Hello Everyone,

I am Dr. Shilpi Bhadani, a plastic surgeon, practicing in Ghorgaon.

So I’ll talk about hair transplantation. Alopecia or baldness is a very common problem nowadays and across younger age group as well as the old age group. Hair transplant offer a permanent solution. In this if basically understood it is the transplantation of hair from one part of the scalp to the other part. In severe cases we may need to use body hair from the axilla or the pubic region of the chest hair even the leg hair, but most commonly in about 99% of the cases we use scalp hair from the back of the head to the front to cover the bald areas. No basically there are two methods for hair transplant. It is either Follicular unit transplantation or Follicular unit exaction. The two methods the implantation are not different that is the putting the hair is almost the same is the method by which we exact the hair is different. So in Follicular unit transplantation or FUT method, we actually take a strip of the hair from the back and the donor area is stitched and then the team of technicians they separate the strip….. the individual follicles from the strip and then they are made ready for the transplantation. Whereas in Follicular Unit Extraction, the individual follicle, the bunch of follicles are extracted and one follicle may contain one hair up to three hair so they are individually extracted and implanted. So there is a myth that floats that FUT is not safe or it leaves a very bad scar and FUE is nonsurgical, so you have to get this very clear that both are surgical methods, FUT or FUE. Hair transplantation is a surgical methods not a nonsurgical procedure and the methods of FUT have improved overtime and we actually now do a trichophytic closure. So the hair… the new hair in the scar actually grows through the scar. So it actually obscure the scar. You can hardly make out that a strip of hair is taken from there and in FUE also. There will be punctate scars that will be left behind. So it’s just a matter of perception whether what you think is right and it actually…. the choice of the procedure depends on the donor area and the recipient area and your level of baldness whether which is used. Sometimes we also use a combination of the two increase the hair yield. Now there are many, you know clinics, which offer that, you know, ‘we offer unlimited hair transplant’ and see donor area is always limited so there cannot be an unlimited hair transplant and also that if a fair amount of density is expected by the person underdoing the hair transplant, you may will have to go undergo a second session to get a good density. So coverage and density are two different things. So initially we target coverage and if you want more density then of course you can a second procedure can be done. The hair that is transplanted grows….. starts growing in about four months’ time and the complete result can assessed after eight months to nine months’ time. And I the follow up period if PRP or Platelet Rich Plasma Theory is done after the hair transplant the hair growth is really better. Some medications are continued after the hair transplant also up to to certain amount of time but it’s a procedure that requires six to eight hours if we are targeting a bigger coverage area and it’s a procedure done under local anesthesia. When choosing a hair transplant center it is important for you to research and the qualifications of the surgeon or the person doing the hair transplant is very important because a lot of unauthorized centers that have sprung nowadays and you safety is the prime concern. As I said that it is a surgical procedure so things may get complicated so you have to choose a center which is well equipped which is well equipped to deal with emergencies if at all there is a need. Usually there is no need but then you safety is the prime concern. So understand that it is a surgical procedure and it has to be done safely and it has to be result oriented.

Hello everyone. I am Dr Shilpi Bhadani, a plastic surgeon in Gurgaon. Today I will speak about Laser Hair Reduction.

Laser hair reduction is increasing in popularity as a method to remove unwanted body hair. The most common areas include the underarms, facial hair, the chest hair in males, the back hair also in males, leg, arms, bikini line. So there is almost no body part which is spared from laser hair reduction. 

But it is important for the people who are seeking laser hair reduction to understand that, "what is the expectation and whether the reality will match the expectation or not?"

So, the first consultation with the laser specialist is important. And one must know the modality that is being used for the Laser Hair reduction. There can be multiple modalities like - Diode, Nd-YAG Q-Switched and IPL. Out of this, Diode and Nd-YAG are equally successfull an popular. However, the gold standard is Diode laser hair reduction. 

The black hair is most effectively targeted and it is mostly targeted in the growth phase. Now, hair grows in a cyclic manner that is why it needs sessions to ultimately reduce the hair growth. And one thing you need to understand that it is not hair removal, it is hair reduction. So, although some people do achieve freedom from hair but gradually you will need maintenance and that will depend on person to person. Gynecomastia - Treating It With Surgery!

Swollen breast tissues in men or boys caused by hormonal imbalances is called gynecomastia. Both breasts or one breast, can be affected by this. At any point in life, from birth to puberty, even in adulthood, one might develop gynecomastia. Most of the times, gynecomastia is caused by hormonal imbalance, but there might be some underlying causes as well. 

Gynecomastia is normally not a serious condition, but it can be difficult to deal with the pain and embarrassment. One may be advised to wait and watch for the regression the gynecomastia but surgery is often necessary to get rid of this condition. 

Symptoms and signs of gynecomastia include: 

  1. Enlarged glands in the breast tissue 
  2. Tenderness in the breast 

The causes that trigger gynecomastia include: 

  1. Hormonal changes: Testosterone and estrogen control the sexual development in men and women respectively. Estrogen is not a hormone that only women produce; men produce it too. When the production of testosterone and estrogen are thrown out of balance, gynecomastia occurs.
  2. Medication: Certain medications such as anabolic steroids, androgens, HIV/AIDS medicines, antibiotics and chemotherapy can cause gynecomastia.
  3. Drugs and alcohol: Substances such as alcohol, marijuana, amphetamines, heroin and methadone cause gynecomastia. 
  4. Health condition: Certain factors, such as aging, hypogonadism (hormonal disorder), hyperthyroidism (overactive thyroid gland), tumors, kidney failure, cirrhosis, liver failure, malnutrition and starvation can lead to gynecomastia. 
  5. Herbal products: Essential oils derived from plants such as lavender or tea tree, used in lotions or shampoos, have weak estrogenic actives. In exceptional circumstances, they may cause gynecomastia. 

