Fissure is cracking down of skin at external anal orifice whereas Fistula is bling tubular structure opening externally in Peri anal region and traversing through wall of analcanal which may or may not open in anal canal cavity. Fistula is more harmful. Commonest dietary precautions is to avoid frequent constipation and staring while passing stool. Only surgeon or proctologist can diagnose by rectal digital examination and if required by doing proctoscopy.
