Many factors are to be ensured for widal interpretation. Pathological lab should be recognised as standard like iso certified. Tests are ideally done in second week of typhoid, meaning not before 6th day of fever considering around 2 days as incubation period. Clinically typhoid is not being presented in a typical manner like progressively, intermittent high fever. Sometimes malaria is also giving positive widal titre. Hence widal 'o' titre above 1: 160 and preferably 1: 320 to be repeated again to show progressively increased titre strongly suggests typhoid fever. I think decision in this background is to be left to treatng physician.
