How do I know, if it's a fistula or an abscess? If it never causes any discharge neither it is painful and present from months? Constipation is present.

Dear user, fistula will be a pin point with discharge and a tract going inwards which can only be seen on radiography after injecting a dye. Whereas an abscess will be a swelling with discharge and can be foul smelling in both cases. It could be a pilonidal sinus too. You will need a proper evaluation from a surgeon to identify and reach a proper diagnosis. Good luck.
