Slowely reduce the dose of the previous drug and at that time start your new drug at a small dose .both drug should use in small dose and reduce dose slowely .when you use small dose of your previous drug at that time you should also take the new drug therefore there is less chances of brekthrough seizures .if you want to explain it further then I will explain you on call and give you the methods from which you can change your drug.
