you are very right about normal range of tsh levels. At 8.5 milli units/ml, with normal t4, the condition is classified as sub- clinical hypothyroidism. This normally is not treated with any drugs. Treatment is advised only once the levels cross 10 milli units or there are troublesome symptoms with lower tsh levels. As regards food intake you have to have a balanced diet, but avoid certain foods like - soy beans, soy containing products, cabbage, cauliflower, broccoli, peanuts, radish, mustard and coffee. Consume good deal of green leafy vegetables, salads and fruits. Use whole grain flour for making chapati or roti. Do not use refined flour like maida. If you are taking non- vegetarian diet then take lean meat, skinned chicken and fish. Exercise regularly, that is important. Further, if one is desirous of having conception, then before conception tsh levels have to be brought down to around 2 mill units/ml. Keep that in mind. Thanks for the question.

It's cause of concern that your insulin is not acting to neutralize the sugar while you are taking good medicines prescribed by your experience doctor. This is non response of drugs. It's not fault in treatment by your doctor. I would suggest the support of ayurveda medicine as support to your current medicine. Both won't counteract with each other. Observing the response of this integrative approach, further directions would be decided. If you agree, I would mention the first phase of safe, side effect less, easy to take and economic medicines which will prevent complications also.

I was diagnosed with dm 2 about two years back. Iam on amaryl-m1 bd & has hba1c of 7. I started feeling extra sensation under the feet, heaviness in foot, leg, thighs from about a month or two, some pain in hip area and finally difficulty in lifting one leg about 2 weeks days back along with knee weakness in one leg although other leg has been affected to a lesser extent till date. I never experienced very sharp pain & 2 weeks back had mild pain in my buttocks for about a week. The loss of movement in the 2 directions direction in the leg & knee weakness was quite quick about 10-12 days back. Now I do not feel any pain but had some falls primarily on account of knee weakness or weakness in right leg. Mri scan, ct scan were ok but problem was reported in nerve conduction test in both legs but mainly in right leg. Was diagnosed with peripheral neuropathy or diabetic amyotrophy which correlates with my overall symptoms. Doctor put me on 8 day course of prednisone (40mg-10mg) with insulin. My sugar with amaryl m1 as on now is under control & I take light food with reasonable physical activity & sugar levels now seems to be ok 92 fasting /140 pp. Also started visiting a physiotherapist from today. As of know there is appears to be some improvements in my walking (can walk 15-20 min at a stretch inside home) though weakness in one leg & knee persist. I cannot drive due to difficulty in movement of my right leg when trying to move it sideways while sitting. It is also not possible to lift the right leg against gravity when laying down though backward lifting & movement on sideways when lying on one side are intact. Have been prescribed pregaba m 75 (hs, neurkind+. Have no pain of any type. My questions 1) has peripheral neuropathy reached a plateau ie. Stopped or not 2) is pregabalin needed in my case as I have no pain, i. E. Whether it play a role in nerve regeneration or is overall beneficial in such situation 3) can I expect improvement in my right leg in 1-2 months time with sugar control & exercises.

1. Peripheral neuropathy is a on going process even in normal individuvals as age advances due to deficiency of vitamins mainly b12 2. In your case pregabalin is needed for some time 3. Nerve regeneration is impossible for anyone 4. Definitely you can expect improvement in the right leg in 1-2 months with good control of sugar & exercises.

Study Suggests Paracetamol Can Alter Fertility of Female Offspring!

Study Suggests Paracetamol Can Alter Fertility of Female Offspring!

As per a study conducted by the Copenhagen University Hospital, it was revealed that consuming paracetamol during pregnancy can be detrimental for your baby, as it can weaken the development of the reproductive system of female offspring. The research was published in Endocrine Connections journal.

Although, paracetamol is an over-the-counter treatment for pain relief that is commonly taken by pregnant women worldwide. The study was conducted on three individual rodents and the results indicated that the rodents, who were given paracetamol during pregnancy at doses equivalent to those that of a pregnant woman may take for pain relief, produced female offspring with fewer eggs.

Further, Dr. Kristensen said, "Although this may not be a severe impairment to fertility, it is still of real concern since data from three different labs all independently found that paracetamol may disrupt female reproductive development in this way, which indicates further investigation is needed to establish how this affects human fertility."

