Types, symptoms and treatment for Diabetes

Hello viewers, mera naam Dr. Ritez Kumar hai. Main M.D Medicine hu and I have a keen interest in treating Diabetes. Aaj hum thoda diabetes ke baare mei jaanege ke kya precautions hai? Kyun hota hai? Aur kaise hum ekatthe ho kar visualize therapy taiyaar karte hai.

Toh pehle hum shuru karte hai ke diabetes hai kya?

Jab bhi hum khaana khate hai toh wo absorb hota hai aur sugar humare blood mein jaata hai. Is sugar ko use karne ke liye insulin naam ki ek hormone ki zarurat hoti hai aur is insulin ka kaam hota hai ki ye aapke cells mein is sugar ko use karwaye. Toh fir, generally diabetes do tarah se hota hai; ya toh aap insulin bana nahi rahe ya aapke body mein insulin resistance hai arthaat insulin aapki body mein kaam nahi kar paa raha hai. Bas inhi do kaarano se diabetes hota hai.

Diabetes ka pata kaise chalta hai?

Generally koi symptoms hote hai jaise ki thakavat aana, bahut zada pyaas lagna, unintended weight loss hona, baar baar pishaab ko janaa, inhi kuch symptoms ke sath diabetes ka diagnosis hota hai. Agar aapke ghar pe kisi ko diabetes hai, agar aap pregnant hai, 30 saal ke upar hai, lifestyle aapki active nahi hai yaani ki baithe rehne ka hi zada kaam hai toh aap apne diabetes ya sugar ka test zaroor karwaye. Diabetes generally do ya teen type ke hote hai ek Type 1 Diabetes hota hai jisme insulin banta hi nahi, iska treatment kewal aur kewal insulin hi hai. Zada tar logo mein Type 2 Diabetes hota hai jisme insulin banta hai par aapki body usse use nahi kar paati. Teesri jo common tarah ki diabetes hai usse hum Gestational diabetes bolte hai jo ki pregnant females mein hota hai. Diabetes ka test bahut simple hai, isme ek blood test kiya jata hai, fasting blood sugar aapka agar 125 ya usse zada hai toh aapko diabetes hai.

Ek test hota hai HbA1c jo ki teen mahine ka average batata hai aapke sugar ka, agar aapka HbA1c 6.5 se zada hai toh bhi aapko diabetes hai. Zadatar time ab hum random ya khane ke baad wala sugar nahi karte lekin fir bhi agar aap kabhi karate hai aur 200 se zada aata hai toh aapko further test ki zarurat hai ye pata karne ke liye ke aapko sugar hai ya nahi hai. Diabetes se diqatte kya hoti hai? Dekhiye, diabetes jo hai wo ek progressive bimaari hai as such bade saare logo ko lagta hai ki ye jo thakavat hai, ye jo weight loss ho raha hai, ye normal lifestyle ka part hai but aisa hai nahi, diabetes aapke body ke har part ko involve karta hai. Major complications jo ho sakte hai jaise aapko dil ka daura padh sakta hai, lakwa maar sakta hai, pair mein gangrene ho sakta hai, pairo ki nase such sakti hai jiske wajah se aapko chalne mein diqqat aa sakti hai, aankhein kharaab ho sakti hai, gurde kharaab ho sakti hai, pet ki diqqat ho sakti hai, sexual activity mein diqqat aa sakti hai, koi bhi chota infection bahut jaanlewa ho jaata hai, isliye diabetes ka control bahut zaruri hai aur jitna early hum iska treatment start kare utna hi aapke liye faidemand hai.

Ab diabetes ke liye treatment kya hai?

