Causes and symptoms of Head and Neck Cancer

My name is Dr. Manish Julaha and I practice Head & Neck Oncosurgery. Today I'll tell you some facts about Head and Neck cancer how to predict them early. So in India approxiately 1.2 million cancer patients are detected every year, and out of this one thirds of the number occur in head and neck, mainly oral cavity and throat. And this is a very big number if you compare with rest of the countries because head and neck cancer they account for less than 10% in western countries and less than 4% in U.S. So what is the cause of such high number of head and neck cancers in India. It is mainly the use of tobacco and another sad fact about head and neck cancer in India is that majority of the patients, you can say more than 65% are present in stage 3 or stage 4. And we say cancer is curable if it is detected early and when patients presents to us in such late stages it effects all the paramaeters like overall survival, treatment outcome, side effects, functional outcome, quality of life everything is affected. And when patients come in early stage, the treatment is less invasive with less side effects and the functional outcome is very good and the chances of recurrence is very less.

So what can we do to detect head and neck cancer at the earliest. We can do this if we've the knowledge of the signs and symptoms which hint towards the head and neck cancer and by doing self examination. So what are the early signs and symptoms. They're mainly any ulcer in the oral cavity which dosen't heal in three weaks or any lump in the lung or oral cavity, any red or white patches in the oral cavity, or there maybe unexplained earache or nasal blockage, mainly on one side, specially with blood stained mucous discharge, ill fitting of denture because of any swelling in the gums. So, these are the early signs and symptoms. If we find any such thing we should take expert opinion. And by doing self examination we can detect head and neck cancer at the earliest. I would say all those people who're at a high risk of getting head and neck cancer they should do the self examination at least once a month. So who're the people who're at high risk of getting head and neck cancer. Most importantly those who use tobacco in any form, then elderly people above the age of 45 and people who have exposure to ionizing radiation in past, and people who have had head and neck cancer in the past they're at high risk of getting a second cancer so they should also be very careful.

Now, how to do this self examination. It is very simple, to do this you've to stand in front of the mirror and you've to feel both the lips, upper lips and lower lips, using your thumb and index finger feel both the lips and by pulling them outward you've to see the inner lining of the upper lip and lower lip and also you have to look at the upper gums and lower gums. And using your thumb and index finger pull your cheek and look at the inner lining of your cheek on both sides and look for any ulcer or lump or any red or white patches. Now just open your mouth and say "aaa" and while saying "aaa" just look at the throat and also at the roof of the mouth. Then by touching your tongue on the back of upper teeth have a look at the under surface of the tongue and the floor of mouth. Then pull your tongue outward and look at it's upper surface, by moving the sideways look at the sides also. Again look for any ulcer or lumps or any red or white patches and you have to examine your neck also. Just by standing straight palpate below the jaw bone and above the collar bones, and by moving the head backwards look for any asymmetry of the neck, if there is any fullness on one side as compared to the other side, there maybe some lump. If you find any such thing just take expert opinion.

Hi, I’m Dr. Shashidhar. I’m a pediatric ENT consultant and I work with a hospital called Artemis in Gurgaon. I’m going to talk about the common problems in children which is cold and why it comes again and again?  What is the solution for it?

It’s a very common problem nowadays with children going to day schools and play schools. At an early age and a lot of times, we’ve been told that this is because of the pollution or the allergies or infection from the surroundings but quite often we miss a very common cause of the cold which is what we call as Adenoids.

What are Adenoids?

Adenoids are small lymphoid tissue, the tissue that makes white blood cells in the body behind the back of the nodes. They are normal in children but sometimes they grow up in size. Like what you see here, like if this is the size of the adenoids, which should be there, is marked in violet, sometimes they grow, the bigger circle here the yellow in the back of the nose and block the nose. When this happens the common symptoms in children are snoring, breathing from mouth, restless sleep where the child is constantly turning this side and that side trying to get a good position in which he can breathe, some drooling and some coughing in the night.
More often than not we tend to miss Adenoids because we can’t see them directly and we tend to treat these conditions in terms of allergies, bronchitis and put the child in to nebulization and in unnecessary treatments.

What I’ve found in my own experience, that lot of the kids who have been told they have allergies and they have been told they have bronchitis have actually had Adenoids. So, if your child has reoccurring problems of sore throat, breathing difficulties and nose blocks, it’s worth getting evaluated for adenoids because the treatment of adenoids is very simple and often very gratifying. Which can be done with simple nasal sprays, saline washes and if it is a severer one it needs a surgical intervention to open up the nose.

