I am Dr. Hemendra Singh, a senior surgeon.

I welcome you to Singh Surgical Clinic based in Indirapuram practicing general and laparoscopy for the last 30 years. The introduction of my topic today in on Hernia. What is Hernia? Basically when tissues procure through a week part of the Abdomen wall or some other parts, it leads to procuration of internal organs. It may be in the abdomen or it may be in the intestines. The most common type of Hernia are, especially in males, in the groins which are called Inguinal Hernia. They may be of one side or both sides. In females, the common Hernia are Ventral Hernia which are usually an old scar or near an old scar. In today’s world when there are lot many busy schedule of people and lack of sleep also will be develop Hiatal Hernia which sometimes is congenital also. The hernia is caused when the muscle is weak in the abdominal wall or the person usually states either for due to constipation or for urination. Pregnancy, recent increase in weight, cough, etc. or weight lifting in gym is also plays an important role in development of Hernia. So the people, even in younger age, because even in small age children the risk of developing Hernia is because of the congenital diseases. In the adults these play accumulated factors play an important role. The symptoms of Hernia are very simple. A place where you never used to find and a bulge or lump, you will find in the groin or in the abdomen wall near by abdomen scars. The treatment options for hernia are a simple strategy of weight reduction, balanced diet, routine exercise like walk etc. but mostly the cases require surgery and the surgery is in the form of a laparoscopic surgery very small keyhole incisions are made and we put a mesh through the smaller holes and we place it into the abdomen wall. Very very advanced technique for development the methodology are developing for the treatment for the treatment of hernias for the bigger hernias because now we at a time take major surgeries in the abdomen for cancers, for trauma etc. and which later on only took large Hernia also where we require all these new techniques the risk of not operating especially patients in home there are a lot of symptoms like bulge, pain is because of the obstruction in the Hernia set. It may bet strangulated when the surgery becomes very very necessary and urgent. If you delay the surgery people may develop perforation of the intestines and contamination of the while abdominal cavity, then it becomes a very very life threatening issue we should take care of the hernias especially at an early age. Sometimes we have seen people asking whether this will come again or not. So if you for an early surgery and take a good surgical course the chances of developing a recurrence are very low. Risks Of Bowel Resection Surgery You Should Be Aware Of!

The bowel is a very important part of your body within the digestive system as it plays a vital role in the absorption of nutrients and minerals within your body as well as excreting toxic matter. The bowel contains the small bowel or small intestine as well as the large bowel or the large intestine. However, parts of the bowel may be infected or damaged in such a manner that they may need to be removed in order to save further worsening. Thus, the removal of sections of the bowels is known as bowel resection surgery.

How is bowel resection surgery performed?

In this procedure, sections of either the large intestine or the small intestine are removed which have either become diseased, infected or have malignant growths within them. The doctors and surgeons first identify the sections that need to be removed and then perform the surgery wherein a section of the tissue is excised and then two ends of the bowel are stitched together to form a new section of continuous small intestine or large intestine.

Types of bowel resection
There are two types of bowel resection, namely small bowel resection performed on the small intestine and large bowel resection performed on the large intestine. The indications for both of them are mentioned below:
Small Bowel resection: Some of the cases wherein small bowel resection may be required are as follows:

  1. Cancerous or benign polyps or growth
  2. Precancerous growths and polyps
  3. Damage to the small intestine due to injuries
  4. Congenital defects in the small intestine
  5. Blockages in the intestine
  6. Growth of ulcers, bleeding, and infection within the small intestine
  7. Infections within the intestines
  8. Other disorders of the small intestine

Large bowel resection: Large bowel resection is also carried out due to many of the same reasons as mentioned above. However, some of the specific reasons for large bowel resection are mentioned below:

  1. Colon cancer
  2. Diverticulitis, disorder that specifically affects the large intestine
  3. Bowel inflammation or ulcerative colitis
  4. Abnormal twisting of the bowel also known as volvulus
  5. Intestines which slide into another section of the intestine; also known as intussusception

Risks of the bowel resection
Like any other surgery, bowel resection also carries its own risks just as any other form of surgery. Some of the common risks of bowel resection are –

  1. Adverse reaction to anesthesia
  2. Infections
  3. Heart attacks or strokes during the operation
  4. Infections
  5. Excessive bleeding among others

Specific risks for large and small bowel resections are mentioned as follows:

  1. Small bowel resection: Certain common risks include accumulation of pus in the abdomen, internal bleeding within the intestine after surgery, diarrhea, infection of the incision area and the stitched area breaking open among others
  2. Large bowel resection: In this case, tissues protruding through the cut causing an incisional hernia is the most common element of risk. Nearby organs may be damaged as well; scar tissue, problems with the passage of material within the colon among others are some of the other complications.

