My mother age 64 is diabetic since 10 years. Presently she is taking Biphasic isophane insulin injection IP (r-DNA origin) 30/70 human mixtard (Wockhardt Wosulin-30/70). Doses taken are 28 IU before breakfast and 24 IU before dinner. Her fasting sugar on 17/11/18 was 240 mg/dL. She has to undergo cataract surgery on her right eye for which ophthalmologist has suggested for reducing the fbs to 130 mg/dL to avoid any infection post cataract eye surgery. Which insulin and doses is recommended to achieve a fbs of 130 mg/dL besides diet control within a week time?

Your mother's diabetes is very badly controlled and the insulins taken are correct and increase the dose by 4 unit both times and see. Add Vildagliptin tablet also and if she is not taking Glimepiride it also need to be added (Glimi 1 mg and metformin 1000 mg twice a day)
