I am having right shoulder pain from past 3 months. After having lots of physiotherapy sessions, ift, ultrasonic and heating therapy. I have got my mri contrast done. The findings of mri are ?there is small focal t2istir hyperintense fluid signal intensity interstitial tear measuring 3-4 mm in mediolateral direction seen involving and distal fibers of subscapularis tendon near insertion s/o interstitial tear. Associated mild edema and tendon thickening?. Mild free fluid seen in subacromial subdeltoid and subcoracoid bursa. And impression of mri are ?interstitial tear of distal subscapularis tendon near insertion?. Please guide me on what needs to be done now or any medication which needs to be taken.

Regards Lybrate user. The findings in the mri indicate there could be a tear in 1 of the tendons of muscle in the front of shoulder joint. No previous history on how this tear occurred had been provided in the question. Hence, generic recommendation would be rest the shoulder by keeping the arm beside the body with forearm against your belly or just hanging along the side of body. Avoid any strong pulling or pushing force through the arm. You may try your routine activity (without forcing or lifting any weight). If you have to lift, ensure the weight is not lifted too away from the body. Keep it close to the body. Keep the weight to minimum. Try to keep things in your house (things that you normally reach for like clothing, self care items etc) below shoulder level to avoid raising your arm up everytime. Try to ice the shoulder for 10-15 minutes x 3-4 times a day, that will reduce your pain. These are just preventive measures to avoid further injury and help the tissue heal. It is recommended to consult an orthopedic physiotherapist or manual therapist to ensure you progress appropriately to get rehabilitated for the shoulder.
