I am hasib 48 years old, I am diagnosed blood sugar in 2007, earlier it was under control but now it was not controlled by 2 years. I am taking geminor 2 twice a day before half an hour of lunch and dinner. I feel very lethargic, tired, sleepy, confused. Kindly advised. Thank you.

thanks for the query. I have seen the details mentioned. However, the information given is very general. There are no necessary details like fasting, pp glucose levels, hba1c%, lipid profile, dietary pattern, extent of daily exercise, bp readings. By simply mentioning "sugar not in control" it is not clear at all. So please come back with all the details as mentioned, preferably in a private conversation (paid consultation). If blood glucose is not in control, then patient does feel tired, lethargic and fatigued. So it is essential to achieve strict control ideally in a well controlled patient fbg should be < 100 mg, pp should be 150 to 160 mg & hba1c% < 6.5%. So please do the needful. Thanks.

Hello this practising geriatrician diabetes setting at younger age ie <35 years of age, tend to respond good to drugs at the start and in the later part, the insulin reserve will be low would advise you to do c peptide, tsh and hba1c and creatinine if c peptide levels are low, would be better to add long acting insulin as an add on agent need some more test to rule out other differentials for his symptoms like tiredness.
