I am 68 year old and have diabetic type 2 and I am taking tablet amryal mfort 1 mg before breakfast and after lunch ajadua25/5 mg after lunch and one tablet amryalmfort1 mg before dinner but my blood level fasting 110 to140 and after 2 hrs 160 to 200 pl advice me glycomet 500 sr benefited to normalise sugar level.

Hello, thanks for the query. I have seen the details mentioned. The glucose levels mentioned are a bit higher than ideally expected ones. Since there is no mention of hba1c%, it is difficult to comment on overall control. There are quite a few options that can be considered as additional drugs for improved control. But most important is to give a closer look at your dietary intake and daily exercise schedule. With a bmi (body mass index) of 2633 kg/sq meter, you are carrying an extra weight of almost 10 kg. If that is reduced by even 6 to 7 kg, the glucose levels will be better controlled. Thanks.

Hello, thanks for the query. I have seen the details mentioned. The glucose levels mentioned look quite good. What is the current hba1c%? That is important to know how the diabetes is controlled. At your age ideally fbg should be < 100 mg, pp 150 to 1660 mg & hba1c% < 6.5%. That kind of control will help in remaining fit and free from long term complications of diabetes. As regards taking ayurvedic medication you can take that as an add on therapy. But do not discontinue the present medications. Thanks.

Hello, thanks for the query. I have seen the details mentioned. Your glucose levels seem to be in good control. As regards taking vitamin supplement there is no hard and fast rule. You can decide as you want. Usually because of dietary restriction vitamin & mineral supplements are advised. Hence, if there is a need then one needs to take regularly. Uneed to start medicine immediately. Along with medicines follow diabetes diet which simply means eating the healthiest foods in moderate amounts and sticking to regular mealtimes. A diabetes diet is a healthy-eating plan that's naturally rich in nutrients and low in fat and calories. Key elements are fruits, vegetables and whole grains. U can take (moderate amount) grapes. Apples. Berries. Citrus fruits. Papaya. U should avoid sugar-sweetened beverages. Sugary beverages are the worst drink choice for someone with diabetes. Trans fats sugar, cake, pastries mithai/sweets chocolate fruits like mango, litchi etc. White bread, pasta fruit-flavored yogurt. Sweetened breakfast cereals. Flavored coffee drinks. Honey 1. Don't take tea empty stomach. Eat something like a banana (if you are not diabetic) or soaked almonds or any seasonal fruit. No only biscuits or rusk will not do. 2. Take your breakfast every day. Don't skip it. 3. Have light meals every 2 hours (in addition to your breakfast, lunch n dinner) e.g. Nariyal paani, chaach, a handful of dry fruits, a handful of peanuts, seasonal fruit, a cup of curd/milk etc 4. Finish your dinner at least 2 hours before going to sleep. 5. Maintain active life style 6. Avoid fast foods, spicy n fried foods 7.take a lot of green vegetables n fruit. 8. Drink lot of water. 9.curd is good for u. Exercise in the form of yoga, cycling, swimming, gymming, walking etc. Now about point no. 5. You should join a gym for weight training. Hello, thanks for the query. Actarapid is a human insulin with rapid action. Apidra is also a rapid acting insulin but the compound is different. Actions are similar as both have rapid action and that stays for 4 to 6 hours. Both are used to control after food glucose rise. Dosage can be similar. Thanks.

Hello, thanks for the query. Both the products are same. The er refers to extended release, which means the drug is released slowly over 12 hours in the gut. That helps maintain a longer therapeutic level. It is beneficial more so if a person has any side effects like gastrointestinal disturbances if plain metformin is taken. Both are fine. Thanks.

Hello, thanks for the query. The person having uncontrolled diabetes, high uric acid, high potassium, low vit d3 & b12, can take the supplements referred. But most important to be healthy and fit is to first & foremost achieve a strict blood glucose control with fbg < 100 mg, pp 150 to 160 mg & hba1c% < 6.5%. Without this all the supplements are not going to do any good. Supplements are helpful only when basic condition is well controlled. Thanks.

Hello doctor I have been taking afoglip m 500 mg in the morning after breakfast and carbophage 1000xr in the night for 7 years. Then last whole year with diet and exercise I slowly came off these medications was only taking carbophage xr 250 mg in the night past 3 months. Last month my hba1c was 6.9 and fasting 110, so now I have been taking carbophage xr 500 mg last 3 weeks now my fasting is 109 and 2 hrs pp after lunch or dinner is 170. Besides I have developed some numbness, tingling pain in feet .can you please suggest what I can do any medicine to take lower sugar.

Hello, thanks for the query. Madam, I have gone through the details mentioned. It is good to know you have been able to control the glucose levels better since some time. The levels mentioned (fasting & pp) are quite good so is the hba1c%. As regards the development of tingling & numbness in the feet, these symptoms are almost always seen when diabetes is present for a few years. This is due to the development of peripheral neuropathy, which a common long term consequence of diabetes seen in a large number of patients. This does tend to develop even when the control is fairly good. In this situation apart from maintaining well controlled glucose level, regular exercise and continuing to lead a modified lifestyle, certain drugs may be needed to control troublesome symptoms present. So please let me know your height, weight, dietary pattern and extent of daily exercise. Also let me know the serum creatinine and urea through the private conversation, so that I will be able to give specific guidance and prescribe the necessary medication if required. In this communication we are supposed to give only general guide lines, no drugs can be recommended. Thanks.