5 Important Tips to Recover From Cold and Flu

MBBS, MD - Pulmonary Medicine
Pulmonologist, Faridabad
5 Important Tips to Recover From Cold and Flu

1. Stay home, rest and take plenty of fluids

Take plenty of rest at home which will give your body time to heal. Water, juices and comforting warm beverages will help prevent dehydration and turn thick viscid phlegm to watery mucus which can be easily coughed out.

2. Inhale steam

Steam inhalation: warm humidified air will ease the congestion in your nose and lungs.

It will help in thinning out the mucus and make breathing better. Alternatively, you can sit in the bathroom with the shower running hot water till the bathroom is filled with moist steam.

3. Humidifier

Use humidifier or vaporizer to moist the dry air in your house/room. It will help in quicker recovery from flu.

4. Over-the-counter medicines for a cough, aches and fever

You can try over-the-counter medicines such as paracetamol, nasal decongestants and cough syrups after consulting with your local doctor. Never give aspirin to people younger than 19 years unless prescribed by a doctor. Lozenges will help to soothe the throat. Avoid taking antibiotics as flu is a viral infection and not bacterial unless prescribed by a doctor.

5. Saline solutions

Use saline nasal sprays or drops to unblock the nose. Put some drops or spray in the nose and gently blow the mucus and saline out. Gargles with a warm saline solution will sooth down the throat and help to clear the infection.

These measures will help ease your suffering and can result in a quicker recovery. Take appropriate precautions to prevent the spreading of flu, like covering of mouth while coughing, nose while sneezing especially when near the children.

Consult a Pulmonologist (Chest physician) urgently if you experience breathlessness, wheezing, troublesome cough, high fever, hoarseness of voice, vomiting or other worsening symptoms.

Knee Arthritis & High Tibial Osteotomy - Understanding It In Detail!

DNB, Fellowship in Pediatric Orthopedics
Orthopedist, Visakhapatnam
Knee Arthritis & High Tibial Osteotomy - Understanding It In Detail!

What is arthritis of the knee?

The knee acts as hinge joint and allows flexion (bending) and extension (straightening). The knee is formed by the tibiofemoral joints, where end of the femur (thigh bone) glides over the top of the tibia (shin bone) and the patellofemoral joint where the kneecap glides over the end part of the femur. The gliding surfaces of the knee are covered with articular cartilage which helps the joint to glide smoothly. Over time the articular cartilage can become damaged or 'worn away' and this is known as osteoarthritis.

What is medial compartment arthritis?

Most people with knee arthritis have predominantly pain in the inner aspect of the knee, which is due to medial compartment arthritis. Patients who are born with varus knees (bow legs) are more likely to get medial compartment osteoarthritis. This is because the weight of their body mainly passes through the medial compartment of the knee rather than spreading the load evenly between the whole gliding surface of the knee.

How do you diagnose medial compartment arthritis?

ClinicaFeatures, examination findings and standing X-rays of the knee joint and the patello-femoral joint are needed to diagnose medial compartment arthritis. The X-ray will often show narrowing of the joint space in the medial compartment of the knee which suggest that a patient has medial compartment osteoarthritis. Long Leg standing X-ray of the whole of both of both legs from the hip joints to the ankle joints ( Fig 2), allows us to carefully examine the overall alignment of your legs. They help to calculate the weight bearing axis of your leg and find out where most of the force is passing through your knee joint. MRI would be done too, to assess degree of cartilage damage. It is imperative to know status of other structures in the knee like meniscus and ligaments.

Non-operative treatment

Some patients are advised for a type of knee brace known as a medial offloading brace to trial. To a certain extent this mimics the result of osteotomy surgery by pushing the leg into a more normal alignment and taking the pressure of the damaged medial compartment. This is usually only a temporary solution whilst waiting for surgery. Patients who are overweight often find their knee pain is significantly improved when they lose weight. Simple analgesia such as paracetamol together with etodolac can help with pain and sleep disturbance form the pain.

Who requires surgery?

People suffering from growing cartilage lesions resulting in pain and activity restriction with proved mechanical axis deviation as the cause, would benefit from surgery. Age and extent of cartilage wear determine the nature of surgery. Younger individuals with smaller lesions are good candidates for a joint preservation surgery in the form of a High Tibial Osteotomy. With advancing age and extent of disease, Arthroplasty would be a
more beneficial option.
