My father is 66 years old person. He is a high blood sugar and high blood pressure patient since 5 years and his sugar and pressure level is normal now with daily medication. He is taking a medicine daily for acidity in empty stomach for long years. He is taking medicine for urea and creatinine also for last 5 months as his urea and creatinine was slight high. But as per report of 2 days ago his urea is 52.60 and creatinine is 1.38 now. His medication details are given below: janumet (50 mg/1000 mg) azulix (1 mf forte) aztor 10 neurobion forte dytor 5 diamicron 5 trajenta 5 telma am.

Their is some duplication in diabetic medication in this case. Janumet has sitagliptin and trajenta has linagliptin (both belonging to same class). Azulix has glimipride and diamicron has gliclazide (both belonging to same class). You need to stop duplications and also get a hba1c done (it shows 3 months average of blood sugar level). Review with reports.
