Hello, I am Dr Parinita Kalita, consultant gynaecologist practising in Delhi, NCR. Today I will discuss about pre-pregnancy counselling. Once a woman decides for pregnancy, she should visit a gynaecologist to know her optimum health condition because even in pregnancy is a physiological condition there may be certain associated diseases which are preventable and gives a better outcome. The history part is very important we try to find out if there is a certain familial disease like diabetes, high blood pressure, thyroid problem, thalassemia. Her haemoglobin should be optimum before conception and if it is not so if she is suffering from anaemia it should be corrected before conception. Thyroid values should be normal before and during pregnancy for a better outcome, her vaccination schedule is also very important she should be immune to rubella because if rubella comes a first time during pregnancy it may give rise to a congenital anomaly of the fetus. So if she is not immune she should be advised rubella vaccination 2 months prior to her pregnancy, her drug history drug intake history is also very important she should not expose herself to unnecessary drugs before and during pregnancy and if she is into a continuous drug resist for certain diseases, she should consult her Doctor to find out whether those tablets, drugs are safe during pregnancy or if she requires some alternative medicines for those diseases. Her hygiene is another important part of the private part, she should discuss this and if she feels that there is a certain infection in that part it should be properly treated before going for a pregnancy. She should know what to do and not to do during those periods, her diet should be very healthy and she should continue with the light exercise. She is advised few test like haemoglobin level, for thyroid value, for rubella vaccination, her thalassaemia profile, if she complains of any irregular period or excessive pain during period her ultrasound is also advised to find out if there is certain ovarian cyst or a fibroid uterus which should be taken care before planning for pregnancy. Smelling Lemon - Does It Help Morning Sickness During Pregnancy?

Morning sickness refers to the nausea experienced by a woman during the first couple of months of pregnancy. Even though it's called morning sickness, it can happen at any time of the day. Starting around the fourth week of pregnancy, and lasting up to the 14th week, the unpleasant symptoms of morning sickness can be managed in the following ways:

  1. By Smelling a Lemon - Pregnancy can make you highly sensitive to different types of smells, and certain types of scents, foul odour to be precise, can be major factors that cause nausea. This can be treated by inhaling a fresh aroma, like that of lemon extract or a fresh sprig of rosemary, or any other smell you find pleasant.
  2. By Drinking Loads of Water - Dehydration or drinking less amounts of water than what your body requires at this stage of your life can lead to increased feelings of nausea. Thus, it’s extremely important to stay hydrated. Although, drinking the standard 8-10 glasses of water when you feel like vomitting might seem like a challenge, there are certain ways in which you can trick your body to increase its fluid intake. Research shows that certain salty snacks (cheese or chips for example) can trigger thirst in pregnant women, while reducing feelings of nausea. Even munching on ice chips or sipping on ice cold water or a hot beverage can do the trick, depending on what works for you.
  3. By Eating Something that Works for You - Even though nausea can be a hunger-killer, eating something that doesn’t make you vomit can actually help control the symptoms of morning sickness. This may be different for each pregnant woman, although certain carbohydrate rich, bland tasting foods such as dry cereals, ice cold watermelons, popsicles and crackers have proven to be very effective in controlling morning sickness. Greasy, fatty and spicy foods should be avoided as they can upset your stomach.
  4. By Having a Little Bit of Ginger - Ginger can be highly instrumental in soothing your upset stomach, and can even quell feelings of queasiness. Just try having ginger tea whenever you feel nauseous. You can even add a thin slice of ginger to your water or have ginger soup, ginger ale, ginger candy or gingerbread; each of these ways can put a stop to the nausea.
  5. By Lying Down - The simplest and easiest way to fight off the nausea is by lying down. Closing your eyes and taking deep breaths as you do so can relax your body. Sometimes morning sickness can be a result of lack of sleep and rest, hence catching a nap can be the best remedy.

Pregnancy is a normal life function that many women go through. Yet, this is a function or a part of life where one has to take special care of one’s health and condition as there are many biological changes that happen in one’s body. Swollen legs and ankles are a common part of one’s pregnancy. This condition is most commonly known as edema and it usually occurs when fluid collects in the tissue. This in turn happens when the blood chemistry changes and the fluid gets routed to the tissue. This is a common occurrence due to the hormonal changes that one goes through during pregnancy. The condition usually troubles pregnant ladies during their third trimester.

Read on to find out the various ways in which one can deal with it.

  1. When to call a doctor: Call a doctor or a midwife for a thorough check up if you happen to see any puffiness or swelling in your face and eyes. This could point at a more serious problem like the onset of preeclampsia. Also, if you happen to notice that one leg is more swollen than the other, then you may want to see a doctor about it, as it could mean that there is a blood clot in the leg.
  2. Put your feet up: This is especially important if you are at a desk all day. The best way to deal with such swelling is to put your feet up every once in a while, so that the swelling may reduce. You can use a foot stool or even a pile of books for this purpose.
  3. Breaks: If you are constantly sitting or lying down, it would be a good idea to give it a break and walk around a little so that there is better blood circulation. This also keeps the blood from pooling into the lower regions of the legs, which can prevent an increase in the swelling.
  4. Comfortable footwear: One should stick to flat and comfortable footwear with an even elevation as well as foam soles so that the comfort level while walking and standing is optimum. Choose your shoes so that they can accommodate your swelling instead of pinching your feet. Also, you can choose to wear snug socks and stockings, which will form a band around your feet. This will ensure that the swelling does not increase to a painful level. At the same time, you must avoid socks that have bands, which are too tight as this may lead to blood pooling and subsequent swelling in the legs and ankles.
  5. Water: Drinking plenty of water ensures that there is less fluid retention. This is one of the best ways of fighting edema whether or not you are pregnant.
