Mr. lybrate-user, Thanks for the query. In diabetes peripheral neuropathy is a common occurrence, causing either burning or tingling and numbness in feet and legs. To reduce that you need to achieve strict blood glucose control with fasting < 100 mg, PP 150 to 160 mg & HbA1c% <6.8. Therefore, apart from appropriate medication, it is necessary to take restricted diet, that gives exact calories based on your ideal body weight (62 kgs), extent of physical activity and blood glucose levels. Plus if need be certain medication like Methylcobalamine and / or pragabalin may be beneficial. Thanks.

Mr. lybrate-user, Thanks for the query. Diabetes T2 or T1 both can only be well controlled with appropriate drugs, diet and exercise. There is no cure possible at all. If proper medication is not taken and control is not kept, then it can lead to serious long term complication of diabetes, affecting almost every single vital organ of the body. Thanks.

Mr. lybrate-user, thanks for the query. The description suggests there is probably an infection of umbilicus. So it is better to show it to your local doctor. As you may need painkillers and antibiotics. Plus there may be a need for cleaning and dressing of the wound. Thanks.

I have consulted Lybrate doctor and took surgeon foo 4 days for giddiness, Problem is still there (10%) but only while getting up from bed. Doctor asked me to go for further checkup if giddiness persists. I am Diabetic and had spondylosis problem 4 years ago. Pl. Advise.

Mr. lybrate-user, Thanks for the query. Giddiness can occur due to multiple reasons. Using Stgeron does help in controlling it. But you may have to take the drug for a longer period. Sometimes problems with inner ear (vestibular portion) can cause such symptoms. So please consult an ENT surgeon and follow his guidelines. That may help. Plus it is necessary to keep blood glucose as well as BP is good control. Thanks.

I am a known case of thyroiditis detected in the year 2015 but wasn't on any medication as TSH Profile was WNL, though I took thyrovol supplement for 3 months when my TSH was fluctuating sometimes high and sometimes low. I had spontaneous conception. During pregnancy I had low TSH and was started on tab eltroxin 50. My post pregnancy TSH aftr 2 months of delivery was 0.01 and after 4 months it has com to 0.18 (2_4. 2) with T3 4.09 (0.6-1.7) and T4 0.77 (0.34-4.25) though I had continued tab eltroxin 25 till now. What should I do now as my TSH is still below normal range. M having severe hair fall, lack of appetite, and irritability, I feel sleepless sometimes.

lybrate-user, Thanks for the query. You are being treated with thyroxine means the condition is hypothyroidism. When a person is on treatment for that with levothyroxine TSH should be maintained <3 mU/L, to be free from all the symptoms. But 0.18 mU/L is bit too low. So you need to reduce the dose to 12.5 mcg per day. Then check TSH again after 6 weeks, to see if desired level is achieved. Thanks.

I just got diagnosed sugar fasting-147 and after eating food-273. I am very worried and tensed that everyone is saying now I have to take medicines for whole life. But I don't want so. Is there any other way to cure this as I started walk, exercise, started taking healthy food cut carbs and fats. Give me solution please.

lybrate-user, Thanks for the query. Fasting blood glucose level 147 mg & PP 273 mg, both confirm that you have diabetes (T2). Diet control and exercise do help in controlling blood glucose levels, but once diabetes is confirmed, then there is absolutely no option but to start medication with out any further delay. Lifestyle modification withexercise, diet control and weight management are useful in pre-diabetes, to prevent progression to diabetes. But once it is confirmed, unfortunately there is no other option. So please start proper medication and also take restricted diet and exercise daily for 1 hour. With this you will be able to live an almost normal life. Thanks.

Mr. lybrate-user, Thanks for the query. Addiction to "cough syrup" is definitely bad. But which cough syrup are you taking is not mentioned. As there are different ingredients in different products. Most often Codeine addiction is observed (Corex cough syrup). Since sough syrups also contain a good deal of sugar, taking those is bound to give additional sugar increasing blood glucose. So please tell me which is the product. To get out of the addiction, it is better to get in touch with a psychiatrist locally and also if there is a deaddiction center near by. Because this condition can not be treated by just advising on line. Thanks.

Mr. lybrate-user, Thanks for the query. Insulin dose decided on the basis of blood glucose level both fasting and after food. Plus there are different types of insulins, it is not which one you are using. There are rapid acting and long acting insulins plus combination of both. It is given in units per dose, not as ml. Taking excess insulin is not advised. It causes hypoglycemia. Thanks.

lybrate-user, Thanks for the query. It is always better to wean the baby over a few weeks, by reducing the number feeds slowly. That also helps less production of milk. It usually takes a few weeks for milk secretion to completely stop. Initially you will have to take out the milk a few times in a day. Since child is not being fed, that itself will help but it will take longer time. There is no immediate solution. Thanks.

