Mam,
Definitely you need surgery for complete tear MCL. Without MCL reconstruction you may damage your knee joint even more. Moreover since medial patellar retinaculum is torn too this may lead to lateral subluxation of patela.
So I would suggest you to immediately consult orthopedic surgeon else more damage may occur.
After that you need intensive Physiotherapy.
