usually seen and after the age of 30. Patients of diabetes type 2 they usually present with increased thirst, increase appetite, loss of weight, general weakness and in many time they have a non specific reasons like I am not feeling good, I am not feeling well and many times they land up in a emergency with some other complication like chest pain, pneumonia and then there diabetes is detected.

Once the diabetes is detected, patient has to be very focused on the treatment because the diabetes has got very very bad complications and the commonest complications are Microvascular , which means small vessel infections, small vessel infections in the eyes lead to retinopathy and can lead to blindness. Small vessels affection in the kidneys lead to nephropathy and patient may land up with kidney diseases, kidney failures and ultimately on a dialysis and death. Patients lands up with infection of the nerves in the peripheries and that is called peripheral neuropathy and that leads to various complications like injury to the limbs, ulcers and deformities of the foot and sometime they require amputation.

Macrovascular complications like infection of the bigger vessels - these are the vessels from the cartrods and beyond in the brain, going in the brain leads to the strokes. Vessels in arteries are called coronary arteries , leading to the heart attack and cardiovascular complications and vessels in the limbs that is called Peripheral vascular disease , and leading to the amputations and various other complications.

Apart from this, the patient may land up with acute complications like Diabetic Ketoacidosis which is again the life-threatening complication and patient may die off. There are other complications also that may be fatal to the life. So once the diabetes is detected , patient has to be very careful, he has to approach a diabetologist take a proper diet advice, advice regarding the exercise and if required medication. If patient controls his diabetes by any means like diet, exercise or medication and keeps his sugar under control then he will lead a normal life. So control your diabetes and be happy.

5 Most Common Types Of Lungs Diseases

Lung diseases are some of the most common diseases suffered by human beings throughout the world. Smoking and infections are responsible for most lung diseases. The lungs perform one of the most important functions of the body. It is also one of the most active organs in the human body and hence lung problems can arise due to problems in any other part of the body. Some of the most common and infectious lung diseases are discussed below...

1. Asthma
Asthma is a common long term disease which is characterized by reversible airflow obstruction and bronchospasm. The symptoms of asthma include coughing, shortness of breath and tightness of chest. These symptoms may occur frequently during a day and depending on the person, it may become worse during night or during a certain exercise.

2. Pneumonia
Pneumonia is another common lung problem which is suffered due to inflammation in the microscopic air sacs in the lungs known as alveoli. Symptoms of pneumonia include dry cough, chest pain, breathing problem and fever. Pneumonia is caused most commonly by viruses or bacteria. It is also caused by certain medications and conditions which are popularly known as autoimmune diseases. There are a number of vaccines available to prevent certain types of pneumonia. Other methods include hand washing and refraining from smoking.

3. Tuberculosis

Tuberculosis is a very infectious disease which is caused by the bacteria Mycobacterium tuberculosis. Although tuberculosis generally affects the lungs, but it has been known to affect other parts of the body as well. Symptoms of tuberculosis include cough containing sputum with blood, night sweats, fever and weight loss. Air is an active medium for spreading tuberculosis. This happens when people who already have tuberculosis sneeze, cough or speak. Infection occurs more in those who have HIV/AIDS or those who smoke. Prevention of tuberculosis includes staying away and keeping those who are at high risk, early detection and treatment and vaccination.

 

4. Mesothelioma
Mesothelioma is a type of cancer which affects the lungs. It affects the lining of the lungs and chest wall. Symptoms of mesothelioma include shortness of breath, swollen abdomen, cough, chest pain, weight loss and general lethargy. Mesothelioma is caused mainly due to exposure to asbestos. Those people who mine asbestos, produce products from asbestos, work with asbestos products are at high risk. Mesothelioma also results from genetical problems and due to infection caused by the simian virus 40.

5. Pulmonary Embolism
This is a disease caused due to blockage of an artery in the lungs by a substance which has travelled from another part of the body by the bloodstream. Symptoms of this disease include chest pain, breath shortness and coughing up of blood. There may also be signs of blood clot in the legs.

Growth Hormone Deficiency - Tests That Can Help Diagnose It!