Adenoids also cause recurrent ear infection, ear pain and sometimes they cause hearing disorder in children. It is quite difficult to evaluate such problem in children. You can only perceive it as, sometimes when you call them they don’t respond, their grades are falling in school and when they watch television they tend to raise the volume of the noise and this helps us to pick up the child to scream for the hearing problems because when you have adenoids you have a higher chance of getting ear infection and fluid inside the ear which is called otitis media, which can also be treated in the same lines. Rarely we need to put some grommets or some surgical intervention to drain the fluid out if we are not able to treat it medically.

Third problem which I wanted to discuss is Allergies in children. Aallergies in children can present in very wide spectrum. Sometimes they present only as cough, sometimes children get rashes on their skin, sometimes they can have allergies to food and recurrent diarrhea and other problems. So, allergies in children can be treated quite effetely with medicines and we have what we call as Immunotherapy to detect what the child is allergic to so we can specifically target that antigen and cure them for the long term. This now is the most gratifying and the latest treatment in allergies called Sublingual Immunotherapy.

Hi I’m Dr. Vijay Verma. I practice at Three senses ENT Allergy and Dental Clinic. We all know that kids are always suffering from continuous sneezing. There is an obstruction and also discharge and sometimes bleeding from the nose. They have breathing difficulties and I know that in every family kids are suffering from these problems because they are going into daycare and suppose that there are 20 kids and one out of them always has cold. Your kid is always prone but in depth, we ignore these symptoms of ‘Adenoids’.

Adenoids are nothing but just like tonsils but are present behind your nose. When they get enlarged they tend to cause nasal obstruction to the child. This infection always gives rise to continuous nasal irritation. Children tend to sneeze which causes nasal discharge. When these adenoids fall back inside your throat it causes sore throat and causes continuous cough for the child plus when they sleep, they are unable to take proper amounts of oxygen and if that is happening, they cannot grow properly

.And this one thing is always missed, so one should consult an ENT specialist for these problems of Adenoids because after 12 years although they regress in size, the changes which they lead to are permanent. They can protrude the tooth out, so sooner or later you will need to visit your dentist; a lifelong problem. There is also a possible accumulation behind the ear drum so the kid starts to have various problems with listening. All these are associated to Adenoids, so they start in childhood but leave permanent changes. So I would suggest you to visit your doctor as soon as possible if you notice that your child is having continuous sneezing, nasal discharge or sore throat.

For adenoids, there are two types of treatment:

(a) The therapy of Spray or

(b) Surgery, so it depends on the size of the adenoids.

Depending on that, it is medically treated or surgically treated. Usually all of us are scared of surgeries but now the tools have become very safe. It is either done with a microdebrider or a coblator which makes it a bloodless surgery and also one which is not a painful surgery; it would lead to your kids being happy.

As you all know allergy is increasing day by day, you can see kids, they are continuously sneezing, coughing, there are lot of increased cases of bronchial asthma. All these things, they are increasing, increasing and increasing. It may be because of the pollution levels. Now there is no proper exposure to sunlight, like we all live in AC, there is no as per cleaning of the room. You go to the office, they centralize AC , so you are never exposed to the environment now. There is always sunlight, there is no sunlight exposure per say. So all these problem they are coming up day by day. Also, it has been seen that allergies are associated with other diseases like headache, menial’s disease, even migraines. So there are so many things which are associated with the allergies. So allergies are the main culprit of your proper lifestyle problems.

Now how to deal with it?

The first thing is you should have a proper sunlight exposure. You should go for a regular walk and you should do your regular exercise something like that. If you have bronchial asthma then you need to consult your chest physician for your bronchial asthma part. Then nowadays newer modalities are coming up which is allergic testing followed by immunotherapy. In allergic testing, you can go ahead with a blood test or we can do the skin prick test. The blood test they are known to usually a good test because in India we don’t have proper labs to test it. Although, they are certain like Lal patch light test labs, they are doing that but still the results are not so good as equal to the skin prick test. In skin prick test you put some allergens over the hand and you test for it.

So if you are found positive to that thing we can start with immuno therapy. An immune therapy works very well and gives a permanent solution, although the duration of treatment is pretty much longer. it is again from three to five years . it is very promising result and you should go ahead with that. So these tests, skin pricking, I do.

Moderate Alcohol Consumption - How It Improves Brain Function?

It is common belief that alcohol consumption is a social menace and is injurious to health. It is the third leading lifestyle causes of death in the United States and takes a toll on the health care costs of the country. Many organs like the liver, kidney, heart, and brain are affected in people who are addicted to alcohol. However, it has been proven that alcohol consumption in minor amounts can have beneficial effects on the human body including the heart.

