Hi,

I am Dr. Anjuli Dixit, Gynaecologist. I am talking about preventive health care in pregnancy. I will tell you about 2 vaccines - Flu and T-dap. These vaccines are given to both mother and child at the time of the pregnancy. They both are at the risk of developing disease due to the lack of immunity and infections. With the help of these vaccines, both are safe and protected. This vaccine can be given if there is any episode of flu. Otherwise, we give after 26 weeks. Baby is also able to survive or handle all those infections. These vaccines can be given at any reputed nursing homes or by any gynecologist at the clinic. These should always be given at any pregnant patient between 26-36 weeks.

I am Dr. Anjuli Dixit and today I am today talking about signs of infertility which you can diagnose early and then you can consult any gynecologist nearby your area to treat your infertility at the earliest. These signs differ in female and male. In the female, the signs are Dysmenorrhea, intermenstrual bleeding or spotting, irregular periods, weight gain with facial hair growth, hair growth on abdomen and chest, pelvic pain. It may also be associated with infertility. In case of chronic pelvic pain, you should consult a gynecologist to rule out any gynaecological abnormality in your pelvic area. Other signs and symptoms include white discharge.

If it is a long-term then you should consult any gynaecologist because this white discharge may be due to pathogenic development of some organism in your vaginal area or in your cervical area which may lead to infertility. If this white discharge is changing according to your cycle then it is normal but if it changes the colour then it is pathological and you should meet any gynaecologist. Besides these, if you are having intermenstrual spotting and bleeding then this may also be because of any polyp or any fibroid present in uterus which may lead to infertility. So you should consult at the earliest at a gynecologist. In cases of male, the signs are different.

These are difficulty in ejaculation, difficulty in erection or difficulty in penetration. This may also be because of hormonal imbalance or due to some abnormality in the genital organs. So in case of these changes you should consult any male andrologist for infertility process. Besides these symptoms their may also be low sperm count or abnormal sperms present in semen. This may also lead to male infertility. Some swelling in genital organ also lead to or some swelling in testis also leads to infertility. So in case you find any problem you should consult at the earliest to an andrologist or gynaecologist to diagnose any problem which may lead to infertility early in the married life.

I am Dr. Anjuli Dixit running a nursing home at sector 84 J-Block, Greater Faridabad. I am going to talk about something regarding cervical cancer. Cervical cancer is the second most prevalent cancer among women which infects about 80 to 90% of the girls before attaining menarche. So it is important to discuss something about this. What is cervix? Cervix is the part connecting uterus to the vagina of the woman and this is the part through which sperms passes into the uterus. So this is the most important organ which gets involved during sexual intercourse. So this is the part which gets infected even before attaining menarche and we should know something regarding this because it is the most common cancer of the women. What causes cervical cancer? Cervical cancer is caused by HPV virus which is present in vaginal secretions and in the environment which infects vaginal discharge and causes cervical cancer.

How it infects the woman? It may cause infection through skin, through sexual intercourse or through secretions which may infect the women. Who are the potential candidates for the cervical cancer? Girls after attaining menarche, girls which start early sexual intercourse, in girls who are having multiple partners and cigarette smokers are also potential candidates for this cervical cancer. How does it present? It may present through genital warts, through vaginal warts, through cervical erosion and any long-lasting infection which infects the cervix. Pain, abnormal vaginal bleeding, pelvic discharge which is long lasting, pelvic pain leading to dyspareunia these are the symptoms which it affects the women and which causes women to seek medical advice. When we should seek advice? We should get Pap smear done even at the age of 20-25 years or just after becoming sexually active.

And early detection always improves the outcome of the cervical cancer treatment because it is a very slow growing disease and its effect, infects the part early and it presents the disease in a later stage. So even in early 20s or 30s women may get infected with HPV but it may present the disease at the age of 50. Earliest detection always brings better outcome. We should get Pap smear done at the age of 25 years. We can even prevent the infection before being infected by cervical cancer vaccine which can be given at the age of 9 years.

It may be given in 2 doses between the age of 9 to 14 year and then 3 doses at the age of 14 years and it can be given up to the age of 45 years. It will prevent cervical cancer because this is the only cancer which can be prevented by vaccine and early detection may lead to better outcome of the disease so early detection and treatment always prevents the cervical cancer spread.

My self Dr Anjuli Dixit speaking on the topic of Polycystic Ovarian disease. Polycystic Ovarian disease is a syndrome affecting most women of reproducing age group. It includes many melodize such as hormonal imbalance, obesity, menstrual irregularity. Generally, it is according to rotatom criteria developed in 2003 any of the 3 features if found they are labelled as Polycystic Ovarian disease. This includes ovulation, metabolic dysfunction or any polycystic ovaries diagnosed on ultrasonography if those not included in diagnosing this as Polycystic Ovarian but insulin resistance and Hyperinsulinemia are generally found in patients of Polycystic Ovarian disease. And it is, through studies we have come to know that Polycystic Ovarian disease patients have Hyperinsulinemia and insulin resistance in 40 to 50% of the patient either they are lean or they are OBs. In OBs patient’s insulin resistance is found to be in 80% of the cases and we treat them according to this insulin resistance and we try to reduce this insulin resistance.
