Shoulder pain k liye I usually give this exercise when the patient has a clear reduction in pain with the use of the Scapular Assistance Test as described by Jeremy Lewis (source) or in those that I think have poor scapular movement, although links to scapular position and movement causing pain and pathology is tenuous (see Chris Littlewoods piece on this here), as is our ability to spot it. However, most people I see with classical sub acromial pain tend to have weak posterior shoulder muscles and this is a great exercise to start addressing this. The prone Y lift is simply done by lying on your front, on a bed or bench, with your arm hanging over the edge, then lift the arm up and out to raise it above your head, and I like to instruct that the thumb is on top pointing upwards to ensure the shoulder is in external rotation (see image below). Again as with the isometrics the exact angle of the lift can be varied, but for best results the research says 120� of abduction produces the stongest contraction in the supraspinatus (source) and upper/lower trapezius muscles (source). I also place a big emphasis on the eccentric action of this an most all other shoulder exercises. Eccentrics are known to produce higher forces in musculotendious units than concentric actions and maintain or increase myofibril length, all essential and positive effects of rehab.
