Mr. lybrate-user, Thanks for the query. TSH 8.9 mU/L is high. This can lead to increase in weight plus there may be other symptoms of hypothyroidism. Usually for most patients the advise is to maintain TSH < 3mU/L to remain free from any symptoms of hypothyroidism. But if there are heart related issues slightly higher level of TSH is good enough. Because of reduction of dosage of thyroxine from 150 to 125 mcg, TSH has gone up. My suggestion is increase the dose to 137.5 mcg per day and then check TSH after 6 to 8 weeks. Thanks.

Mr. lybrate-user Kumar, Thanks for the query. When a person is on treatment with adequate dose of Levothyroxine, then TSH values is maintained <3 mU/L, at that all the symptoms of hypothyroidism are in check. Since her TSH is now 6.05 mU/l, symptoms are bound to reappear. So she needs to increase the dose. To calculate the dose following details are needed: T4, & TSH values, weight and height of the patient. Thanks.

Male Infertility - How It Can Be Managed?

Male Infertility - How It Can Be Managed?

Male infertility is when the male is not able to get a fertile female pregnant. Though not openly admitted, it is an extremely common condition. For a man, it is highly hurting to the ego when it is identified. Given the discrete nature of the topic, it is difficult both for a man to admit that he could possibly be the reason the couple is not able to conceive. If that happens, the next challenge is to get them to undergo treatment.
With times, things are changing and more men are open to getting tested. The changed lifestyles are also contributing to male infertility, and the incidence has risen by a couple of percentages.

Causes: Male infertility is caused by a variety of issues leading to poor sperm quality and/or quantity, ranging from:

  1. Sperm production, which can be reduced in numbers.
  2. The quality of the sperm could be poor, with reduced mobility being extremely common. When this happens, they are not able to travel up the uterus and penetrate the egg.
  3. Erectile dysfunction, where the erection is too soft or short for penetration into the female
  4. Premature ejaculation, where the sperms are released very soon before they can enter the female.
  5. In addition, lifestyle causes are another major cause of male infertility. These include smoking, increased stress levels, alcohol abuse, lack of exercise, poor dietary habits, and increased junk food.

Management: Male infertility is not as easily accepted as female fertility, though it is the cause for the couple’s inability to conceive in about 50% of the cases.
Once identified and accepted, treating male infertility is quite easy compared to female infertility. In order of progression, the treatment modes are listed below.

Counseling: In most cases, a sexual counseling session with the couple can help iron out performance anxiety and help build trust and frankness in the relationship. In many couples, this helps in overcoming the problem. Given the discrete nature of the topic, it takes time for a couple to get to know each other. There is also the male performance anxiety which can lead to poor performance. All this needs some time and some external help, which can improve the sexual compatibility and overall relationship.

Hormonal therapy: This is another simple and effective way to manage male infertility. Topical or systemic androgens can help in treating male infertility.

Surgery: In worse cases, where there are blocked sperm tubes, surgery may be required to unblock them.

In vitro fertilization: In couples whom none of the above work, IVF could be used to help in conception.

Male infertility needs to be accepted and then it becomes easier to treat.

Substance Use In Youngsters And Adolescents!

Substance Use In Youngsters And Adolescents!

Substance use among adolescents is on the rise and is happening at a very young age. A study conducted by NIDA and SAMHSA portrays some alarming statistics. The percentage of substance use among school kids has gone up by 11.33 percent. This is close to a 60 percent rise compared to the previous decade. Around 9 percent of girls and 20 percent of boys meet the adult diagnostic criteria for alcohol use. Furthermore, 20 percent of high school students were found to be daily smokers. Studies also found that while the percentage of overall abuse of the drug is steadily decreasing over the years, the availability of drug has increased to a considerable extent.

The consequences:

  1. Mortality: Motor vehicle accidents caused by drunk people have increased in the past couple of decades. Road accidents under an increased influence of alcohol have risen by a staggering 16 percent as per a CDC report. Annually, the country witnesses close to one million road accidents by adolescents and young people in the age group of 20. This is by far the biggest economic loss – a loss of productivity to the society.
  2. STDs (sexually transmitted diseases) - The sexual health hazards of adolescents remain high due to the possibility of acquiring STDs. Substance use by adolescents and young adults often leads to unprotected sex with the partner, choosing multiple sex partners and choosing partners who have a high potential of STDs. Studies have shown that young females in the age group of 15-19 suffer from gonorrhea and those in the age group of 20-24 suffer from syphilis due to high usage of substance.
  3. Crime and juvenile delinquency: There is a strong co-relation between substance use and conduct disorder among young people. Many young individuals entering the juvenile justice suffers from an array of problem ranging from substance use disorder, impaired emotion, sexual victimization, physical abuse etc. There is a growing trend of criminal activities among young people who are involved with substance use.
  4. Developmental problems: Multiple studies conducted by the Newcomb and Bentler (1989), Havighurst (1972) and Baumrind and Moselle (1985) have proved that substance use is a major barrier to the development for young people. Activities such as establishing a career, dating, raising and bearing a child, building a relationship etc. are affected because of the way people experience feelings.