The side effects depend on a number of factors listed below.

  • Type of alcohol consumption (social or habitual drinker)
  • Amount of alcohol consumed
  • Frequency of drinking
  • Age, gender, and genetic predisposition of the person to develop alcohol-related diseases
  • Family history of alcoholism and its related diseases
  • Age at which the person started drinking
  • The number of years that a person has been consuming alcohol
  • Overall health condition of the person
  • Exposure to alcohol as a fetus

Like the adage goes, anything in excess is bad. It is not necessary that a habitual or social drinker who consumes a drink or two per week would end up with these issues. In fact, if recent studies are to be believed, mild to moderate intake of alcohol does have a benefit to overall health.

  • The cardiovascular benefits are the most prominent. Red wine and beer, in particular, are shown to provide benefits against cardiovascular damage in the long run. This could be due to the antioxidant properties of red wine. Alcohol itself (ethanol or ethyl alcohol) also has a positive effect. Some varieties of beer, porter and stout varieties in particular, are also shown to be beneficial. The anti-inflammatory properties and antiatherogenic (effect on plaque formation in the blood vessel) are what contribute to these positive effects.
  • There is evidence to show that cholesterol profile is improved (good cholesterol increase) and clotting function is improved in people who have mild alcohol consumption.
  • The insulin sensitivity is also better, leading to better control of sugar levels.
  • Light alcohol consumption also is believed to reduce the risk of hemorrhagic stroke. However, heavy alcohol consumption is believed to increase the risk of stroke and other cerebral events.
  • Limited alcohol use is also linked with reduced risk of dementia. Heavy consumption leads to faster memory decline.
  • The key to note is that these benefits are only when the alcohol consumption is mild to moderate. In excess, the negative effects of alcohol are well established and numerous. Some of the most significant ones being cirrhosis of the liver and cancers of various organs including mouth, liver, larynx, oesophagus, colon, breast, pancreas, etc. To reduce these effects, alcohol screening and brief counselling to reduce habituation are helpful.

The brain is said to weigh only 2% of the weight of a person but at the same time, it accounts for 20% of the energy consumed by him or her. There are so many great things by the brain, quite a lot of which are quirky.

The brain is constantly developing and changing. As a matter of fact, one thing which is said to change is the way the brain functions i.e. it is said that the brain could alter the way it functions in accordance with the seasons!

Researchers from Belgium found that when people performed some actions; while the manner in which these actions were performed did not change, what was observed to change was the amount of brain intensity which was warranted in order to complete these tasks.

Seasons on your mind?
Some of the weird things that were found by them include the fact that if a person wished to sustain his or her attention, the effort in neural terms was the least in the month of December. What this means is that if a person wishes to focus on doing something which is of quite some importance, then he or she is best off doing it in December as the stamina for doing so would be the greatest. On the other hand, the month of June was seen to demand the greatest effort for a person to concentrate.


In many situations, things seem to have been just out of mental grasp. It becomes difficult to recollect certain memories of a particular time. This problem is usually faced by people during the months of autumn. This is because the autumn time demands the most of the working memory of a person. On the contrary, if a person seems to have the ability to remember something off the top of his or her head, it is most likely to occur over the period of time around the spring equinox.

Now, a lot of people are likely to wonder why this is the case and whether this phenomenon can be seen in each and every person. The answer to that is that people who have SAD or Seasonal Affective Disorder may be more likely to have the working of their minds affected due to changes in seasons. This has not been proven yet.

Getting hallucinations is a mental condition where a person sees, feels, hears, and tastes things that actually don't exist beyond one's heightened imagination or delusion. It involves the experience of perceiving something not present.  Hallucinations can be pleasing or frightening. However, there is almost always an identifiable cause behind it. 

They can be triggered by:

  1. Taking hallucinogenic or psychotropic substances
  2. Mental conditions like dementia and schizophrenia
  3. Neurological conditions like Alzheimer's disease or Parkinson's disease
  4. Macular degeneration, leading to loss of vision
  5. Migraines and brain tumor can also lead to such delusions

Some of the signs of hallucinations:

  1. Hearing voices: The medical term for hearing voices is called an 'auditory hallucination'. A person may sense sounds or noises coming from inside or outside of their mind. The noise might be random or disrupting. One might also feel the voices talking to each other or trying to tell them something. Most of the times, these voices come from inside the person's mind; or in some cases, one's heightened perception may make a normal noise delusional.
  2. Seeing things: This is also called visual hallucinations. For instance, one may see unnatural things like a floating chair in thin air. It all depends on a person's perception. Sometimes these hallucinations appear as bright flashy spots or rays of light.
  3. False sense of taste and smell: Technically, these are known as gustatory and olfactory hallucinations respectively. One may feel a kind of odor coming from one's body or surrounding; or a person might feel that something he/she is drinking or eating has an odd taste. This is again too much thinking, causing delusional sensory activities. 
  4. Tactile hallucinations: This is when a person feels things that don't exist. One may feel that he/she is being touched or tickled even when no one else is around or that insects are crawling beneath the skin. One may experience strange sensations, which are not a part of a reality.