Mr. lybrate-user, Thanks for the query. Blood glucose fasting 98 mg is normal. However, PP 176 mg, shows that you have pre-diabetes state. If proper care is not taken like food restriction, regular exercise and weight control, then you will progress to diabetes. Presently no medication is needed. Only adapting a healthy lifestyle. Hi I'm 37 years old I hv hypothyroidism for which I tke eltroxin (Mon to Thurs) 100 mcg & (Fri to Sun )125 mcg & my Tsh levels 1.15, T3 1.03, T4 15.90 these days I always fell like spinning in my head so I checked my Bp level it keep fluctuating sometimes 130/90,125/85 120/88 like so on please can you give me any suggestions bcoz we r planning for ivf treatment as we don't hv kids.

lybrate-user, Thanks for the query. For IVF your TSH level is quite right. There is no need to worry. Even the BP readings are very much normal.(diastolic should ideally be <80 mm Hg). That is unlikely to cause any symptom. Thanks.

Hello, Thanks for the query. It is difficult to give any opinion as to why suddenly you have increased heart rate or may be palpitations. I need to know if you are having hyperthyroidism (which thyroid problem), high BP and any other cardiac condition. Unless some of the other details are known, no opinion can be given. Usually in diabetes there is a dictum "No fasting & No feasting" The reason being if some one eats too low quantity that can lead to hypoglycemia and taking excess is not at all good as it leads to high blood glucose levels. However, I am unable to understand the question - what diet should be taken? Fasting means no or less food. So you can decide what can be taken. Only thing is to avoid too much of fried and high carb foods. Take a lot of green leafy vegetables and fruits. Thanks.

Hello, Thanks for the query. It is necessary to maintain TSH <3 mU/L with adequate dose of levothyroxine to be free from all the symptoms of hypothyroidism including swelling of the face. Since there is no mention about your TSH level, it is difficult to comment. Thanks.

lybrate-user, Thanks for the query. When on treatment for hypothyroidism, it is essential to control TSH very well. It should be maintained <3 mU/L for being free from all the symptoms of hypothyroidism including swelling of face and legs & feet. Therefore, it is necessary for you to increase the dosage of thyroxine tablets to 12.5 mcg, then check TSH after 6 weeks. By then if TSH level reaches the desired number, then continue the same if not then dose may have to be increased by another 12.5 mcg and TSH should be checked again after 6 weeks. Thanks.

Thanks for the query. Insulin is usually recommended to be taken half an hour before food. Because by the time digestion of food converts it to glucose, insulin is available for action. Insulin does not cause any other side effects. Occasionally if food delayed or if inadequate food is taken, then there is a possibility of glucose level going down (hypoglycemia). lybrate-user, Thanks for the query. HbA1c% 6.1 signifies you are in a pre-diabetes state. However, after food glucose level (128 mg) is normal. So it is clear that there is no diabetes. If you have a family history, then there is a need to adapt a modified lifestyle, by which it means - avoid too much of sweets, deep fried foods, pastries, bakery items, foods made from maida. Use whole grain flour for roti and chapati. Increase the intake of green leafy vegetables, salads and fruits. Exercise daily for one hour, brisk walking is a good option. With this lifestyle you can prevent developing diabetes or postpone it. Thanks.

Mr. lybrate-user K, Thanks for the query. Diabetes develops because a person has either less secretion of insulin or there is resistance to its action or both the conditions are present. Once diagnosed (with Fasting 126 mg and above, PP 200 mg and above & HbA1c% 6.5 and above), it can only be controlled by proper medication, diet and exercise. Plus maintaining near normal weight is also very essential. To control diabetes it is essential to adapt a modified lifestyle. Which means taking restricted diet, that gives exact amount of calories needed based on ideal body weight, extent of daily exercise, blood glucose levels. Refraining form alcohol and smoking. As regards efficacy of other alternative therapies being effective in controlling diabetes, well, there is no clinical evidence of it. Unfortunately lot of tall claims are made by some saying diabetes can be cured, which is totally untrue. If you still want to try then it is your choice. Thanks.

Mr. lybrate-user, Thanks for the query. Swelling in lower limbs can occur due to a multiple causes like uncontrolled high blood pressure, some heart related issues and even due to improper functioning of kidneys. Sometimes certain drugs also lead to that, like Amlodepine used in reducing BP and pioglitazone used in treating diabetes. EVen over weight and inactivity can also be a cause. So please check his kidney function test, BP readings and also level of physical activity. Thanks.