Diabetes ke treatment ko hum generally do bhaago mein divide karte hai; ek hai lifestyle management aur dusra dawaiya. Lifestyle management mein main humesha apne maarizon ko samjhata hu ke ye dawaiyon se zada important hai. Lifestyle management mein teen cheezein aati hai: weight control, diet and exercise. Weight control mein bhi fir teen cheezein aati hai ke aapko motivated hona hoga ke aap weight lose karna chahte hai ya nahi aur iska exercise pattern bhi hum individualize karte hai ke aapko kya karna pasand hai. Agar aapke ghutno mein dard hai toh shayad aap walking na kar paaye, aapke liye cycling better hai. Agar aap yoga karne chahte hai toh uske liye bhi hum individualize karege but depend karta hai aapko pasand kya hai. Issi tarah se se diet ko bhi individualize kiya jata hai. Ye sabhi ko pata hai ke diabetes mein sugar nahi khana hota, aaloo ki cheezein nahi khaani hoti hai, but regional differences hote hai, cultural differences hote hai aur us hisaab se hum aapko batate hai ke kya khana hai aur kya nahi khana. By and large, complex carbohydrates, fresh fruits, jo ki bahut zaada meethe na ho, healthy portion of vegetable and salads.

Ek rule of thumb hai jo ki har diabetic patient ko follow karna chahiye. Exercise ke baare mein kam se kam aadha ghanta, bahut acha ho agar aap 45 minutes har din walk kar paaye, kam se kam hafte mein 5-7 din. Diabetes mein medicines bahut tareeke ke hote hai, maine pehle bhi bataya ki Type 1 diabetes ka ilaaj sirf aur sirf insulin hai, Type 2 diabetes mein different medications hai aur diabetes bahut hi advancing field hai jisme ke din ba din naye drugs aa rahe hai. Ye aap apne doctor se mile, aapke kis level ka diabetes hai, aapko sath mein kya complications hai, aapke tests kya reveal karte hai, aur aap kis regimen ko follow kar sakte hai, us hisaab se hum aapke diabetes ka plan banate hai. Aqsar hum sugar ki mareezon ko ye pata nahi hota kea cha sugar control bolte kisko hai. Acha sugar control hum tab bolte hai jab aapka bhuke pet wala ya fasting wala sugar 140 se kam ho. Agar random karwa rahe hai ya khan eke do ghante baad wala karwate hai toh wo 180 se kam hona chahiye aur teen mahine ka average batane wala HbA1c, 7 se kam rehna chahiye. Agar aap ye teeno goal achieve karte hai, iska matlab hai ke aapka sugar acha control hai.

Agar aap humare sath judna chahte hai, diabetes ka treatment karwane chahte hai ya kuch aur jaanna chahte hai toh, please contact me through lybrate.com. Thank you so much.

Dear sir My mother is suffering problems in left lungs that problem is according to doctor report 1.Secondary spontaneous pneumothorax (L.L) 2.broncho pleural fistula 3. F/C/O DLPD (likely sarcoidosis) 4. Diabetes mellitus type 2 Please help me what can you do for best treatment.

For pneumothorax you need to put a chest tube. For bronchi pleural fistula she will probably need videoscopy assisted pleuro deisy. For sarcoid treatment will depend on stage. Diabetes treatment has to be individualized also. Let the chest physician decide what best to do. Regards.

Stomach Infection - Easy Tips To Help You Treat It!

MBBS, MD - Internal Medicine
General Physician, Delhi
Stomach Infection - Easy Tips To Help You Treat It!

Food is one of the most common ways of getting an infection, and so the stomach is more prone to infections than other body parts. Whether it is eating raw or uncooked foods or unhygienic foods, the stomach bears the brunt of one’s bad food practices. The only consoling factor is that it is equally easy to treat a stomach infection if some simple measures are adhered to.

Read on to know some of the easiest and most effective ways to treat a stomach infection.