Growth hormone or somatropin is responsible for cell growth as well as reproduction. But insufficient somatropin production by the pituitary gland may result in lack in height. It is mostly caused by a serious brain injury, any prevalent medical condition or might occur as a birth defect.

Types:

  1. Congenital GHD - This form of GHD appears from at the time of birth itself.
  2. Acquired GHD- GHD can be acquired during later life as a result of trauma, infection, tumor growth within the brain or radiation therapy.
  3. Idiopathic GHD- Idiopathic GHD is the third, and comparatively worst kind of GHD since it has no treatment.

GHD is mostly permanent, but can also be transient. Read on to know how to detect whether you or your child is suffering from growth hormone deficiency(GHD).

Symptoms:
Childhood onset

  1. Restricted height- Compared to other children of your child's age, he/she may be of shorter height, which is a rather conclusive sign to detect somatropin deficiency.
  2. Chubby and comparatively younger appearance- Your child may have a proportionate body, but if he/she is unnaturally chubby and has a baby-face compared to other children, he/she may be suffering from GHD.
  3. Late puberty- Your child's puberty maybe later than usual or even not appear at all, depending upon the gravity of the GHD.
  4. Hypoglycemia and exaggerated jaundice- Low blood sugar is amongst the most primary manifestations of GHD, along with extended duration of jaundice.
  5. Micro-penis- Micro penis condition is one of the incident signs of GHD, which later escalates to growth deficit as the infant gets older.

Adult onset:

  1. Fatigue- Adults with GHD may experience extreme tiredness throughout the day, with reduced muscle strength.
  2. Osteoporosis- Osteoporosis, along with bodily deformities, is a common sign in adults with GHD.
  3. Lipid abnormalities- A test of your lipid profile may reveal abnormalities in LDL cholesterol, insulin resistance, and impaired cardiac functions.

Tests to determine GHD:

  1. Physical test- A chart is drawn to determine the proportion of height and weight with respect to your age to detect anomalies.
  2. Hand X-ray- A hand X-ray can determine whether the age of bones are at par with your age.
  3. MRI-MRI scan can determine the health of your brain and pituitary gland.
  4. Test for other hormones- Growth Hormone may not be solely responsible for your health condition, so it is important to determine if other hormone levels are all normal.

Treatment:

  1. Hormone supplements- Hormone supplements like corticosteroids (hydrocortisone or prednisone), Levothyroxine (levoxyl, synthroid, etc), and others work to replenish the deficiency of pituitary hormones.
  2. Growth hormone injection- GH is injected beneath your skin, to cure GDH. This is a long-term treatment and requires constant monitoring.

When it comes to COPD or Chronic Obstructive Pulmonary Disease, not many people are aware of this condition. COPD is a chronic medical condition characterized by the inflammation of lungs. The inflammation interferes with the functioning of the lungs resulting in breathing problems. An early diagnosis and timely medication can improve the condition to a great extent. However, lack of proper knowledge and myths surrounding COPD often make the situation worse. In this article, we will discuss some myths and facts related to COPD to help people understand the condition better.

Myth: COPD is an incurable chronic lung disease mainly affecting the aged people (above 60 years).
Fact: There is no denying the fact that COPD is indeed a chronic disorder that severely affects the lungs and its functioning. However, with timely diagnosis and treatment, the condition and the deleterious consequences can be managed and controlled significantly. Since COPD triggers breathing problems (the airflow from the lungs gets obstructed), the use of bronchodilators, oxygen therapy or corticosteroids can be highly beneficial. Though COPD is rare among young adults, the condition can affect people in their late 30's or early 40's as well.

Myth: COPD only affects people who smoke.

Fact: Mild to moderate exercise (especially breathing exercise) are indeed helpful and go a long way in the effective management of COPD. Exercising regularly (for 20-30 minutes or as the doctor recommended) help to lower the blood pressure and improve the circulation that provides fruitful results for people with breathing problems. However, doctors do advise people with shortness of breath to avoid strenuous exercise.

Myth: COPD only affects the lungs.
Fact: With time and left unmanaged and untreated, COPD acts as a catalyst making a person more susceptible to heart ailments. In some cases, the condition can also trigger depression and anxiety.

