As per your history it appears to be chronic Febrile illness. The doctors must have advised you for blood reports. If the blood reports are non suggestive of any disease then we have to go for bone marrow examination to find the cause of fever of unknown origin. You can discuss this things with your doctor. Also as she is not gaining weight it is important to keep a check on the diet that she is taking your doctor will help you in this also.

As the mother is already pregnant with three months and so now wait till delivery and get the infants blood checked for thalassemia and there is 50% chances for getting a normal child with carrier gene and 50% chances for the child being a thalassemia major but nothing to worry because there are treatment options available with bone marrow transplant where the child can live a healthy lifestyle and so please don’t too much about this factor now and continue with the pregnancy happily

There are more than hundred varieties of blood cancer, but the most common ones are leukemia, lymphoma, and myeloma. Acute myeloid leukemia (AML), a type of leukemia, is when the white blood cells in the marrow begin to grow uncontrollably. This is an acute condition and both the onset and progress are very rapid. The cause is not known, and given the rapid progress, even treatment is often difficult. A more severe form of AML is the APML which is acute promyelocytic leukemia, which leads to reduced number of white and red blood cells and plasma.

This causes the following symptoms:

  • Anemia
  • Increased incidence of infections
  • Excessive bleeding from even minor cuts
  • Bleeding from nose and gums
  • Easy bruising
  • Blood in the urine
  • Extreme paleness and tiredness

What is different about APML?

There is one distinguishing factor with APML, which is that they contain a protein which when released into the bloodstream can cause severe bleeding, which may be very difficult to control. Chemotherapy kills these cells and so releases the proteins into the bloodstream. These cells need to be managed without chemotherapy, as the patient can even die of uncontrolled bleeding. With medical advances, two non-chemotherapy agents have been identified – all-trans retinoic acid (ATRA, vesanoid, or tretinoin) and arsenic trioxide (ATO or trisenox).

  1. ATRA: This is a type of vitamin A which is used either alone, in combination with chemo, in combination with arsenic trioxide, and also in combination with both chemo and arsenic trioxide. The effect is different in different people – helps control spread, helps prevent recurrence, and helps control symptoms. In many people, it has been successfully used instead of chemotherapy. ATRA is also used as a long-term maintenance agent. Side effects from ATRA include fever, dry skin, rashes, mouth sores, increased cholesterol, and swollen feet. These go away with stoppage of the drug.
  2. ATO: Arsenic is poisonous when given in large amounts. However, it was discovered that it could be used to treat APML with effects similar to ATRA. It is used alone, in combination with chemotherapy, with ATRA, or with both. It helps control the growth of cells and also helps in long-term maintenance therapy. ATO dosage needs to be monitored as it can cause heart rhythm issues.

With both these drugs, there is a significant side effect, known as differentiation syndrome. Often seen during the first cycle of treatment, this happens when the leukemia cells release a protein into the blood. Anemia (or anaemia) is a medical condition which generally occurs in a human body because of the lack of required healthy red blood cells (RBCs) or hemoglobin in the blood. Diagnosis of anemia is done through a blood test, a man is anemic when his hemoglobin level is less than 13.5 gram/100 ml and a woman is anemic when her hemoglobin level is less than 12.0 gram/100 ml.

Risk factors: People suffering from long-term or chronic diseases such as cancer are more susceptible of developing anemia. Hemoglobin is the salient constituent of red blood cells and is an iron rich protein. It provides the red color to the blood and binds oxygen. Red blood cells carry this oxygen to all the vital tissues and organs of the body. Due to lack of appropriate amount of red blood cells, the amount of blood required in a body to function normally is also low. One of the most prominent causes of occurrence of anemia is the deficiency of required amount of nutritional iron, vitamin B-12, and folate in the body. This iron is needed for the formation of hemoglobin. Apart from these causes, the other causes of anemia comprise of the following:

  • Loss of blood
  • Insufficient production of red blood cells (inadequate intake of iron, vitamin B-12)
  • Fast rate of destruction of red blood cells (disorders of liver and hemorrhage)

Diagnosis: It is relatively easy to detect an anemic person. The symptoms of anemia include pale appearance, complaint of cold, giddiness, unsteadiness, laziness, particularly when the person is in action or is standing up. Sometimes people suffering from anemia crave for eating clay, ice, or even dirt. Also, there are complains of exhaustion along with issues of constipation and concentration. Other indications of anemia are brittle nails, gasps for breath even in mild activities, and pain in the chest. Since the level of oxygen in the blood is low, there could be even scope of a heart attack.

A person suffering from anemia is usually diagnosed on the basis of a complete blood count. The blood test brings out if serum folate levels are quite low. In case the anemia is severe, sometimes doctors take the help of erythropoietin injections so as to foster the production of red blood cells in the bone marrow. In case there is bleeding or if the level of hemoglobin is low, it might become essential to carry out blood transfusion.

Treatment: Anemia is straightforward to diagnose and treat, but at the same time could prove to be deadly if it is left undiagnosed. Exocrine Pancreatic Insufficiency (EPI), also called pancreatic deficiency, is a disorder where the pancreas is unable to produce the sufficient amount of enzymes that are required to digest food. The pancreatic enzymes help to break down and absorb nutrients from the food in the small intestine. So, this disease causes nutritional deficiencies.

Causes of Exocrine Pancreatic Insufficiency
This condition is generally caused as a complication of other diseases because EPI develops only as a result of severe damage to the pancreas. The following reasons can cause EPI:

  1. Inflammation of the pancreas: After surgery in the pancreas, stomach or the intestines, there may be inflammation in the pancreas as a post-surgical complication. A high content of triglyceride fat in the blood can also cause pancreatic inflammation and hinder the secretion of the enzymes.

  4. Shwachman-Bodian-Diamond syndrome: This is an autosomal recessive genetic condition where enzyme-producing pancreatic cells are not formed properly. This rare disorder causes a number of associated disorders like bone marrow diseases, skeletal defects and exocrine pancreatic insufficiency.

Symptoms of Exocrine Pancreatic Insufficiency
The symptoms appear in the middle stages of the disease when the process of absorption of nutrients has already been affected. The common warning signs are:

  1. Digestive problems like gas, diarrhea and indigestion because the food remains mostly undigested.
  2. Severe and frequent stomach pain in the lower abdominal region
  3. Greasy stools due to the excretion of undigested fat
