Good morning. I am Dr Vijay Kakkar, senior consultant plastic and cosmetic surgeon in kakkar health group. Today, we will be discussing about Rhinoplasty.

As we know Rhinoplasty is commonly known as Nose surgery. Most of the patients complain about de-shape of the nose, depressed nose or very broad nose. Some patients come with the sharp nose and some patients come with teh different photographs of different celebrities and  ask to get the similar nose.

Let us see what all we can do in nose surgery.

If we see the nose, we see two parts.

1) Bony part

2) Lower soft part is the cartilage part which is internally supported by a separate septum

So, if we want to change the nose shape, we have to change all the structure like bone structure, soft tissue or cartilage structure and we have to re allign the septum in many cases.

Nose can be changed in two ways;

1) Internal approach                                       2) External approach

In Internal approach, we go through inside the nose so taht no cut or scar is visible from outside and change the shape of the bone, cartilage and septum. In this way we can achieve our desired nose shape. We can not completely chnage the shape of a nose. It completely depends on the facial profile. We analyze the length of the nose, chin, upper face. After analyzing we choose the best suited nose on the patient's face.

After the surgery, the patient is put into some kind of bandages or dressing for a week or 10 days and then we remove the bandages. The swelling on the nose after the surgery will remain for 8-10 days.

Once we do nose external approach, we give a small cut on teh central part of the nose which is called columella and lift the whole nose through this sand we can expose the whole nose. This kind of surgery is better done because we are doing all these surgeries under vision.

Again after the surgery, we put bandage for a week or 10 days time. Swelling of a nose may remian from 6 weeks to 6 months or may be in some cases upto 2 years.

This is the major part of the nose which we modify.

Good morning, I am Dr Vijay Kakkar, a senior consultant cosmetic and plastic surgeon. I am working in Kakkar health care group which is a clinic of long standing and have been doing all kind of cosmetic surgeries. We will be discussing Gynecomastia or a male surgery today.

As we know Gynecomastia is a problem in the young adult in which the male breast starts increasing. The problems start in the teenagers. Once there is little bit of problem in hormones. In the case of Gynecomastia, there must a slight elevation of the female hormones during the developing period. If the female hormone is more in the developing period of say,13-17 years of age. This gets hypertrophic and increase in size. There may not be any investigating factors.

In certain cases, it may be associated with some other diseases also. In such cases we have to find the cause of Gynecomastia. Where as in most of the cases we do not find any cause for increase the size in male breast. Once the patient of male breast comes to us, we examine him and analyse what kind of grades of gynecosmastia the person is having.

IN most of the boys or young adults are going to gym due top which muscle size increase, in such cases the gynecosmastia becomes more prominent. It is a myth that going to gym decreases the gynecosmatia, rather it increases. Once these people go in the society, they can not remove the t shirt and there are lot of problems and pressure that they can not go for swimming or they can not go to common public places wearing white t shirts. In such cases, the young adults suffer from psychological pressure. So we need to correct these problems.

It is a very simple surgery and a day care kind of surgery. We call the patient in the morning. We give an anaesthesia which is either local or general anaesthesia. Most of the fat can be removed with the process of Liposuction. It is a very minor surgery. We make a 2-3 mm size of cut and put a kind of cannula and lot of saline into the breast and suck all the fat. After sucking all the fat, we analyze if there is any breast gland left behind. If there is any breast gland left behind then we excise that breast gland with the help of a small cut which ios given in the infra cellular plane and that scar is also not visible.

After the surgery, the patient is given garment to wear for the month approximately. The swelling will go down within a week to 10 days time. The patient will be fine within a 6 days of time and patient will be back to normal. It is a permanent procesdure where no scar no cuts are left.

Facts and myths about cosmetic surgeries

I'm Dr. Vijay Kakkar. I'm a Senior Consultant in Cosmetic Surgeon. I'm attached to different hospitals like Max Hospital, Saroj Superspeciality Hospital and Mata Chanan Hospital in Janakpuri. Today I'll be discussing something about cosmetic surgery which most of the people are not aware what kind of surgery it is.

There is some basic difference between plastic surgery and cosmetic surgery.

In plastic surgery basically, we try to achieve normal things while in cosmetic surgery person is already normal and we're trying to achieve supernormal things. That says someone is having a nose which he wants to be very sharp and thin and to look extra smart so we change that. This comes under cosmetic surgery. Similarly, if somebody's having problems with the face like if the face is loose due to age, or having wrinkles on the face, ears are more prominent, chin is short, we can do all these changes to make a person look better. Similarly cosmetic surgery can be done on all parts of body, it can be cosmetic surgery of breasts, it can be tummy tuck like we do a different kind of liposuction in which we reduce body fat, in the breast segmentation we can increase size of breasts that can be done through implants or through draftings, then we can remove heavy, flabby bags from the person like if the arms are lost and hanging we can make them tightened with surgery, it becomes really good.

Similarly, all areas of the body can be tackled with cosmetic surgery and in cosmetic surgery we also cover the non-invasive kind of surgery like Botox, like the fillers, if there are any depressions in the person we can just do instant fillers and he can look much better, with Botox we can remove all the wrinkles. All these procedures are short procedures and they're called Lunch Hour surgeries or Office procedures. And if somebody is having baggy eyelids or lose eyelids and looks much older than his age we can do Blepharoplasty in the eyes and make him much younger and better looking. Then we've lasers in our armamentarium which can remove scars, which can make the face much younger and better. Then we also do a lot of Hair Transplants. In the hair transplant, we take the patients own hair from the back and put it in the front so that your look is improved and you look much younger. These are basically the kind of cosmetic surgeries which we do routinely every day.

Hair Transplantation procedure and its advantages

Hello good morning. I am doctor Vijay Kakkar. I am a senior consultant in plastic surgery. Today we will be discussing something about Hair Transplant.

There are many myths and many ads in the market but what is hair transplant? Who should do it and who are doing it? What are the problems? Recently we have writer very sad news about the death during hair transplant.

So let us discuss what it is and who should do it and what are the precautions to be taken and not to get fleeced by people who are not qualified are competent to do this kind of surgeries. Basically in hair transplant we distribute the hairs that means you have permanent hair on the back of the scalp which we take it and then put it on the front to make it your hairline much younger and healthier.

So now in hair transplant there are two kinds of procedure-

One is FUE that is follicular unit extraction and FUT method.

FUT is a strip kind of method that is we take a strip from the back of the scalp from the permanent donor zone which these hair which we take are resistant to male hormones and put them on the front area so that they do not fall off and again extraction method is little different but putting the here is the same in both the techniques. In FUE we take out individual hairs. This is almost called a scarless surgery. All the ones we shave it off you can always see the small tiny dots of harvested hair.

Now there are two basic different people go most of the time after a FUE that we don't want a cut or we don't want a scar but we must understand if you want more hair then we have to go for FUT method only that is sticker strip. Because a strip gives a large volume of hair and if you take a strip then you can always go for a second session of FUE. While if you take FUE if you talk about permanent zone there are hardly 2000-2200 here in the permanent zone which we take at one go. If we are taking more than that that means we are going through a non permanent zone and these hair will fall off.

So we have to be careful about these issues and once we transplant hair then we have to take certain cares that initially in the beginning there is a little swelling, the little pain when doing we give good antibiotic and it has to be done in a good setup and clean and all surgical steps have to be followed because it is a surgical procedure and should be done with surgical Precision and should be done by either qualified Dermatologist who is qualified to do this kind of surgery and has been trained or preferably by a surgeon who are routinely trained in this kind of surgery and do it routinely.
