Depression And Anxiety: Treatment, Procedure, Cost and Side Effects

What is the treatment?

There are multiple reasons which lead to a person being utterly depressed and facing social anxiety. These factors include loss of a loved one, getting fired from workplace, difficult situations like going through a divorce or even a separation, and many more. These conditions leave a person mentally and emotionally exhausted and miserable. These feelings are usually very normal reactions to the stressful life conditions. However, there are certain patients who are diagnosed with depression as a psychiatric disorder. These individuals as compared to other patients are more prone to mood swings and the conditions become more severe and persistent in their case. Women undergo depression more as compared to men. Anxiety disorder on the other hand is somewhat different than depression. It involves irritability, nervousness, concentration problems and upheavals in sleeping.

There are several drugs available for the treatment of depression and anxiety disorder. It includes SSRIs (selective serotonin reuptake inhibitors), such as citalopram (Celexa), escitalopram oxalate (Lexapro), luvoxamine (Luvox), setraline (Zoloft), fluoxetine (Prozac), and paroxetine HRI (Paxil). There are newer medicines too which act as serotonin receptors. Bupropion (Wellbutrin) is a drug which has unique mechanisms. A health care provider will determine which drug is best for the patient. There are certain cases in which a combination of drugs is required. Sometimes the combination of one antidepressant with another antidepressant of some other class can boost the effect of a single drug working alone. An example of this is a mood stabilizer like lithium. Anxiety disorder treatment drugs are lorazepan (Ativan), alprazolam (Xanax) and many more. ECT or Electroconvulsive therapy is also a treatment option in severe cases.

How is the treatment done?

The treatment starts by seeing a specialist, physician or a doctor of mental health. They will ask you about your mental health or how he is feeling. They will figure out the cause of depression or anxiety and will come to a conclusion whether the problem is caused by a physical condition or is related to mental health. The doctors will then recommend you antidepressants that will help relieve the indications of depression by the adjustment of certain chemicals in your brain which directly affects the circuits that control mood swings. Antidepressants increase serotonin levels in the brain and start showing noticeable effects after 2 to 3 weeks.

In case of severe depression, there may be a recommendation of an electroconvulsive therapy or ECT. This therapy is associated with a small and painless electric current passing through your brain while you’re under general anesthesia. This leads to a brief seizure which relieves the indications of depression. ECT is also called electroshock therapy. It was earlier a controversial therapy but is now considered quite effective as a treatment method for depression. Its performance is based on a series of sessions of either inpatient or outpatient for up to 4 weeks. You’ll be made asleep through general anesthesia so that you don’t feel the seizure or the current.

Who is eligible for the treatment? (When is the treatment done?)

Patients undergoing depression and anxiety are eligible for the drug treatment. Every patient who is going through severe depression which is uncontrollable and has made the person suicidal is eligible for the electroconvulsive therapy or ECT.

Who is not eligible for the treatment?

A pregnant woman and children need to consult their health care provider before the usage of antidepressants so that they may be able to make a detailed and informed decision regarding their condition.

Are there any side effects?

The side effects of the treatment include slight hesitancy in urination, dry mouth, nausea, diarrhea, constipation, blurred vision, drowsiness due to a sedating effect, and sweating.

What are the post-treatment guidelines?

In case the symptoms of your depression start returning, consult or share your problems with a medical professional, a therapist, or someone close to you and try to work things out for your betterment. Joining a support or a mental health group will combat depression and help you to a great deal. Make as many lifestyle changes as possible in order to maintain your mental health. These changes may include your workplace or even your relationships which are making you unstable in life. Avoid the consumption of too much alcohol and stop smoking. Eating a balanced diet induced with fruits and green leafy vegetables is a significant part of a healthy lifestyle.

How long does it take to recover?

If a patient is finding the treatment helpful after 3-4 weeks, he/she may continue the treatment for 6 months after all the indications have been erased. In case the medications are stopped too soon, the indications may develop again. Patients with recurrent depression are advised to have the treatment for a longer period.

What is the price of the treatment in India?

The price of the treatment ranges from Rs. 100 to Rs. 2,000.

Are the results of the treatment permanent?

The results of the treatment are not permanent as depression is known to be recurrent and may develop again if there are no lifestyle changes made or if the medications are discontinued before the completion period.

What are the alternatives to the treatment?

The alternative treatment methods available for depression and anxiety disorder include vagal nerve stimulation (VNS), deep brain stimulation (DBS) and repetitive transcranial magnetic stimulation (RTMS).

