I have been on paroxetine 75 mg and olanzapine 5 mg a day, in connection with anxiety and dysphoria. I was still feeling low on these medications and needed an addition. We tried a lot of antidepressants and antipsychotic but none of those worked on me. Thus my doctor increased paroxetine to 100 mg and after much thought has put me recently on arcalion (sulbutiamine) 200 mg a day. 1. I just wish to know in how many days I will start noticing the good effects of sulbutiamine 200 mg? 2. I am on 3rd day and feeling that anxiety has increased slightly. How long shall I wait to gauge the efficacy of sulbutiamine? Thank you. Do not wish to go for therapies.

You will probably need the addition of a mood stabilizer like lithium or valproate if you've never tried these and have been feeling low since a long time. Addition of those drugs is the standard line of management if we consider your illness as dysthymia / resistant depression. Do ask you doctor for about the same.