  1. Drink it right: What you drink and how you drink makes a lot of difference to the way your stomach infection will respond. The vomiting, diarrhoea and sweating is robbing the body of a lot of fluids, and so replenishing this fluid along with minerals is very essential. Ensure you drink a lot of clear liquids like water, coconut water, clear soups, sports drinks, and other energy drinks. Electrolyte replacement can be added to improve their efficacy. Herbal teas like ginger tea and peppermint tea are highly recommended to soothe the stomach too. At the same time, avoid caffeine, strong black tea, alcohol, and chocolate as they are detrimental to the already affected stomach.
  2. Eat right: Even on a regular basis, you are what you eat. So, when your stomach is already infected, eating right assumes a lot more importance. Go easy on the stomach and give it something easy to digest, yet of high nutritional value. Banana, rice, apple, and toast (abbreviated as BRAT) is very easy and packs in good nutrition for the recovering stomach. Avoid a lot of fibres, fatty foods, greasy foods, sugars, and dairy products which are hard for the stomach to digest. Eat small meals, but eat a lot more frequently than you otherwise would.
  3. Rest well: With the body losing a lot of fluids and nutrition along with it, the body would naturally feel very tired and fatigued. Let the body run its course, and give it the rest it demands. In the next couple of days, once the virus has run its course, this will resolve.
  4. Medications: Stomach infection is usually caused by a virus, and so antibiotics do not play a curative role here. However, if you have high fever or body aches, Tylenol or acetaminophen can be used sparingly. If diarrhoea or vomiting is severe, loperamide or promethazine can also be used. However, this should be substituted with sufficient rest and nutrition (solid and liquids).

If these are not helping and you are not able to keep the food down, visit the doctor. Sometimes, a day or two in the hospital may be required for complete cure and recovery. If you wish to discuss any specific problem, you can consult a general physician.

Hypertension - Complications and Treatment

MBBS, MD - Internal Medicine
General Physician, Delhi
Hypertension - Complications and Treatment

Hypertension or high blood pressure is very common disease. One is labeled as having hypertension when the patient's BP is more than 140/90.

Hypertension is of generally 2 types.

  1. Essential hypertension: No particular reason is found. This is the most common type And usually due to life style habits.
  2. Secondary hypertension: Here the BP goes up because of some other disease And once the main disease is treated the bp also becomes better.

Some causes of secondary hypertension are

  • Kidney diseases
  • Hormonal problems like thyroid disease, cushing's syndrome
  • Certain medications
  • Some rare tumors like adrenal tumors, pheochromocytoma etc.
  • Pregnancy
  • Alcohol abuse
  • Drug addictions

Complications

Uncontrolled hypertension for long period can cause life threatening diseases like

  • Heart attack.
  • Paralysis due to brain stroke
  • Kidney failure
  • Blindness

Treatment

Treatment consists of two parts.

  • Lifestyle changes
  • Medicines

Lifestyle changes include

  • Low salt diet
  • Eating fresh fruits and salads as at least one third of you diet as these are rich in potassium which helps to maintain low blood pressure.
  • Exercise daily, such as walking for at least 45 minutes everyday.

Medicine

  • That would depend whether you have essential or secondary hypertension.
  • In essential hypertension, No one medicine is for all. Your doctor will individualize the medicines as per your blood pressure, your blood report results and any Co existing disease like diabetes or kidney problem etc.

Goal of treatment

  • Is to keep your blood pressure below 130/80 at all times And to prevent complications.
  • Remember hypertension is deadly. However it can be easily managed. Lifestyle changes And medications complement each other. They do not substitute each other.
  • With proper individualization of lifestyle changes And medications you can enjoy a normal and healthy life. If you wish to discuss about any specific problem, you can consult a general physician.

Dear lybrate-user. It is nearly impossible to get pregnant during periods. So relax. However there is nothing 100% in medical science. From next time please use protection like male or female condoms. Intercourse during menses increase your risk of having infections. Please keep that in mind too regards.

Living With Diabetes - Diet, Lifestyle and Foot Care Changes You Must Ensure

MBBS, MD - Internal Medicine

Living With Diabetes - Diet, Lifestyle and Foot Care Changes You Must Ensure

Diabetes has become a modern lifestyle disease. Younger age groups are affected by it, and with changing lifestyles, it is only poised to get worse. While this may sound scary, the news to cheer about is that it can be controlled to a large extent. By making lifestyle changes, altering diet, and improving foot care, diabetes control can be planned effectively. Both the onset of the disease and disease progression and complications can be controlled to a large extent.

