Respected panel of doctors, I am 45 years old it professional from bangalore my weight 69 kg and height 162. Fasting blood sugar 139, post prandial 264, hba1c 8.9. I am talking metformin 509 of + glimepiride 2 mg, once a day. Should I take insulin. Your valuable advice would be appreciated.

Madam, to advise whether you should take insulin or not, I need to know the following your dietary pattern, extent of daily exercise, bp readings, lipid profile and serum creatinine and urea. My suggestion is please come back with all the information in a private conversation, where apart from giving specific advise I will be able to prescribe necessary medication s including insulin if required. In this communication we supposed to give only general advise. I am sure you are aware that in a well controlled diabetes patient fbg should be < 100 mg or closer to100 mg, pp 150 to 160 mg & hba1c% < 6.5% at your age. Thanks.

You might need an increase in metformin. But check your creatinine first. Also vildagliptin or empagliflozin may help. Your weight goes against insulin initiation. If you can afford semaglutide once weekly should be helpful for u, especially helping to decrease the cardiac risk.