Post-traumatic stress disorder (PTSD) is an anxiety disorder which develops due to a terrifying traumatic incident. Traumatic events like violent personal assaults, man-made disasters, accidents, military combat etc. can trigger PTSD.

What are the symptoms of PTSD?
People with PTSD mostly re-experience the whole incident repeatedly in the form of flashback episodes and nightmares. This happens more when they are exposed to the events or objects which are reminiscent of the trauma. The other symptoms are emotional numbness, sleep disturbances, depression, anxiety and irritability leading to outbursts of anger on others. People with PTSD feel intense guilt mostly. Who will most likely develop PTSD?

When does PTSD occur?
PTSD can occur at any age, given the intensity and frequency of symptoms. The symptoms typically are seen within 3 months post a traumatic event, although it may not begin until next year. Once the PTSD symptoms appear, the severity and duration of the illness may vary.

What are the treatments available for PTSD?
Cognitive-behavioral therapy, group therapy, and exposure therapy have been very effective in treating PTSD where the patient gradually and repeatedly relives the frightening experience under controlled surroundings which helps him/her work through the trauma. Medicines also help ease associated symptoms such as depression and anxiety.

Do other illnesses accompany PTSD?
PTSD can present with many associated conditions like depression, alcohol or substance abuse, anxiety disorder etc. Certain symptoms like headaches, gastrointestinal ailments, immune system issues, dizziness, chest pain or discomfort in other parts of the body are common. To treat PTSD entirely, it is necessary to take these symptoms into consideration as well.

Kidney stones are one of the most common urinary tract disorders. These stones are the result of an abnormal concentration of urine. The stones might get unnoticed, till it moves around the kidney or passes into the ureter. The stones are basically small deposits of calcium that can cause excruciating pain.

Here is a list of signs that can indicate that a person might be suffering from kidney stone disorder:

  1. Painful Urination: The most common sign of kidney stones is a sudden urge to urinate. The urination occurs even though an individual has maintained the normal rate of fluid intake. When an individual does try to urinate, the amount of urine expelled is less than normal. In case of a normal volume of urine, an individual might experience extreme pain. The pain is the result of kidney stones travelling from the bladder to the urethra.
  2. Back Pain: People suffering from kidney stones often suffer from localized back pain in the early stages. The pain initiates at the side and back, just near the kidney area. As the condition worsens, the pain might spread to the groin, lower abdomen and lower back area. The waves of pain might last from a few minutes to few hours, depending on the number and size of the stones. For men, the pain might also be felt in the testicles and scrotum area.
  3. Blood In Urine: Apart from pain and discoloration, visible signs of blood in the urine are seen in patients who have kidney stones. The color of urine might vary from brown, pink to slightly red with stains of blood. With kidney stones increasing in size, it blocks the urethra, thereby ticking blood along with urine. This condition is referred to as hematuria.
  4. Vomiting and Nausea: People suffering from kidney stones often complain of nausea. It can become so severe that can lead to vomiting. Some other complications related to this condition include dehydration, nutritional deficiency etc. Another theory suggests that, throwing up is the result of body’s endeavor of clearing the toxic material from the body.
  5. Pungent Smell: It is quite common among patients suffering from kidney stones to have a pungent smelling urine. The smell is foul and sticks for a long time. The smell is the result of creatinine waste being accumulated and slowly passing through the urine from time to time.
  6. Difficulty in Sitting: Once the kidney stones gain in size, it becomes increasingly difficult for a patient to sit or lie down for a longer duration. Sitting or lying down puts pressure in the kidney area thereby causing intense pain. People try to combat this condition, by frequently standing up or by having a small walk.

  4. The ability to deal with stress increases, if you maintain better health. Get adequate rest, exercise and eat well and set a regular routine. Abstain from drugs and alcohol as they only provide relief for a temporary amount of time, but increase isolation feelings, anxiety and depression, thus worsening trauma symptoms.
  5. Learn to truly let go. Accepting implies embracing the trauma and deciding what you will do about it. It can dominate throughout your life unless you let it all out.

No one can be moral policed here for the choices they make. While there are still many people who believe in love, the younger generation is more inclined towards casual sex.

