Hello doctor I am male 27 years. Having problem of pile for last 7 months. I have got fissure in anal that causes too much pain that I cannot handle. I have consulted th doctor and he gave me first gudlax plus syrup, it worked only for 1 time then he gave me lactihep syrup, tab ciplox 500, tab metrogyl 400, pilex ointment, smuth ointment. And tab. Pantop d. These all worked for a time only not again I got piles with no blood but lots of pain. No loosing syrup working now with my constipation. Please help me with some medicines that I can take to get relief. Please help.

Hi, thanks for the query. As per your history, it looks like fissure in ano. The treatment whatever you have taken till now is correct and is given as conservative treatment for fissure and it works only temporarily. You may have to alter to your diet and include more fruits, fibre rich vegetables, green leafy vegetables and lots of water. If the pain is unbearable you may take, tab zerodol sp bd x 3 days (if you don't have any drug allergy. You have to take a) sitz bath atleast twice daily, and b) continue using smuth ointment /2% lignocaine jelly & c) laxatives (syrup cremaffin plus 15 ml at night x 5 days. If it still doesn't not subside, please show to general surgeon in your locality. Â

It most probably this is chronic anal fissure. You can used high fibre diet with high intake of water or liquid. If your symptoms is more severe then you used duphalac syrup and apply oientement cremagel h with sitz bath three times daily for 10-15 minutes. If you have still problem you can contact with surgeon for examination. Â

See think you have a increased sphincter tone which would have caused the fissure and thats why you have pain wen stools pass through. Better to take laxatives and stool formations drugs. Try softovac powder or cremaffin syrup. It would help you in passing loose stools. And its better to have a lateral internal sphincterotomy done. Hydration is the most important one that has to be taken care and high fibre diet. Â

lybrate-user. Tariq, you should apply ointment at least for 4 weeks and then follow up with doctor, if fissure not getting better get a laser done. You need a colonoscopy as well. Â