Dear user, There is no cure for migraine. Medicines are given to prevent the attacks of migraine. There are 4 to 5 medicines which are available. All are having equal efficacy ; but we choose the medicine according to patients.

Essential Tremors - How To Know If You Are Suffering From Them?

Essential tremors refer to a neurological disorder that causes uncontrollable shaking of the hands, arms, head and other parts of the body. The lower part of the body is rarely affected by this disorder. Also, both sides of the body may not be affected in the same way. This disorder does not usually need treatment unless it is severe enough to impede daily functioning and make the person dependant on others. However, patients suffering from this condition may often find it difficult to complete simple tasks like buttoning a shirt or writing.

Some of the symptoms of this disorder are:

  • Brief spells of uncontrollable shaking
  • Involuntary nodding of the head
  • Tremors that worsen under stress
  • Unsteady voice
  • Internal tremors
  • Difficulty coordinating voluntary movements such as walking
  • Balance problems in rare, severe cases

Essential tremors also affect the psychological make-up of a person. Depression and anxiety are often associated with essential tremors. The frustration of not being able to control tremors may also make a person withdraw from friends and family. People suffering from essential tremors also have a higher than normal risk of suffering from conditions such as Parkinson’s.

The exact cause of this disorder is not yet known and hence it cannot be prevented. Genetic mutations have been credited with causing this condition is many cases but the gene responsible for it has not yet been identified. Thus, essential tremors can be passed down from parent to child. However, the severity of the tremors and the age at which symptoms first become visible may vary. While some people show signs of tremors in their early teen years, others develop tremors only in their late 40s.

These tremors can also be caused by abnormal electrical activity in the thalamus. The thalamus controls and coordinates muscle activity. Tremors caused by old age or those caused by excessive consumption of alcohol, emotional distress etc are not categorised as essential tremors. However, ageing may make essential tremors more frequent and pronounced.

Blood, urine and other lab tests do not help in diagnosing this condition. A diagnosis is usually made on the basis of an understanding of the family medical history, a physical examination and complete neurological exam. In order to rule out other triggers for the tremors, a doctor may ask for thyroid tests. At present this condition cannot be cured but medication may help reduce the symptoms and improve the patient’s quality of life.

Levodopa - How To Maximize Its Affects For Parkinson's?

If you are being treated for Parkinson's disease. How can you maximize the effect of Levodopa? The effect of levodopa can be maximized by increasing its absorption from the digestive system. In particular, taking levodopa on an empty stomach is very important. 

  1. Take on an empty stomach: Do not eat anything 1 hour before taking levodopa. Do not eat anything for at least 30 minutes and if possible 1 hour after taking it. Take the pill with 2 glasses of water so that it can dissolve. If taking the levodopa on an empty stomach makes you nauseous, confirm that your medication has the right amount of carbidopa in relation to the levodopa (25%). If you continue to feel nauseous, you can eat a piece of white bread around the time that you take levodopa. Do not apply any butter, yogurt, cheese or similar high protein spreads. Protein can drastically decrease the absorption of levodopa. 
  2. Get your constipation treated: This is critically important too. Being constipated can slow down the passage of levodopa through the digestive system. In most patients, drinking adequate water and taking stool softeners is helpful. 
  3. Get your ulcer treated: It can be difficult to recognize that you have a gastric ulcer. However, if you have a burning sensation in your stomach at any time, or belch frequently, you may have a gastric ulcer. If you think you have an ulcer you should get tested (there is a simple breath test now) and treated with antibiotics (not antacids!) 
  4. Do not take levodopa with iron tablets: The iron binds to levodopa and prevents its absorption. Keep a gap of at least two hours between these two medications. 
  5. Try taking levodopa with orange juice and carbonated water (soda): Mix equal quantities of orange juice and carbonated water (soda). Take your regular levodopa with a glass of this preparation instead of water. The carbonated water hastens disintegration of the tablet, and the acidity of both things helps in levodopa absorption. There are special tablets of levodopa (dispersible levodopa, Madopar) which can be dissolved in this preparation. Talk to your doctor about it.
