Respected Lybrate user. Please be continued to your physical therapy for your problem and regular check-ups/ follow-ups. As also orthopaedic doctor's check-ups / follow-ups. But remember one thing. Do home ex's, do postural corrections, ergonomical precautions, don't gain weight. If you're already gained. Please reducing it. Eat immune system boosters and low calorie diet. Take an advice on dietitian and take diet chart regularly follow this.

Hi, bulging discs need not necessarily contribute to sciatica, try meeting up with a physiotherapist to resolve your problems. During menstrual cycle, muscle cramps are normal and they tend to aggravate back pain.

Take him to physiotherapy clinic, through electrotherapy it can be reduced.
The treatment time will depend on his body condition.
Few sessions of ift+us for two weeks will give him relief from sciatica.
Also he needs to concentrate on his weight reducing exercises after the treatment.
Hot pack twice a day with usage of diclifenac ointment will be helpfull.

According to me surgery is not required. I can assure you if you consult a good qualified physiotherapist you will not have any issues at all. What I can suggest you is 10 days of intermittent pelvic traction followed by interferential therapy further to that sciatic nerve stretch, however stretching hamstrings, calf and quadriceps muscles will definitely help for better improvement.