Myth: In the case of COPD, giving up on smoking will not produce any positive results.
Fact: Giving up on smoking may not help to reverse the damage that has already been caused by COPD. However, quitting smoking along with proper medications and lifestyle changes does help to control the condition better. In some instances, there were also significant improvements in the associated symptoms (such as shortness of breath, fatigue, wheezing, a chronic cough).

Why Is Low Glycemic Index Diet Important?

Did you know that some carbohydrates can make your blood sugar shoot up faster than others? So, when you eat a lot of these good slower acting carbs, you can potentially control your blood sugar levels. The glycemic index is a handy tool that indicates the slow acting good carbs from the fast bad carbs.

What is glycemic index?
It is a simple number that lets you know how fast your body converts carbohydrates in your food into glucose. Two foods with the same number of carbohydrates can end up having different glycemic index numbers. The bigger the number, the higher impact it has on your blood sugar levels.

  • 55 or less is low and good for controlling sugar
  • 56-69 is mediocre
  • 70 or higher indicates a high number and is not so good

Hello, I have numbness in hands difficulty in grip and weakness in hands plus have varicose veins. My weight is 80 kg, height 4'11" and hypothyroidism.

These are symptoms of peripheral neuropathy detail evaluation is required to find out cause including kidney function test vitamin b12 levels liver function tests sugar test and repeat thyroid function test to check whether thyroid is under control or not. Sometimes nerve conduction studies are also required. Very much treatable problem. Get back with said test reports.

Ever been in a situation where a medicine you used to take regularly to deal with something, has suddenly stopped being effective? This is because of antibiotic resistance. This refers to the ability of bacteria to change their structure so as to resist the antibiotic. Thus, an illness that was once easily treatable is no longer affected by the same medicines and the bacteria causing the infection multiplies, thus worsening the condition.

How do bacteria develop antibiotic resistance?

When you take a drug, not all the bacteria present is affected equally. While some bacteria die out, others turn dormant. Repeated and improper use of antibiotics can lead multiplication of the dormant bacteria, which results in spreading of disease, instead of containing the same. One of the main causes of this is using strong antibiotics to treat minor viral illnesses.

What can you do to fight antibiotic resistance?

  • Follow your prescription: When taking a course of antibiotics, it is also essential to complete the course and not leave it midway just because you are feeling better. Antibiotics are also most effective when taken regularly. Hence, avoid skipping a dose in your treatment. Doing this does not completely cure the infection, but increases the bacteria's resistance to the drug.
  • Do not take someone else's medicine: Self medication can do more harm than good. Each drug attacks a particular combination of symptoms. Taking medication prescribed for someone else can increase the bacteria's resistance to antibiotics. It can also delay correct treatment, thus allowing your condition to worsen.
  • Discard the leftover medication: When stored for a long period of time, medicines lose their potency. Thus if you were to take them later, they would be less effective against the bacteria.
  • Do not take antibiotics for a viral infection: In any case, antibiotics to not address viruses and hence, the medicine will not have a beneficial effect on the virus. However, it can affect the bacteria present in your body by making it resistant to the drug.

Why drastic deficiency of vitamin D is becoming increasingly common among urban Indian youth?

Vitamin d is unique because it can be made in the skin from exposure to sunlight. We get a really good amount of sunny days as opposed to many western and northern countries where vitamin d deficiency is ought to be common.

It is surprising that vitamin d deficiency is such a common problem in India nowadays that it finds prevalence among 70-100 percent of the general population in the country. More than 80.63 percent of 73 lakh Indian youth suffer from the deficiency of vitamin d. What's more shocking is that it can lead to a variety of health issues ranging from osteoporosis to diabetes. Our youth is never out in the sun because they are too occupied with their laptops or video games. Their lifestyle is such that it does not allow them to spend some good time out in the sun, which is a major source of vitamin d for the body. Technological advancement and cut-throat competition have made the youth forget the sun as a source of a healthy dose of vitamin d.

2. No milk, no fish

A lot of Indian youth follow a strictly vegetarian dietary regimen and they avoid milk very commonly. Vitamin d is found in foods like fish, egg yolks, fish liver oils etc. And not consuming them is a major reason why youth face vitamin d deficiency.