8 Signs Indicative of Body Lumps

Lumps are usually harmless and do not raise serious concerns. However, if the lump persists for more than two weeks, it may be a serious issue. Lumps are categorized based on their place of occurrence and each of them demand separate treatment. Learn the signs of danger related to lumps and take action accordingly.

Reason behind formation of Lumps
Injury is one of the major causes behind lumps. If the lump is painful, one must consult a doctor immediately. There can be various other causes behind lumps, which depend upon its place of occurrence.

  1. Epidermoid and pilar cysts (sebaceous cysts) appear as small smooth lumps under the surface of one’s skin. Such cysts are non-cancerous, hence, can be treated easily.
  2. Swollen lymph glands generally occur in neck or in groin or under the armpit. Usually, any infection results in swollen lymph glands. However, in certain cases it may be caused by cancer.
  3. Skin abscess is a term given to collection of pus. e.g. boils. Symptoms of abscess consist of redness and swelling.
  4. Style or Chalazion causes swelling on the eyelids.
  5. Lump in the salivary glands happens due to mumps
  6. Swelling in the thyroid glands may cause lump in the neck region. This lump may encompass the entire third gland or a part of it.
  7. Breast lumps occur in both men and women. Generally, they do not indicate breast cancer. Any abnormal swelling in the breast must be immediately shown to the doctor.
  8. Hernia or enlarged lymph gland may result in a lump in the groin.
  9. If a person experiences swelling in the scrotum, he must immediately consult a doctor.
  10. Lump in the anus can be caused due to piles. The small vessels engulfing the anus may get swelled piling up more blood than usual. Other than a pile, abscess could also be the reason behind lump.
  11. Lump on the hand, wrist, finger may occur due to ganglion. Such cysts may develop around joints or tendons.

Signs which indicate you must get your lump checked by a doctor

  1. If the lump gets hard or firm
  2. If the lump gets sore
  3. If the lump gets enlarged
  4. If the lump doesn’t vanish within few days
  5. If your body temperature rises more than normal
  6. If you experience abnormal loss in weight
  7. If you experience any such abnormalities
  8. If the lump reappears post removal

The bile in your body contains enough chemicals to dissolve cholesterol excreted by your liver. In case your liver starts excreting more cholesterol than your bile has the capability to dissolve, the excess cholesterol crystallizes and forms stones. Bile may become concentrated if the gallbladder doesn't empty in the time leading to the formation of gallstones. A number of factors can increase the risks of formation of gallstones like:

  • High cholesterol
  • Being obese/overweight
  • Being an American Indian or Mexican American
  • Being female
  • Falling in the age group of 60 or above
  • A family history of gallstones
  • Rapid weight loss
  • Diabetes
  • Consumption of cholesterol-lowering medications
  • When should you consider undergoing surgery?

In some cases gallstones that are detected during ct scan or ultrasound (without any symptoms) don't require surgery. If you do not feel aggravating pain in your upper right abdomen or other signs and symptoms, then one can postpone the surgical process according to the need. In few instances, it has been seen that small and solitary gallstones can be dissolved with oral medication like ursodiol and chenodiol. A widely used surgical procedure, laparoscopy involves the insertion of certain surgical instruments, small tubes and video cameras in your abdomen through small incisions and cuts. This procedure is used to diagnose a number of ailments such as ovarian cysts, endometriosis, and pelvic inflammatory diseases to name a few. Even though laparoscopy is a very popular form of surgery, there are quite a few myths associated with it, which are:

1. Myth: The images taken through a laparoscope are of poor quality

This is not true. In fact, the visuals obtained through a laparoscope are clearer and much more accurate when compared to those obtained via an open surgery. The visuals of a video laparoscopy provide a detailed magnification of even those parts of the area that are inaccessible by the human eye.

2. Myth: If you've undergone multiple abdominal surgeries in the past, you can't opt for a laparoscopy

The truth is that you can go for a laparoscopy even if you've gone through multiple surgeries previously, irrespective of the location or size of the previous incisions. This is done through the use of a special instrument, called a microlaparscope that enables safe entry into the abdomen of the patient.

3. Myth: If you have large fibroids and ovarian cysts, you can't have them removed through a laparoscopy

False. Even though the incisions made by a laparoscopy are really small, they still allow the safe removal of large abdominal structures by the use of certain devices. For example, a cylindrical tool known as morcellator can be inserted through a laparoscopic incision and be used to remove large fibroids and ovarian cysts.

4. Myth: If you're overweight or underweight, you can't undergo a laparoscopy

No matter if you're obese or too thin, you can still undergo a laparoscopy as the tools used for this surgical procedure are available in different lengths and sizes, and can be adjusted as per the body type of the patient before the incision is made.

You seem to be having UTI. Get urine Culture and sensitivity done and antibiotics accordingly. Use barrier contraceptives regularly.
