What is the status of disease post chemo-radiation, have you got a direct laryngoscopy done to know the cause of breathing difficulty and what is thestatus of lungs. If both are disease free then tracheostomy can be closed once radiation edema settles down with steroids, nebulization and steam inhalation. If residual disease then he would require further treatment. Pain should be managed with good analgesics.
