Hi Doctor, My father is having a kidney tumor of size approx 6 cm in one kidney and also prostate is enlarged, using catheter now for urination. Tumor is in there from last 3 years. Size was decreased from 5 cm to 3 cm before last year as was doing homoeopathic medication. But after that since last 1 year due to discontinuation, tumor size is increased to 6 cm. Now urologist has told to do the surgery. In want to take a second opinion keeping in mind about kidney tumor and prostate. Not metastasized to lymph node. Father is having acne problem and aged above 65 years.

Kidney cancer is known to be a slow growing cancer with protracted course and also spontaneous regression is known. So whether the homeopathy controlled the tumor or was it the inherent tumor biology, is difficult to tell. I would go with the later. Biopsy is not done to confirm the kidney mass as all are presumed to be cancer. I do imaging with a CECT scan and metastatic work up with a CT thorax + bone scan or a PET CT scan. If non metastatic we proceed with surgery. (even oligomets can be respected) If tumor size is less than 4 cms, I prefer to do a partial nephrectomy. If the size is more than that then a radical nephrectomy with paraaortic nodal clearance. These usually donot require adjuvant therapy. If metastatic and Patient, tumor is resectable and Patient is not poor prognostic as per IMDC criteria, then I prefer to do a cytoreductive nephrectomy to reduce the tumor burden and then start on targeted therapy like Sunitinib (poor prognostic may start on immunotherapy with Nivolumab, but it is a very costly affair) I hope I was able to solve your query.
