You have mentioned about serum creatinine (2.2 mg) but no mention is made of serum urea, microalbuminuria, fasting & pp glucose levels, hba1c% and lipid levels as well as bp readings. Looking at the age of the patient and serum creatinine his egfr is only 31 ml/minute. Which is very low signifying chronic end-stage kidney disease. This is most often a result of uncontrolled blood glucose levels over a long time. Another important aspect is patient is already in grade 1 obesity with a BMI of 33.33 kgs/sq meter, almost going towards grade 2. This also complicates the issue. With such a low egfr his metformin needs to be stopped metformin should be stopped when egfr goes below 45 ml) he needs to be on insulin and there should be better control of blood glucose.
