The patient is on paroxetine 75 mg and bupropion 300 mg. Patient has ocd and and to manage the thoughts, olanzapine 2.5 mg had been prescribed along with antidepressants, but it made the patient devoid of emotions. Next we tried arpizol (ability) 2 -5 mg but it increased the anxiety hence had to be discontinued. Now the patient has been put on risperidone 1 mg but in one week, it seems that it is exacerbating the mood as well. Thus shall we continue trying antipsychotic? Is there any other antipsychotic with least side effects? Thanks a lot.

Hello, In severe ocd, small dose of anti-psychotics are added to augment ssris/antidepressants. If patient is not tolerating risperidone 1 mg, discuss with your doctor to cut down to 0.5 mg and see whether it helps. Also make sure the patient gets cbt for ocd. In the long run it will decrease dependence on medication hope this helps.

It is not recommended that the patient continue the antipsychotic since it seems to be exacerbating the mood.
