Doctor, good morning. I am type-2 diabetic and taking vildagliptin-50 mg & metformin-500 mg both after breakfast and after dinner & glaritus insulin 10 units at 9 pm. My fasting levels are 130-140 and post-lunch is 220-245 and hba1c is 7.4. What dose or medicine should I consider to achieve goal of 110 fasting, 160 post-lunch and 6.6 hba1c. I appreciate you help on this. Regards allopathy / diabetic doctor only please.

Increase glatirus insulin 14 units. It will control both fasting and p.p. Blood sugar brisk walking regularly take lot of salad before each meal. Fruits of low glycemic index in between meal.

I personally suggest you have strict diet control along with less carbohydrate intake as I can notice your post-meal blood sugars are on the higher side, in my opinion, su can also be used.
