Follow up case of recurrent l4-l5 disc herniation - post operative status l4-l5 bilateral laminectomy with small adjacent fluid collection measuring ~ 2.4 x 1.2 cm near the post-op site. Diffuse bulge with discectomy change of right paracentral herniation seen. In-addition there is new left forminal small focal disc protrusion indenting upon exiting left l4 nerve root, which appears mildly thickened and inflamed. No evidence of active spondylodiscitis. After 2 times spine surgery still suffering radiating pain in my lumber region.

From the report it appears there is still some irritation of the nerve root. There is a combination of pressure on nerve root from a recurrent disc prolapse and thickening of nerve from inflammation/early scar formation. When was your surgery? Though report days there is further disc, looking at the scan reports will help in knowing what exactly is the issue. You should see an orthopaedic surgeon who specializes in spine surgery for advice.
