Shaking a little bit may be due. Excessive biting force applied on that area. Any trauma in that area.
Sometimes any gum disease can also cause a little shaking just consult a dentist and know the cause of that shaking. Treatment can be given according to the cause minor shaking can be treated by splinting. And also by doing some bite corrections.

there are no such precaution as coming empty stomach to visit your dentist. Infact we always encourage patient to have food before the have any treatment to avoid hypoglycemia during the treatment.

First of all visit a dentist/ oral surgeon, need to inspect clinically and may require few investigations based on clinical examination.
Till then follow :-
Avoid opening mouth wide
Avoid chewy and hard food stuffs
Try supporting your jaw while yawing and sneezing
MUST visit dentist.

Tmj pain what is the history? Any trauma or Injury to the joint either or due to functional or any other has to be ruled out.
You must take a CT scan preferably in mouth open and closed positions.
Temporarily you may take a NSAID pain killer and it will relieve your inflammation too.
A topical fomentation can be tried.
Have soft diet. Use the other side for chewing. Support the lower jaw with your hand under when yawning.

Yes you should dilute the mouth water with water and rinse because long term use of concentrated mouth wash will cause discolouration of the teeth.
