I am 36 years old having diabetes type 2. (as told by endocrinologist) last year in september 2020 I was suffering from diabetic papillopathy and I was treated for the same and it was cured. Now I got for eye check up after every 3 months and now my eyes are ok. Now my hba1c is 9.2 and fasting sugar is 190 and post 230. Now since I was suffering from diabetic papillopathy can I take tablet istamet 50/500 once a day? Will it have any effect on my eyes since I was having diabetic papillopathy? Thanks.

Insulin will be a better option for you.

Your sugars are not controlled fasting blood sugar should be less than 130 post lunch should be less than 180 and hba1c less tha 6.5 for your age for preventing the papillopathy to come back again you have to strictly control your sugar. Istamet is a good drug but you need some more better drugs for your sugar to be under control for preventing diabetic complications such as diabetic retinopathy ,nephropathy, neuropathy etc.

Hello, thanks for the query. I have seen the details. Pease tell me in spite of having such a difficult condition (papillopathy) why you have not kept blood glucose under strict control? It is mandatory to have strict glucose control when some has affection to retina. Now with 9.2 hba1c%, it is essential that you start therapy with insulin, get strict glucose control and then may be shift to oral drugs. Sitagliptin is safe enough. But your present situation demands insulin. If you have any questions please do come back. Also it is better to consult the endocrinologist whom you had consulted earlier. With present situation, it is absolutely essential to start insulin. Thanks.

Sir, I would like to suggest test like c peptide assay & gad antibody to confirm the type of diabetes. Their is a distant possibility of lada. Insulin is the only option if such is the case. I also would like to know whether you are obese and how diabetes was suspectes and diagnosed initially.
