The patient is on highest dose of paroxetine for past three years. He has mild ocd and dysphoria. Thus to manage the thoughts of ocd (moderate) olanzapine 2.5 was given but it made patient emotionless. Then the doctor put me on arpizol (ability) 5 mg but results were the same. Recently we tried risperidone 1 mg and it exacerbated my dysphoria and I was again emotionless. Could you suggest me a good antipsychotic to be concomitantly used for managing thought. Or shall I continue trying antipsychotic? Do not wish to go for cbt. Thank you.

Dear Ankur, There are plenty of choice of antipsychotic. You can try one by one for at least 3 weeks in low dose. Something will be suitable to you. Don't decide with only 1-2 tablets. At times paroxetine can make you emotionless. Change to another SSRI may be helpful.

risperiodne is the best antipsychotic to act on OCD, but paroxetine is not the 1st line SSRI. Get a second opinion and right combination of medications for best control of OCD
