Good eve sir. am suffering dysthymia, anxiety neurosis, soft mania (sudden angry) and overactive bladder and l5 -s1 slipped disc last 20 years .jam taking some scars, snris.escitalopram is stomach upset. paroxetine is sexual did function, mirtazapine over sedation, fluoxetine is weak molecule, venlafaxine is sexual side effect. Recently doctor prescribed tab duloxetine30 (1-0-1, tab olanzapine 5 mg (0-0-1), tab divalproex500 (0-0-1) and clonazepam 0.5 mg (-0-01 ). I tired all medicines. But not working. 3 months regularly jam taking medicines and discontinued because side effects. Now jam 88 kgs belly stomach. Jam male age 40 years. Is duloxetine effective than other serious, snris.tcas. Why quetiapine not prescribed me. I fear to lithium. Aripiprazole why not. I am pharmacist.

Hi lybrate-user, medications itself cannot reduce all symptoms of dysthymia. Studies suggest combination of medications and cbt/talking therapy is better than medications alone. If there are improvement 7 out of 10 that would be good achievement rather than aiming for 10 out of 10 for eg in cbt we may look at what makes you feel angry then advice strategies to manage your anger.

Treatment of any illness is both pharmacological and non pharmacological, as a pharmacist, you may think, only a tablet will solve all your problems. It is not true. You need life style changes like diet and exercise. You may need therapy to detect early signs of recurrence and deal with it promptly. As a pharmacist, you may know that no drug is without any side effect. You need to assess whether the disorder is worse or the side effects are worse. All antidepressants need to be given with a mood stabiliser like lithium or divalproex in adequate dose (serum levels). Read pharmacology for dispensing drugs to others - follow your doctor for your prescription. Use non medical methods to tolerate side effects of drugs. Finally duloxetine may work better for overacting bladder.