Anxiety and depression oftentimes go together. If you want to come off of the medication, you will need to work really hard. You must go and meet with a counselor immediately and you can meet with a doctor too but you must cooperate to your utmost with both therapies. In the meantime please do the following sincerely because you could resolve the problem better with good cooperation: Have a good night’s sleep, have a good breakfast of more proteins, meditate often, remain free of stress, eat a lot of fiber, nuts, avocado, exercise regularly, eat dark chocolate, do Yoga meditation exercises, spend time in nature or gardening, etc. I suggest you do the opposite of what this depression makes you feel like doing (actually, not doing): you will need to fight this condition. You must become active; stay upright during the daylight time; meet people; never sleep during the day, wake up by 6 am every day, play some active games, especially contact games, do physical exercises, talk to people and join some social clubs, attend Yoga classes etc. Watch sitcoms on TV or comedies and cheer yourself up. Go for excursions in groups, for outings, camps, conferences, and religious conventions. Get a pet dog and spend time training it, exercising it and relating to it. Expose yourself to some sunlight every day, at least, 30 minutes but not in the scorching heat. Whatever happens, please incorporate these three important adaptations in your life: always be responsible, be respectful, and be functional. If you did these three, lots of things will go well in life. Please pray and have faith in God to alleviate your sufferings. Don’t wait for others to help. Use your own motivation, which might be at its lowest, but persevere and win this battle. Above all to be really happy, you need to live in love and for love. Learn all about emotions and how to handle them and that will get you out of the depression rather easily and quickly. A counselor is there only to facilitate you, all the hard word must come from you, and your cooperation with that person is very critical for your success. Be positive every day and learn to be contented with what you have. Do some left brain exercises: it is the happy brain. Here are a few suggestions: shut your left nostril and breathe, move your eyes from right to left and vice versa for at least half a minute at a time, and do calisthenic exercises with some form of counting, regularly. Whatever happens please cooperate with the therapy and do not discontinue until the condition is completely resolved.

I live in hostel away from home. When I came home this Diwali holidays I see my father all changed like in a way that he has a mental problem. Makes my mother's life miserable. We don't have much money but he thinks that he is very wealthy. He sold my mother's gold and invested in business my mother didn't protest it. He doesn't has regular sleep cycles no regular eating patterns and gets angry all the time I know he loves us deep down but when his mental condition strikes he is like a different man he has this problem from last 20 years this condition strikes once or twice a year and he is like this for two or three months and spends money like a crazy person all this family chaos has gave me a depression and I am not able to focus on my studies.

Dear lybrate-user, Most probably your father is suffering from a Bipolar Affective Disorder - presently in mania. It is recurrent biochemical disorder of mind, runs in families, self limiting by itself. During mania he may be talking too much, spending too much, irritable and over confident of all his business, etc. It lasts fro few months and he becomes normal for few more months, Because it spoils his financial status, name and prestige, we need to treat him with tranquilizers. The other pole of this bipolar disorder is depression where he will bad and sad for his losses, lacks confidence, irritable, no energy, no interest not only in business but also in tv, news or other pleasurable things. Since suicidal risk is high in this phase, we need to treat with antidepressants. Since it is a recurrent disorder, we need to prevent future episodes, by prescribing mood stabilizers for a long term. He may not be willing for treatment in this manic phase, so you consult a psychiatrist along with your mother and discuss how to being him to treatment. During a normal or depressive phase, he himself will go to psychiatrist. Let him take your mother so that the mood chart can be reported to the doctor. Since this bipolar disorder runs in families, you may be vulnerable for depression. Don't blame your father's behaviour for your depression. He will be blaming his business partners or your mother. Blame either heredity or the biochemistry, and get into treatment early before it damages your studies or career. All the best.

Hello Everyone!

My name is Dr. Aparna Deshmukh. I am a consultant psychiatrist sexologist. Today I will be talking on postpartum depression or postnatal depression. Postpartum depression is a mood disorder which is characterised by depressive symptoms in the period that follows childbirth in either of the parents. More commonly in mothers. It is important to know about postpartum depression because no.1 is common. About 15% of women suffer from postpartum depression in the postnatal period. Secondly, not only it is important from the point of view mother s mental health but mother who suffers postpartum depression is unable to care for her infant effectively.

And it leads to profound effect, mental health and well-being of the child in the future too. And thirdly, in postpartum depression in its severe form which is postpartum psychosis, the mothers may have thoughts of harming their infants. And they may actually act upon this thought leading to actual harm to an infant and in some cases even death. So how do patients with postpartum depression presents. The postpartum depression onset is usually 2 weeks to 1 month after delivery or sometimes even miscarriage. Some professional even diagnose it after 1 year of child birth. Here the patient typically presents with behaviour and cognitive symptoms. The emotional symptoms include the patient saying that they are sad, sometimes they feel empty inside.

