My dad is 78 type 2 diabetic since 2002 he was on oral med for many years and from last 6-7 years on insulin currently he takes huminsulin 30/70 20 units in the morning and 10 units in the evening and he also takes dynaglipt m in the morning but his sugar level is not controlled and today after lunch 1.5 hours later it was 240 please suggest if we need to give insulin before lunch as well? Or is there any other good insulin he is also a cld patient since 7 years but stable.

Kindly stop insulin 30/70 and dynaglipt start huminsulin-m 50 22 unit s/c 30 min before breakfast 12 unit s/c 30 min before dinner kindly do blood sugar test in the morning and 30 min before dinner on daily basis and wrote down those values on paper along with date than revert me back.

Yes he needs uptitration of insulin regimen. Options include adding one insulin before dinner Changing type of insulin. All these needs close monitoring of sugars and dose adjustment.
