Hello this practising geriatrician diabetes setting at younger age ie <35 years of age, tend to respond good to drugs at the start and in the later part, the insulin reserve will be low would advise you to do c peptide, tsh and hba1c and creatinine if c peptide levels are low, would be better to add long acting insulin as an add on agent need some more test to rule out other differentials for his symptoms like tiredness.