Some patients are irritable, and they get angry on trivial issues. On the other hands. Some patients become lethargic and complaint of low energy or loss of pleasure, loss of interest, loss of enthusiasm. And some patients become withdrawn, non-communicative. In different towards their infant. And in general, they have this fear and worry that they would not be able to care for their infant which leads to a feeling of inadequacy. And sometimes inappropriate guilt and sheen. Many patient s complaints about a loss of concentration, poor memory that they are not able to take their decisions. The decision-making capacity has been reduced. Or they are unable to think clearly. Some have thoughts of hopelessness or helplessness. And even worklessness. And some patients in their severe illnesses may also have thoughts of humming themselves or even others.

The relatives might notice that the patient has either become withdrawn or non-communicative or they may feel that the patient is overwhelmed and anxious. They may notice that the patient is very lethargic. Or some patients, they sleep a lot. Mostly the whole day. And some patients do not sleep at all. So, sleep patterns may vary depending on the patient. Even the appetite changes are noticed in some patient. The severe form of postpartum depression that I was saying formally postpartum psychosis. Now in postpartum psychosis other than these symptoms, there is also presence of psychotic symptoms. These includes suspiciousness towards family member that they will harm them which leads to an agitative and aggressive behaviour of the patients. Sometimes the patient hallucinates i.e. the hear of voices that are inaudible to others. And the relatives may notice that the patient muttering to self, talking to self. Sometimes patient may also have excessive physical activities. Or they may talk excessively or there is the running of thoughts in their mind. So, these symptoms may also be the part of postpartum psychosis.

Another entity, that needs to be differentiated postpartum depression is postpartum blues or baby blues. Now this entity is more common. But fortunately, it is a milder form. And the depressive symptoms are milder. Patient may complaint of depressed mood, fluctuating moods, irritability, change in sleep and appetite pattern. But the good thing is that it is self-limiting. However, in some cases postpartum blues may progress to postpartum depression. So, how common is postpartum depression? Like I said 15% of women in their postnatal period suffer from postnatal depression. However, postpartum psychosis is rarer. Postpartum blues on the other hand much more common. Affecting 50-70% of the women in the postnatal period. What causes postnatal depression?

There are multiple factors: biological, psychological, social factors. Usually it is an interplay between these factors. Sometimes the combination of these factors that leads to postpartum depression. So, the hormonal changes that occur during and after pregnancy, the sleep deprivation, the emotional and physical stress of childbirth, the added responsibility of the child and poor social and family support can lead to postpartum depression. So, how do we treat postpartum depression? In mild cases, like psychotherapy like counseling helps but modern to severe cases the anti-depressants are the main stay of treatment. In postpartum psychosis. Antipsychotic medication is also prescribed. In severe cases, electroconvulsive therapy is also recommended.

Psychotherapy can be combining with the biological therapy. Mainly cognitive behavioral therapy and interpersonal therapy helps. Counseling and peer support group are also recommended. So, these were the some of the basic questions that I tried to answer about postpartum depression.

Stress is one of the most important factor for causing psychological illnesses. People believe that stress is not a psychiatric problem and it is a part of life. That's why most person donot come for treatment. Some misbelieves about stress are there. Stress effects everything, it effects your mind, your body from head to toe, it effects everywhere. When it effects your mind it causes migraine, headache, vomiting, nausea, paralytic attack, heart diseases, blood pressure, diabetes, acidity, stomach problems, restlessness, muscle cramps, sexual dysfunctions and a lot of illnesses like constipation, diarrhoea, anxiety problems. It effects every part of your body, and not only the body but it also effects your mind, it effects your emotions.

When the emotions are effected the person becomes irritable, he remains in a sad mood, and gradually other faculties of mind get effected and the person goes in depression or other problems like anxiety, sleep problems, psychosis, mood disorders and addictions. People believe that stress is not a problem, and when it becomes a big problem they come to us for treatment with a disease like depression, anxiety, mood disorder, psychosis or with suicide attempts. At that time the treatment becomes dfficult and the person has to take treatment for a longer period.

There are a lot of factors causing stress. Most of the people, they blame others for their own problems like inflation and job problems. Most of the time the person himself is responsible like his character depicts irresponsible behaviour, suspiciousness , not taking responsibility of family, child or at work place, unmanagebility, procastination, there are a lot of character defects that lead to problem either at home or work place or in society that leads to stress. So when it comes to stress management most of the people either go for changing their lifestyle, starting exercises, pranayam, moving around or reading some books, listening to music or going outside.

It relieves your stress but only for a couple of days or couple of weeks and again after those couple or days or weeks it becomes as it was before because the reason for your stress was character defects. Until and unless you change yourself the problem is not going to reduce. So when the management of stress comes, initially the most important character defect that leads to stress has to be managed.