Read on to know some changes in terms of lifestyle, diet and foot care to improve diabetes:

  1. Diet: You are what you eat, so it is advisable that you plan your meal carefully. Eat healthy and you will stay healthy. Include a lot of fiber in the form of whole grains, fresh fruits and vegetables. Reduce oils, sugar, salt, white rice, refined flour, etc. Increase unsaturated fatty acids and reduce saturated fatty acids. Ensure you get the right amount of antioxidants, vitamins, and minerals. Ensure ample hydration for the body. Get into a pattern of eating small, frequent meals and avoid binge eating. This goes a long way in controlling sugar levels in the body.
  2. Lifestyle: Engage in exercise, both to keep your weight under control and to improve the function of insulin. This also helps to keep body fat under control. Talk to your doctor about what routine would work for you. Whether it is jogging, running, cycling, swimming or plain walking, take a pick and regularly do it. Your goal should be to spend 30 to 45 mins every day on brisk walking (at least 5 days a week). This helps the function of insulin and also helps in reducing complications arising from diabetes. If you are a smoker, this is the time to quit – the hazards and the benefits are way too many. A healthy lifestyle with exercise also helps to manage stress, as the exercise helps in stress management and weight management too. Reduced severity of diabetes induces a feeling of well-being too.
  3. Foot care: One of the main complications of diabetes is known as diabetic neuropathy, where the nerves are affected. There is loss of sensation in the extremities and therefore a blister or a corn on the foot can go unnoticed and continue to grow. This may develop into an ulcer, which may then require extensive repair and heal at a slow pace because of diabetes. Diabetic people are advised to watch and wash the feet regularly, dry it nicely, watch between the toes, keep the toenails trimmed, smoothen out the calluses and corns, and use proper footwear. Go for foot exams once in six months or a year. Any small wound should be attended to by the doctor.

These are some simple ways to combat diabetes, which can be planned into your daily routine. If you wish to discuss about any specific problem, you can consult a General Physician.

Dear lybrate-user, please remember that doctors do not treat tests. They treat the patients. So first tell me what problems are you having. And why did you do the test? If you are not having any symptoms and still you have high uric acid levels, nothing needs Tobe done unless and until it is more than 12.5 mg/dl. Suffering from Chronic Cough - How to Manage it?

Suffering from Chronic Cough - How to Manage it?

Coughing is a common problem of the respiratory tract and often indicates a deeper problem. It can be very irritating and can affect eating habits and socialization. A cough which persists for more than 4 weeks is believed to be chronic in nature. Tuberculosis (TB) is the most common cause of chronic cough in India. TB is an airborne bacterial infection caused by the organism, 'Mycobacterium tuberculosis', that primarily affects the lungs, although other organs and tissues may be involved. Cardinal features are:

  1. Chronic cough with or without sputum.
  2. Fever more in the night time.
  3. Poor appetite.
  4. Unintentional weight loss

Read on to know about other common causes of chronic cough and how to manage them.

  1. Allergies: People with allergies usually have a chronic cough, which gets aggravated when exposed to the allergens such as pollen or environmental dust. Staying away from allergen provides relief. Antihistamines are also useful, which most people detected with allergies keep in handy. Inhalers might be required, if there is associated congestion or shortness of breath.
  2. Asthma: This is a very common chronic condition, caused by inflammation and swelling of the lungs, in addition to wheezing and shortness of breath. Asthma patients often suffer from a chronic cough. An asthma attack usually happens during the night or early morning, and is triggered by colds, cigarette smoke, air pollutants or allergens, which can even include certain foods. Most asthmatics usually carry their steroids, inhalers, and bronchodilators, as the onset of attack can be unpredictable.
  3. Bronchitis: This condition of inflammation of the bronchial region is very common among smokers and among the elderly. There is excessive mucus accumulation, which the body tries to clear via coughing. The lungs are weakened and therefore, the coughing is painful.
  4. Gastroesophageal reflux disease (GERD): This is a problem of the digestive system, but regurgitation of food into the airway induces cough. GERD is a chronic issue and so most people also end up with a chronic cough. It is worse with sitting up than with sitting down. Treatment includes H2 blockers to suppress acid production and dietary changes including small, frequent meals and avoiding spicy food items.
  5. Upper airway cough syndrome: There could be multiple problems in the upper airway, which leads to a constant postnasal drip. Here, there is constant dripping of the sputum into the air passage, which can lead to a chronic cough whenever there is impaired air flow. This can be caused by various forms of chronic sinusitis and rhinitis. People diagnosed with this condition usually resort to antihistamines and decongestants for relief.
  6. Other causes: There are a number of other causes as noted below. Smoking, tuberculosis, congestive heart failure, pertussis or whooping cough, foreign body in the airway, cancer in the airway passage, chronic aspiration of foods, etc. can all lead to chronic cough.

