It is difficult to answer your query without assessing nature, size etc of lump or glands which is possible only by personal examination. But what you have described at foot, that does not give me impression of Tuberculous lesion (not primary comp which is diagnosed in lung lesion). If his wt is about 16 kg and otherwise active and illness is not continuous of 6 months duration, it is unlikely tubercular. Even then get him properly investigate for tub lesion since its tt is continued for 6 months uninterrupted for complete cure.

You can go for investigations to rule out tuberculosis. Also the lympnodes might be enlarged due to local infection. A course of antibiotics if causes regression of size of lumps then tb is less likely. Also for primary complex (TB) the child must be having features of decreased appetite, weight loss, low grade fever, contact with person having TB or taking drug treatment for tuberculosis.
