Hi, I am an AS patient and had a flare up in mid 2013 after which the disease seemed silent till end of 2017. I have now had neck spasms for the past 4 months and my rheumatologist has recommended that I take Folitrax 15 mg (1/ week) folvite (any other 4 days in a week) Flexirol (2 times a day for 10 and then once a day for 10 and then as SOS) and Uprise D3. I am continuing this medication for the past 10 days but my neck spasm reduces only marginally with these medicines and if I miss a day of flexirol, it tends to come back. My question is, whether my neck pain is owing to a mechanical problem or is it because of AS. I have conducted both xray and MRI of my neck and it is unremarkable (no signs of fusion yet) except for signs of spasm. Am I undergoing the wrong treatment for my neck? Is it actually a mechanical problem that needs to be addressed? Do you recommend I visit a separate doctor for my neck pain?

Dear lybrate-user, Thank you for your query as this may be useful for many others in a similar situation. AS management can be challenging with frequent flare up episodes. It is important to have faith in the doctors treating you and if the treatment is not helping then its best to go back to your rheumatologist (as he probably knows you well) to discuss other alternatives. Using a multi speciality approach may help in improving outcomes, what I am saying is it may be worth combining medications with physiotherapy. Discuss this with your doctor if you are not already receiving this. Having dealt with others in a similar position I feel there may be a role of doing muscle and /or neck joint injections to relieve the spasm and break the pain circle.