The key is to identify the reason that is causing the chronic cough. Most people who are diagnosed are educated to handle it and carry the required medications including steroids, bronchodilators, and inhalers. If you wish to discuss about any specific problem, you can consult a doctor and ask a free question.

Vaccinations are Not Only for Kids

Vaccinations are Not Only for Kids

Dear readers
When was the last time you got vaccinated.
Probably when you were a kid. Or after an injury you might have got a tetanus shot.
However with evolving medicine vaccinations in adults have a major role to play.
Who should take what?
Well that depends on what other diseases you have like diabetes or respiratory conditions or kidney disease or liver disease etc.
However routine adult vaccinations that almost any adult should take are
1. Seasonal influenza vaccine or flu shot. This saves you against swine flu and 2 other common influenza virus strains.
Frequency- once a year
2. Typhoid vaccine - it has been around for some time. But there is a newer version. Which has much more effectiveness in preventing typhoid.
Frequency-once in 3 yrs
3. Pneumococcal vaccine- the newer vaccine prevents you against multiple strains of pneumococci and as the name suggests it will prevent you from severe pneumonia.
Frequency-once in a life time.
4. Hepatitis b- if you have not taken it already then this the time to take. Prevents against hepatitis b. If hepatitis b occurs it can cause chronic liver disease and even liver cancer. The disease can even be spread by sexual routes or contaminated blood transfer. It is about thousand times more easily spread than compared to HIV.
Frequency - 0, 1, 6 months. Booster dose after 10 yrs.
In conclusion as we all know-how prevention is better than cure.
Upcoming vaccine-dengue vaccine, very eagerly awaited.

I have itching problem.
Please tell azithromycin & isotretinoin dose take or not?
Dear lybrate-user itching is just a symptom and a common symptom but with many reasons. Azithromycin is an antibiotic isotretinoin is an anti metabolite. Both medicines are commonly used for acne or pimples. None of the above medications is indicated for symptomatic treatment of itching. Besides both the drugs can have serious side effects if taken without a doctors prescription. So definitely do not take these medicines regards.

Dear lybrate-user, congratulations on the great initiative taken. Yes these are certain effects that your body's showing once it gets rid of the toxins. It's likely to persist for next 3 to 4 days. However your appetite should improve earlier. I am sure that you couldn't have lost 4 kgs in just 3 days. The colour of your stool is because of what you ate. Whatever you do, do not stop doing the great work that you have done by quitting to smoke. Keep it up. For any further information or help you can consult me privately on Lybrate.

cost-phrenic angle unsharp in my right lung. My recent x-ray reports sugest but my x-ray is clean. No sign of any lesions or effusions. I earlier had pleural effusion due to koch's origin. But I became totally cured. And it my treatment for it ended in october only. But I was facing some little bit of irritation in my chest right side so I got a x ray and it suggests dat costophrenic angle is unsharp. Does it means that I have MDR TUBERCULOSIS this time? Or is it some sort of normal chest infection or cold I have got? Well I do not have any chronic cough or low grade fever or any sort of fatigue and loss of appetite or weight loss. Then what is it?

Dear Mr. lybrate-user whenever you have a cut on the skin or anywhere else, when it heals it leaves a scar. This scar night itch sometimes but it does not mean that the cut has not healed. Similarly the pleural blunting seen is a scar of the tuberculosis that you had before. It does not mean that you have MDR TB. It might cause some irritation and pain from time to time. But relax it is quite normal. And this scar is going to remain so do not continue getting X-ray's done. Regards.

Dear lybrate-user, nervz b is a vitamin b Complex tablet. Probably will have some effect. However sciatica is a chronic problem. If the pain is so much that it is hammering her daily activities and or producing difficult in walking or any loss of sensation in the leg then she might need long term medications and further tests like an MRi of the lower spine. However the most effective therapy is physiotherapy. If you have done any tests please upload it and give some more details that I have asked for in the para above.
