Hello!

I am Dr. Dimpy Irani. I am a gynecologist practicing in South Bombay. I am affiliated with all the major hospitals- Saifee Hospital, Breach Candy Hospital and Wockhardt Hospital. I have done my basic obstetrics and gynaecology course from KM h hospital after which I did my robotic registration from France and also did gynae aesthetic course through London, UK. I would like to talk about the recent advances in obstetrics and gynaecology. With the invention of drug delivery system, new endoscopic types of equipment, the introduction of robotics worldwide and many scientific innovations have changed obstetrics and gynae practice. Today we talk about major symptoms which Gynae patient comes about is Menorrhagia. Once upon a time it was all surgical intervention but now we have drug modulators for even fibroids. We have short surgical procedures like an introduction of IUD loops, TCRE which is like a day procedure and even if a patient requires surgical interventions we also have endoscopic surgeries. But now endoscopic surgery has moved on to robotics. Worldwide all major hospitals do robotic surgery.

I have been trained for the robotic surgery. We have a big robotic instrumentation at Saifee hospital where I do all my robotic surgeries. The big advantage of robotic surgery is- much decrease in the pain level of the patient, post-op the patient is totally pain-free ready to get discharged, very fewer complications, easy surgery, easy accessibility, 3D vision. So robotic surgery is here to stay. We also talk about current advancement which has come into gynaecology. We also have laparoscopic instruments. We have cancellations. We have new instruments like resection of the endometrium which was like not there before. Today we have the wide area of patients who come from adolescence to menopausal. All have their own problems. Adoloscents have their own issues cosmetic issues and polycystic ovaries and menopausal women, they are looking at their own benefits and avoiding any postmenopausal complication like osteoporosis and genitourinary problems.

We also have new machines for genitourinary complaints. So it is better to talk to your doctor rather than suffer with pain of menopausal and genitourinary complaints like urge incontinence when you are cannot control your urine. Let's move on to the Obstetric practice. Once upon a time when the patient was pregnant she was all into pregnancy, she would eat, she would forget about the cosmetic part and post-delivery she would realise that her body has taken a big toll. Right now No! Right now woman wants everything. Anti natally she wants to exercise, she wants to keep her cosmetics, skin, hair, dental everything in control. So we do have a full package where will look into the patient cosmetic physical health and also after delivery we also have a mummy makeup program. Today the major chunk of our consultations comes from infertile patients. Because of the pollution, stress, complications to late marriages, with women being educated and want to pursue their own career.

We have many options where we need to talk to your doctor- we have ovarian preservation, egg preservation, IVF where we look into older women we tell them not to waste time and go into an earlier treatment for infertility. So it is for patients to come to the doctor and talk with them and take early intervention and early treatment whether it is gynaecology, obstetrics or infertility treatment. And today we don't even treat patient by looking at the medical problems but we also take in total- what cosmetics they want? what are the issues they have? So it's a complete holistic approach which we today believe in obstetrics and gynaecology which would help wide area of patients from adolescents to young adolescents to women to menopausal women. If the blood flow is low at 27 weeks, it is not normal. It is very common in CKD patients. She should have regular doppler study for the fetus to keep a watch on baby growth. Also get steroid shot from your doctor for baby's lung maturity as baby is likely to be born premature. And should register her in good hospital with better icu and nicu facility.

There is no harm in eating ripe papita. In fact it is healthy for her and baby. But she should avoid eating unripe papita.

In pcod patients, ovulation is difficult if ur weight is not controlled. Try to reduce weight by at least 8 kgs to have better chance of getting pregnancy with treatment.

I have PCOS and I had pale brown/pink discharge a month ago, but nothing really came of it. Last Sunday I got a lot of brown discharge, almost the amount you’d get on a normal period. Then, mid-week small amounts of blood began to show but only when I wiped after peeing. And more recently the blood has been showing in the toilets and on pads/tampons, but not enough to fill one up. Today, I had clear discharge tinged with blood and then bright red blood. What is my body telling me and how can I stop it taking a toll on me mentally and physically?

Brown discharge which you had should be considered as menses. These kind of bleeding and spotting which you are having is because of hormonal changes related to pcod. As you did not mention your weight, it is difficult to advise. Also please mention about excessive hair growth and acne. Weight reduction is the only way to controll these symptoms. Also if thyroid levels are not tested yet, please get it done.

Likely reason for pain is ovulation. You should have intercourse during this period to get pregnant.

She should not become pregnant again as she has a history of rupture uterus. If she becomes pregnant, then her uterus will rupture again before 9 months and it would be life treatening to her. Only option is to go for surrogacy.

I am pregnant and my pharmacist told me to take 2 capsules of autrin daily is it right?
Autrin is iron tablet, to be only started after 4 months of pregnany. Within first 4 months, it will increase your nausea and vomiting. Double dose is given to only those who have less haemoglobin. If you are in first 4 months of pregnancy, please take only folic acid.

I had protected sex with my partner and I ejaculated in condom itself. But after sex. She rubbed her upper vagina on penis with having condom. But later we came to know that condom was felt down. So we worry that sperm touched her upper part or may be inside her vagina. Will she get pregnant? After 2dys or within 48 hrs of sex she took unwanted 72. And her periods was on 03rd. But she gt date on 02nd Dec and problem was she got bleeding only 1 day but regularly her bleeding was 2 or 2 and half days. Our sex was on 14th Nov. From Dec 15th she was suffering from vomiting and motion what was the problem please help us.

She is can not get pregnant with the act you described. Also as she took the pill, it will prevent pregnancy. Menstrual irregularity or less menses can happen after the pill. Loose motion is not the symptom of pregnancy. But as she has vomiting, you can get urine pregnany test done, which is likely going to be negative. From next month her normal menses will resume.

Hi, I am a 26 weeks pregnant lady, I had TIFFA SCAN. The report is saying that (1) single live fetus about 26 week gestational age with Breech presentation. (2) Single loop of cord around the fetal neck. (3) Mild Left renal pelvis dilatation 6.0 mm. Otherwise Normal late TIFFA. Please let me know, what does it mean? Is there anything to worry.

At 26 weeks pregnany, renal pelvis diameter of 6 mm is normal. Breech means your baby is upside down, which keeps changing throughout pregnancy. So that is also normal. And single loop of cord around neck is present in 20% pregnancies, is also considered as normal. So your report is all normal, there is no need to worry.
