Hello Dr. Good morning. My wife is in 5th week of pregnancy now. She is a thyroid patient. Now her TSH is 4.96. And our gynecologist suggested for 75 mcg to 100 mcg of thyroxine. Can you please suggest me what is the minimum for pregnancy 1st trimester range. And is 100 mcg is enough to control the thyroid in pregnancy. Thank you in advance. Have a great day. Regards

Dose of thyroid hormone depends up on several factors in pregnancy like gestation period, level of TSH, if you have any doubt take a second opinion from a Physician or Endocrinologist.

THyroxine is started in small dose and increased as per the need. Increases in levothyroxine dosage administered in pregnancy appear to be indispensable in the majority of patients with well-controlled hypothyroidism, especially in the first trimester. However, this change was not universal and levothyroxine dosage decreased in a few cases and remained unchanged in others.Adequate Levothyroxine Doses in Pregnant Women With Newly Diagnosed Hypothyroidism Hypothyroidism Type Baseline TSH (mIU/L) LT4 dose (g/d) LT4 dose (g/kg/d) Group 1aa (subclinical) 3.56 77.98 1.20 Group 1bb (subclinical) 5.43 95.35 1.42 Group 2 (overt) 40.33 147.08 2.33
