Bad breath may be due to gum disease or sometimes due to digestive problems. Also due to unhealthy lifestyle habits and the type of food you eat. Brushing and flossing daily and use of anti bacterial mouth rinse will help in curbing the bad odour but will not eliminate it completely. To eliminate it one needs to get a scaling done at the dental office.
Sometimes long term medications are also responsible for bad breath.
And the other reason for this condition is due to accumulation of plaque and debris which is not easiely removed with your daily brushing, so consult nearest dentist for scaling.
