Hello, thanks for the query. I have seen the details mentioned. It is clear that current treatment is adequate. There is a need to look at all the aspects of it. That is your diet, extent of daily exercise, plus drugs with dosage, plus lipid profile, serum creatinine, urea and bp readings. Please come back with all the necessary details in a private conversation, where I will be able to give specific guidance as well as prescribe the necessary drugs including additional insulin. Because in this communication we are supposed to give only general guidelines. No drugs can be prescribed. If you are not comfortable with on line consultation please consult a good diabetes specialist in your place. It is very essential to achieve a strict glucose control to prevent or postpone long term complications associated with diabetes affecting all the vital organs & systems. So please take early action. I am sure you are aware that ideally fbg should be < 100 mg or closer tp it. Pp should be 150 to 170 mg & hba1c% shpuld be < 6.5% considering your age. Thanks.

If you're having ckd hba1c is not a good option to assess your sugar control. I would suggest you shift to a basal bolus regime with a continuous glucose monitoring device or a glucometer readings at least 5 times a day. Please consult an endocrinologist.