Treatment 
There is no definitive medical treatment. The main answer for Gynecomastia is -SURGERY. It is a daycare procedure in most situations. It is done by a Surgeon, usually, a Plastic surgeon does a better job. 

Anesthesia- Either Local or General anesthesia is given to make it painless. My preferred anesthesia is general anesthesia because it makes the person very comfortable during the surgery. It can also be done under intravenous sedation combined with Local anesthesia. 

Procedure- Through a small incision in the area around the breasts the swelling is removed by both Liposuction of the Fat part and Excision (removal) for Breast gland part. As the incision is through the areola and a small incision in the surrounding skin, the final scar is hardly visible. In post bariatric or massive weight loss patients, skin also needs to be removed, where in a scar around the areola border is visible. 

Recovery- Usually can go back to work in 2-3 days time.

Laser Hair Removal - What to Know Before Getting One!

Body hair are typically seen as an unwanted attribute. For this reason, both men and women are constantly trying to keep their body hair free. Common ways of removing body hair include shaving, waxing and the use of Depilatories. However, none of these techniques can permanently remove hair and hence need to be repeated regularly. The only way to permanently reduce body hair is through laser hair reduction therapy.

Laser hair reduction can be used to address and reduce hair on almost any part of the body including the face, arms, chest, back, legs and bikini line. This procedure involves focusing laser light on the hair follicle pigments known as melanin without affecting the surrounding skin. The melanin absorbs this light energy and converts it to heat energy which in turn stops the stem cells in the hair roots from regenerating and burns the roots off. This weakens the hair follicles to the point of falling off. 

Hair removal is considered permanent when the hair follicles do not grow back. After the first session, new hair growth is often lighter and less dense than before. On an average, long term hair reduction may require multiple sessions. Since laser therapy can be used to address only those follicles that are in the growing phase, these sessions are usually spaced 4 to 6 weeks apart. During this time the hair follicles that were in the resting phase can grow out. The number of sessions required also depends on the quality and number of hair follicles, individual’s hormone levels, and metabolism. In some cases where hair growth is very dense, maintenance treatment may be required once in six months or annually. 

The procedure is relatively pain free. During the treatment, the patient may experience a faint prick or sensation of warmth. When treating sensitive areas, topical anesthesia may also be used. This form of laser therapy can take anywhere between 5 to 45 minutes for a single session depending on the amount of area being addressed. 

Laser hair removal is considered a very safe procedure and has minimal side effects if done under supervision and by a trained technician. In some cases, it may cause a temporary change in the colour of the skin of the treated area, but this usually resolves itself without medication. This can be prevented by using a good sunscreen with UVA and UVB protection and avoiding exposure to sunlight.

Remove Acne Scars With Professional Derma Treatments!

With the current self-obsession of the need to look good all the time, acne is seen as a major problem by the teenagers and even adults. It is related to the hormonal changes but also aggravated by food habits. During their active phase, acne causes a lot of pain and burning. There is a constant tendency to pick at them to get rid of the pain and the itching

They dry over a period of time and leave scars which are not as painful but are aesthetically a major concern. For someone with oily skin who are more prone for developing acne, these scar marks could be very traumatic emotionally. 

There is good news for these people as cosmetic advancements give multiple options for getting rid of these scars. Read on to know some of the most effective ones. 

  1. Laser: A beam of light is aimed at the scar tissue, which does not affect the surrounding tissue but gets rid of the scar tissue. In addition to acne, laser is used in multiple skin ailments (whiteheads, cysts, blackheads, etc.) There is an intense bacterial presence in the acne, and when the laser beam is aimed at the acne, the bacteria are killed. This also reduces further incidence of acne and amount of sebum produced. The number of sittings required would depend on the severity of acne, and most people would require 2 to 3 sittings for visible reduction of the scars. 
  2. Photofacials: The outer layer of the skin (epidermis) is cooled and the inner layer (dermis) is heated. This stimulates the production of collagen and fibrous tissue, which replaces the acne tissue, giving a fresher, youthful look.

  1. Avoid use of irritants like perfumes and deodorants in the affected area 
  2. Avoid waxing or shaving for at least 10 days before the procedure 
  3. Avoid exposure to harsh sun for about a week prior to the procedure 
  4. Avoid medicines which increase the risk of bleeding, aspirin for instance 

Post treatment: In both treatments, the skin may appear red, swollen, and inflamed for the first day. There also may be a prickly sensation, similar to a sunburn. Cool packs or moist clean clothes and a good moisturiser can be used to soothe the skin. Avoid makeup until the skin is back to normal appearance. Depending on the type of skin and acne, the appearance will take about weeks to months after the procedure.

  2. Swelling: A lot of swelling occurs right after a facelift surgery is completed. However, this is nothing to be worried about as swelling is a part of your body’s natural reaction to any kind of injury. It subsides on its own as the body starts healing itself. The major swelling goes away within a few days, but some swelling may be experienced for several weeks after the surgery. 
  3. Bruising: Similar to swelling, bruising is also a natural aspect of the body after a facelift surgery. It occurs as the body reacts to the changes. Bruising is most common during the first few days after a facelift surgery, and it is most likely to occur around the eyes. It takes about 10-15days for the bruising to go away completely.   1. Injury to the nerves which control your facial muscles, although this is temporary. 
  2. Problems related to proper skin healing. This is more common in patients who smoke. 
  3. Crusts on the incisions made during the facelift surgery are also indicated. This too is temporary. 
  4. A numb and tingling sensation around the incisions may be experienced.

It is too early to assess the result of hair transplant. You should wait for at least 6 months before you can say that the procedure has worked well for you or not. You can visit or ask your surgeon to prescribe you medications for this stage which will help in strengthening the transplanted hair.
