My father has stent operation one year back. Is there any chances to form artery blockage again as he is diabetic patient and using medicine regularly?

Not all stents will function well, there is about 10% chance in first year, and about 2% per year for next ten years of a stent failing. The other arteries can be protected with proper diet and some medicines. Reduce items rich in glucose, be willing to increase low glucose foods.