Treatments: Administering treatment to a young guy suffering from substance use is of paramount importance. It helps to ensure the possible future abuse of drugs. While there are many treatment options, the one that is applicable to a drug addict depends largely on the present condition and the advancement of the effect of the drug on the body. Most doctors start the treatment with medication and lifestyle changes and suggest community service. Self-help groups also help a great deal.

What is Scarring Alopecia?

Dermatologist
Dermatologist, Delhi
What is Scarring Alopecia?

Scarring Alopecia, otherwise called Cicatricial Alopecia, alludes to a gathering of hair loss issue that might be diagnosed in up to 3% of hair loss patients. It happens worldwide in generally healthy men and women of any age.

Every particular diagnosis inside this class is genuinely uncommon, yet a few cases incorporate analyzing Cellulitis, Eosinophilic Pustular Folliculitis, Follicular Degeneration Disorder (beforehand called "Hot Brush" Alopecia), folliculitis decalvans, lichen planopilaris, and pseudopelade of Brocq, to give some examples. Scarring Alopecia may likewise be a piece of a significantly bigger condition, for example, Chronic Lupus Erythematosus, where numerous organs of the body can be influenced.

While there are many types of Scarring Alopecia, the normal topic is a conceivably changeless and irreversible demolition of hair follicles and their Replacement with scar tissue.

Most types of Scarring Alopecia initially happens as little Patches of Hair Loss that may extend with time. Now and again the hair loss is steady, without recognizable indications, and may go unnoticed for quite a while. In different examples, the hair loss is related with Serious Itching, Burning, and Pain, and is quickly dynamic.

The Scarring Alopecia Patches normally appear to be somewhat unique from Alopecia Areata in that the edges of the Bald Patches look more "battered." The annihilation of the hair follicle happens beneath the skin surface so there may not be much too really observe on the scalp skin surface other than Patchy Hair Loss. Influenced areas might be smooth and clean, or may have redness, scaling, expanded or diminished pigmentation, or may have risen rankles with fluids or Pus originating from the influenced area.

These visual markers may help with diagnosis; however it is hard to diagnose a Scarring Alopecia just from the pattern of the hair loss and the nature of the scalp skin. Regularly while Scarring Alopecia is suspected, at least one Skin Biopsies are done to affirm the diagnosis and help recognize the specific type of Scarring Alopecia. A little biopsy of 2 to 4 mm in distance across is taken and analyzed under a microscope. A Pathologist or Dermatologist will search for annihilation of the hair follicles, scar tissue deep in the skin, and the presence and area of aggravation in relation to the hair follicles.

Regularly, the early phases of a Scarring Alopecia will have Inflammatory Cells around the hair follicles, which, numerous scientists believe, actuates the demolition of the hair follicles and advancement of scar tissue. In any case, there is some argument about this among Dermatologists, as in some cases a biopsy from a Scarring Alopecia person shows very low irritation.

Scarring Alopecia quite often burns out. The Bald Patches quit growing and any irritation, tingling, burning, or pain goes away. In this end phase; another skin biopsy generally demonstrates no inflammation around hair follicles. Bald areas more often than not have no more hair follicles. Here and there, however, hair follicles, in any event those at the fringe of a Bald Patch, are not totally annihilated and they can re-grow, but rather frequently all that is left are only a couple of Longitudinal Scars deep in the skin to indicate where the hair follicles used to be.

Treatment

Scarring Alopecia can include a great deal of harm and lasting Hair Loss. Hence treatment of Scarring Alopecia ought to be very forceful. The nature of treatment changes relying upon the specific Diagnosis. Scarring Alopecia’s that include for the most part Lymphocyte Inflammation of hair follicles, for example, Lichen Planopilaris and Pseudopelade, are by and large treated with Corticosteroids in topical creams and by Injections into the affected skin. Likewise, Antimalarial and Isotretinoin Medications might be utilized.

For Scarring Alopecia’s with aggravation of generally Neutrophils or a blend of cells, treatment includes Antibiotics and Isotretinoin. All the more tentatively, drugs like Methotrexate, Tacrolimus, Cyclosporin, and even Thalidomide have been utilized to treat a few structures.

Once a Scarring Alopecia has achieved the burn out stage and there has been no more Hair Loss for a couple of years, Bald areas can be either surgically expelled on the off chance that they are not very huge or the Bald Patches can be transplanted with hair follicles taken from unaffected areas.
